Bisphosphonate-associated mandibular osteonecrosis.

A 66-year-old man with multiple myeloma treated with zoledronic acid, melphalan, and prednisone after a tooth extraction developed severe osteonecrosis of the mandible that was nonresponsive to antibiotic therapy. A CT scan showed innumerable fragmented sequestra in the mandible and some adjacent soft tissue fullness around the mandible. The association between the use of bisphosphonates and osteonecrosis of the jaw has been described only recently.

Cryofibrinogen levels are increased in non-traumatic osteonecrosis: a new pathogenic clue to osteonecrosis?

OBJECTIVE: To determine whether levels of cryofibrinogen are increased in non-traumatic osteonecrosis (ON) and could correlate with disease staging. METHODS: We prospectively analysed cryofibrinogen levels by immunofixation electrophoresis in 50 patients with non-traumatic ON, 50 healthy volunteers and 8 patients with traumatic ON. Staging of disease involving the femoral heads and the size of necrotic lesions were assessed by the Association Research Circulation Osseous (ARCO) classification system. RESULTS: Mean cryofibrinogen levels in patients with non-traumatic ON were significantly inc…

Etiologic Classification Criteria of ARCO on Femoral Head Osteonecrosis Part 2: Alcohol-Associated Osteonecrosis.

BACKGROUND: Although alcohol is a leading risk factor for osteonecrosis of the femoral head (ONFH) and its prevalence reportedly ranges from 20% to 45%, there are no unified classification criteria for this subpopulation. In 2015, Association Research Circulation Osseous decided to develop classification criteria for alcohol-associated ONFH. METHODS: In June of 2017, Association Research Circulation Osseous formed a task force to conduct a Delphi survey. The task force invited 28 experts in osteonecrosis/bone circulation from 8 countries. Each round of the Delphi survey included questionnaire…

Etiologic Classification Criteria of ARCO on Femoral Head Osteonecrosis Part 1: Glucocorticoid-Associated Osteonecrosis.

BACKGROUND: Glucocorticoid usage, a leading cause of osteonecrosis of the femoral head (ONFH), and its prevalence was reported in 25%-50% of non-traumatic ONFH patients. Nevertheless, there have been no unified criteria to classify glucocorticoid-associated ONFH (GA-ONFH). In 2015, the Association Research Circulation Osseous addressed the issue of developing a classification scheme. METHODS: In June 2017, a task force was set up to conduct a Delphi survey concerning ONFH. The task force invited 28 experts in osteonecrosis/bone circulation from 8 countries. Each round of the Delphi survey con…

Bone scintigraphy screening for osteonecrosis of the shoulder in patients with non-traumatic osteonecrosis of the femoral head.

OBJECTIVE: In patients with non-traumatic osteonecrosis of the femoral head (ONFH), the shoulder is one of the major affected sites secondary to the proximal and distal femur in cases of multiple osteonecrosis. The present study attempted to investigate whether technetium bone scintigraphy is useful for screening of non-traumatic osteonecrosis of the shoulder (ONS). DESIGN AND PATIENTS: A total of 170 shoulder joints in 85 patients with ONFH were evaluated by bone scintigraphy and the findings compared with those of magnetic resonance imaging (MRI). The MR diagnosis was used as the gold stand…

Osteonecrosis of the patella in patients with nontraumatic osteonecrosis of the femoral head: MRI findings in 60 patients.

We examined both knees in 60 patients with nontraumatic osteonecrosis of the femoral head with three-dimensional MRI. We found osteonecrosis of the patella in 9 patients (10 knees) and osteonecrosis of the femoral or tibial condyles/metaphyses in 35 patients (61 knees). In the patella, the necrosis was located in the proximal half and tended to develop in patients with other necrotic lesions in the knee.

Comparison of non-medication-related osteonecrosis and medication-related osteonecrosis of the jaw: a pilot study.

OBJECTIVES: This study aimed to compare composite radiographic index (CRI-index) findings of medication-related osteonecrosis of jaws (MRONJs) and non-medication related osteonecrosis of jaws (n-MRONJs) and predict factors predictive of recurrence and healing delays. MATERIALS AND METHODS: Patients referred to our clinic for stage 2 and 3 MRONJ and n-MRONJ between 2020 and 2023 were included in this study. Microorganisms were isolated in samples of excised bone obtained from the adjacent bone by surgery. CRI-index was evaluated using radiological data. Patient age, sex, causes and localizatio…

Clinical and radiological outcome of the treatment of osteonecrosis of the femoral head using the osteonecrosis intervention implant.

The aim of this study was to evaluate the clinical and radiological outcome of the treatment of osteonecrosis of the femoral head by implantation of an osteonecrosis intervention rod. In this retrospective study the follow-up of 19 patients with 23 osteonecrotic femoral heads treated with implantation of an osteonecrosis intervention implant was assessed. From 19 patients with 23 necrotic femoral heads, there were 13 cases in which a total hip replacement was necessary. This implies a survival rate of 44% after implantation of an osteonecrosis intervention rod after a mean follow-up of 1.45 ye…

[Osteonecrosis and osteochondrosis : Clinical introduction].

BACKGROUND: Osteochondrosis and osteonecrosis affect epiphyseal and apophyseal ossification centres and lead to structural changes in bone and cartilage. They differ in terms of aetiology, age and prognosis. Osteochondrosis dissecans and Osgood-Schlatter disease primarily affect adolescents, while spontaneous osteonecrosis of the knee (SONK/Ahlbäck's disease) predominantly occurs in older adults. OBJECTIVE: This article presents four clinical cases to illustrate typical manifestations, imaging diagnostics, and therapeutic strategies for osteochondronecrosis in children, adolescents and adults…

Knee osteonecrosis after COVID-19.

BACKGROUND: COVID-19 is an emergent infection, the long-term complications of which are still under study. While hypercoagulability is a common feature in severe cases, the incidence of ischemic complications such as osteonecrosis remains unknown. Previous studies on SARS-CoV1 found an increase in osteonecrosis 3-36 months after infection, and it is still unclear if this was related to the use of corticosteroids or to the virus itself. METHODS: We introduce a 78-year-old woman who complained of right knee pain and swelling a month after COVID-19 infection onset. Her knee radiography showed no…

Mesenchymal stromal cells for osteonecrosis.

Osteonecrosis (ON) is an acquired debilitating skeletal disorder, which is caused by a multitude of traumatic and non-traumatic etiological factors. Vascular damage, mechanical stress and increased intraosseous pressure have been discussed as contributors to ON. The optimal treatment of ON remains to be determined, since the current gold standard, core decompression, is insufficiently effective. Specific properties of mesenchymal stromal cells (MSCs) provide the rationale for their assessment in advanced stages of ON: Osteoinductive potential has been demonstrated and MSC preparations of suita…

Mandibular Osteonecrosis Associated With Raloxifene.

Osteonecrosis is a disease with diverse pathophysiology, clinical presentation, and management. It may be associated with some medications used to treat systemic issues with bone metabolism. A few cases of jaw bone osteonecrosis have been associated with raloxifene. In this paper, the authors present a clinical report of a 64-year-old woman who presented with a necrosis foci in the right alveolar ridge of the mandible, associated with continued raloxifene use.

Osteonecrosis in Sickle Cell Disease.

Osteonecrosis is one of the most devastating musculoskeletal manifestations of sickle cell disease and most commonly affects the femoral head. Although the exact pathophysiology of this condition in patients with sickle cell disease is unknown, it is suggested that red cell sickling and repetitive vaso-occlusion may be associated with tissue hypoxia, inflammation, and subsequent bone necrosis and collapse. If left untreated, osteonecrosis can be extremely debilitating and may lead to severe pain, loss of function, and degenerative joint changes. Although several conservative management approac…

Histologic analysis of postmeniscectomy osteonecrosis.

Bone marrow signal changes on magnetic resonance imaging (MRI) after meniscectomy have been reported as evidence of postmeniscectomy osteonecrosis, but this pathology is unclear. We conducted a study to follow-up cases with bone marrow signal changes on MRI after meniscectomy and investigate the pathology of underlying lesions. Of 136 patients with no presurgical evidence of osteonecrosis, 29 had juxta-articular bone marrow signal changes on MRI after arthroscopic meniscectomy and subsequently underwent conservative therapy. In 6 of these 29 patients, clinical symptoms and radiographic changes…

Treatment of bisphosphonates-associated osteonecrosis.

This mini-review will focus on the management of a patient with bisphosphonate-associated osteonecrosis (BON). In order to review the subject the authors report a case of a patient with prostate cancer and metastatic bone disease who was treated with zoledronic acid. Prior to cancer the patient was treated with risedronate for osteopenia but had discontinued this treatment when cancer was diagnosed. During the description of each aspect of the case, a discussion of the rationale used for the case management is presented with support of the available literature. Aspects of interest include the …

Granulomatous osteonecrosis in Crohn's disease.

A 25-year-old white woman was diagnosed with Crohn's disease involving the small and large intestines. She had a complex clinical course that required treatment with multiple pharmacological agents, including intravenous, oral and rectal corticosteroids. She also received parenteral nutrition with lipid emulsions. Finally, repeated intestinal resections and drainage of perianal abscesses were required. Her disease was complicated by gallstones, urolithiasis and hip pain. After osteonecrosis was diagnosed, joint replacements were performed. Review of the pathological sections from the resected …
