Anterior Cervical Huge Osteophyte Causing Dysphagia: A Case Report.

The prevalence of anterior cervical osteophyte among elderly patients is high due to many causes such as trauma, degenerative changes, and diffuse idiopathic skeletal hyperostosis. Severe dysphagia is one of the main presenting symptoms for anterior cervical osteophytes. We describe a case of a patient with anterior cervical osteophyte with severe dysphagia and quadriparesis. The 83-year-old man presented to the emergency department following the incident of falling on his face. CT and X-ray were done in the emergency department, which showed huge anterior osteophytes at the level of C3-4 comp…

Quadrilateral space syndrome induced by a large degenerative osteophyte.

A 41-year-old gentleman presented with decreased range of motion. Initial radiographs demonstrated extensive osteoarthritic changes involving the glenohumeral joint with a large inferior oriented osteophyte. Subsequent MRI of the shoulder was obtained which demonstrated isolated fatty atrophy of the teres minor and, to a lesser extent, deltoid muscles. The axillary nerve was visualized entering the quadrilateral space which, although, was severely narrowed secondary to the large osteophyte. The patient's clinical symptoms and MRI findings were consistent with quadrilateral space syndrome. The …

Controlling Abnormal Joint Movement Inhibits Response of Osteophyte Formation.

Objective Osteoarthritis (OA) is induced by accumulated mechanical stress to joints; however, little has been reported regarding the cause a … ficantly inhibited in the CAJM group as compared with the ACL-T group ( P < 0.001). At 4 weeks, the osteophyte formation had also significantly increased in the ACL-T group ( P < 0.001). These results reflected the microscopic scoring and soft X-ray analysis findings at 4 weeks. Real-time synovial membrane polymerase chain reaction analysis for evaluation of …

Endoscopic Removal of a Scapular Osteophyte Using Scapulothoracic Arthroscopy.

This article describes the removal of a scapular osteophyte from the subscapular space by scapulothoracic arthroscopy. The endoscopic technique allows a gentle approach to the subscapular space without causing a large amount of surgical trauma and therefore leads to good cosmetic and functional results.

Autologous Osteophyte Grafting for Open-Wedge High Tibial Osteotomy.

Osteophytes are physiological bony outgrowths that develop at the margins of the articular surfaces during the progression of osteoarthritis; they are associated with active endochondral bone formation processes and expressions of various growth factors. We believe they could be a source of bone grafts as a result of a potentially strong osteoinductive effect. Moreover, osteophytes can be easily harvested by arthroscopy in patients undergoing open-wedge high tibial osteotomy (OW-HTO) for medial unicompartmental knee osteoarthritis. Therefore, we have been using osteophyte autografts for osteot…

The effect of β-carotene on lumbar osteophyte formation.

STUDY DESIGN: Cross-sectional cohort study of elderly people. OBJECTIVE: The relationships of osteophyte formation on plain lumbar radiographs with serum levels of antioxidants (carotenoids, vitamin A, vitamin E) and other factors were investigated to examine whether antioxidants are involved in lumbar spine degeneration. SUMMARY OF BACKGROUND DATA: Antioxidants have inhibitory effects on the onset of many diseases. However, the association of lumbar osteophyte formation with antioxidant levels in the general population has not been investigated. METHODS: The subjects were 286 people (103 m…

[Acromial osteophyte in shoulder impingement syndrome. Diagnosis and prevalence].

INTRODUCTION: The shoulder impingement syndrome (SIS) is caused by the compression of the rotator cuff tendon tear and the subacromial bursa between the humeral head and the structures that conforms the subacromial arc. OBJECTIVE: To determine a proportion of the acromial spur in the SIS comparing it with the general population. A second one would be to evaluate the quality of the radiological technique. MATERIAL AND METHODS: A comparative cross-sectional study was made in 148 patients, divided in two groups: one conformed by 52 persons, for patients with SIS and the other, conformed by 96,

Synovial lining macrophages mediate osteophyte formation during experimental osteoarthritis.

OBJECTIVE: In human osteoarthritis (OA), various forms of pathology are observed. Besides cartilage damage and fibrosis, neogenesis of bone, osteophyte formation, also occurs. Osteophytes are thought to limit joint movement and cause pain. The objective of this study was to investigate whether synovial macrophages are involved in osteophyte formation in experimental OA, and if they are, to study which mechanism may be involved. DESIGN: Experimental OA was induced by two intra-articular injections of collagenase on alternate days into murine knee joints. The role of synovial lining macrophages…

Dysphagia and dyspnea due to an anterior cervical osteophyte.

Hypertrophic anterior cervical osteophytes have been reported as a cause of dysphagia, with about 100 cases described in the literature; on the other hand, chronic or acute dyspnea due to edema of the laryngeal inlet or bilateral vocal cord adduction-fixation is rare. We report a 57-year-old patient with a 2-year history of dysphagia and episodic dyspnea, who suffered sudden, severe respiratory distress necessitating emergency tracheotomy. A voluminous anterior cervical osteophyte at the C5 level was diagnosed by computed tomography (CT) and barium swallow test and removed by an anterior appro…

Osteophyte of the femoral head after transtrochanteric rotational osteotomy.

The relationship between change in the shape of the femoral head and the clinical results after transtrochanteric rotational osteotomy for osteonecrosis of the femoral head was investigated. Fifty-six hips in 50 patients, followed for at least 2 years, showed no progressive collapse and no osteoarthrotic change during that time. The growth of a superolateral osteophyte of the femoral head was observed postoperatively at a higher rate with progression of the preoperative stage. Excessive restoration of the rotated necrotic lesion of the femoral head was observed at a higher rate in hips with th…

Bone formers: osteophyte and enthesophyte formation are positively associated.

OBJECTIVE: To test the hypothesis that enthesophyte formation and osteophyte growth are positively associated and to look for associations between bone formation at different sites on the skeleton so that a simple measure of bone formation could be derived. METHODS: Visual examination of 337 adult skeletons. All common sites of either enthesophyte or osteophyte formation were inspected by a single observer who graded bone formation at these sites on a 0-3 scale. The total score for each feature was divided by the number of sites examined to derive an enthesophyte and an osteophyte score. Cron…

[Osteophyte formation in the knee joint: a radiological study].

Reported here are the results of a study on the significance of osteophyte formation in the osteoarthritic knee joint. The osteophyte formation was examined in a total of 795 cases involving 1040 joints. Of these, 415 cases (660 joints) had no symptoms of osteoarthritis of the knee joint (Group A); while the other 380 (380 joints) had primary osteoarthritis of the knee joint (Group B). The osteophyte location and osteophyte length were determined in all cases of Group A and Group B. The rate of annual increase in osteophyte length was determined in 224 cases (369 joints) of Group A (average st…

Early osteophyte formation after chemically induced articular cartilage injury.

We examined the early changes that follow injection of sodium iodoacetate (IA) into the guinea pig knee joint, using Safranin-O (SO) to stain the proteoglycan (PG)-rich matrix and the incorporation of Na35 SO4 to determine which cells have maintained, lost, or developed the ability to synthesize significant amounts of PGs. Within 24 hours following injection of IA, articular chondrocytes exhibited a marked loss in ability to synthesize PGs; this inhibition was complete after 2 weeks. Focal loss of SO staining of the interterritorial matrix was noted after 24 hours, progressing to a complete lo

Osteophyte evolution: studies in an experimental partial meniscectomy model.

Osteophytes are considered characteristic manifestations of osteoarthritis (OA) but may be seen independent of structural cartilage erosive lesions. Etiopathogenic mechanisms for osteophyte formation may involve responses to synovial inflammation, stretching of the synovial membrane, general mechanical instability, and vascular metaplasia. Experimentally induced OA following partial menisectomy in rabbits demonstrates active cellular replication at the normal joint periphery. Vascular responses are seen late in parallel with osteophyte formation rather than preceding it. Osteophyte ossificatio…

Diagnosis and treatment of cervical osteophytes causing dysphagia.

Anterior cervical osteophytes (ACO) are common in adults over the age of 65, and symptoms can range from dysphonia, throat pain, and, the focus of this case presentation, dysphagia. Conservative treatment is often advised, but when no relief is exhibited or symptoms contribute to a worsening patient condition, surgical osteophyte resection is often the treatment of choice. We present the case of a 74-year-old man with severe exacerbation of dysphagia that led to an anterior C3-C5 osteophytectomy, with significant improvement.

Classification of Osteophytes Occurring in the Lumbar Intervertebral Foramen.

BACKGROUND: Surgeons have limited knowledge of the lumbar intervertebral foramina. This study aimed to classify osteophytes in the lumbar intervertebral foramen and to determine their pathoanatomical characteristics, discuss their potential biomechanical effects, and contribute to developing surgical methods. METHODS: We conducted a retrospective, non-randomized, single-center study involving 1224 patients. The gender, age, and anatomical location of the osteophytes in the lumbar intervertebral foramina of the patients were recorded. RESULTS: Two hundred and forty-nine (20.34%) patients had …
