Transillumination of the fingers for vascular anomalies: a novel method for evaluating hereditary hemorrhagic telangiectasia.

PURPOSE: We describe a novel approach for detecting vascular abnormalities deep in the digits by means of a handheld illuminator. METHODS: Ten patients with hereditary hemorrhagic telangiectasia were compared with 10 controls for telangiectases in the fingers using a handheld otoscope. RESULTS: This noninvasive transillumination method revealed telangiectases in 9 of 10 patients with hereditary hemorrhagic telangiectasia and none in controls. CONCLUSIONS: Transillumination of fingers with an otoscope identifies telangiectases in patients with hereditary hemorrhagic telangiectasia. Further s

Pediatric tympanostomy tube assessment via deep learning.

PURPOSE: Tympanostomy tube (TT) placement is the most frequently performed ambulatory surgery in children under 15. After the procedure it is recommended that patients follow up regularly for "tube checks" until TT extrusion. Such visits incur direct and indirect costs to families in the form of days off from work, copays, and travel expenses. This pilot study aims to compare the efficacy of tympanic membrane (TM) evaluation by an artificial intelligence algorithm with that of clinical staff for determining presence or absence of a tympanostomy tube within the TM. METHODS: Using a digital oto

[Otoacoustic emissions in hearing screening in children].

BACKGROUND/AIM: Prevention of the consequences of hearing loss can be accomplished by early hearing screening of the cochlear function in newborus, but also with continuous hearing screening in the early childhood. The aim of this study was to investigate the reliability and feasibility of otoacoustic emissions (OAE) in hearing screening in children, evaluate the frequency and type of hearing loss and determine etiological factors of these losses. METHODS: In the study, 133 children, aged 2 to 7.5 years, were tested on both ears with transient evoked otoacoustic emissions (TEOAE) and distorti

External and middle ear effects on infant hearing screening test results.

This study investigated the relationship between external and middle ear factors and hearing screening results by automated auditory brain stem response (ABR) and transient-evoked otoacoustic emissions (EOAEs). The ears of 200 healthy newborns aged 5 to 48 hours underwent screening by ABR and EOAE, followed by otoscopic examination. The pass rates for ABR and EOAE were 91% and 58.5%, respectively. On otoscopic examination, 28% (112/400) ears had occluding vernix obscuring the view of the tympanic membrane. Cleaning of vernix was successfully performed in all but 2 ears that had occluding verni

Potential usefulness of otoscopy in the evaluation of hanging deaths: A report of two cases.

The significance of examination of the tympanic membrane (TM) has been previously reported in cases of strangulation, traumatic asphyxia, and drowning. We report otoscopic findings in two victims of suicidal hanging, including hemotympanum, bleeding from superficial vessels of the TM, perforation of the TM, and gross bulging of the pars-tensa with a bluish hue. These two cases also show petechial hemorrhages in the TM, which is a previously reported finding in cases of hanging. Postmortem otoscopic examination of victims of hanging may provide information useful in the determination of vitalit

Incidence of occlusion due to impacted cerumen among mentally retarded adolescents.

The incidence of occlusion of the external auditory canal due to impacted cerumen was determined for 44 mentally retarded subjects in comparison with 44 nonretarded subjects. Otoscopic data were taken by qualified personnel for both groups and subjected to statistical analysis. Results confirmed that retarded adolescents have a significantly higher incidence of occlusion of the external auditory canal due to impacted cerumen than do nonretarded adolescents. Routine otoscopic examinations should be performed on the former group to ameliorate loss of hearing due to impacted cerumen.

Acoustic impedance measurement in the otological assessment of multiply handicapped children.

This survey investigated the use of acoustic impedance measurement on a population of multiply handicapped children and correlated the results with otoscopy. Thirty-one children were tested with an impedance meter studying tympanometry, tympanic membrane compliance, and acoustic reflex. Otoscopic examination was also scheduled for each child. The results show that 42% of children tested had some kind of middle-ear problem, mostly unrecognized at the time of this survey. There was 100% agreement between impedance and otoscopic findings. Apparently impedance audiometry can be useful for identify

Classification of Ear Imagery Database using Bayesian Optimization based on CNN-LSTM Architecture.

The external and middle ear conditions are diagnosed using a digital otoscope. The clinical diagnosis of ear conditions is suffered from restricted accuracy due to the increased dependency on otolaryngologist expertise, patient complaint, blurring of the otoscopic images, and complexity of lesions definition. There is a high requirement for improved diagnosis algorithms based on otoscopic image processing. This paper presented an ear diagnosis approach based on a convolutional neural network (CNN) as feature extraction and long short-term memory (LSTM) as a classifier algorithm. However, the s

[Pneumatic otoscopy in pediatrics].

Pneumatic otoscopy is based upon the reaction of the tympanic membrane after sending a small air volume. A normal mobility means that air is present in the middle ear. Instead absent or reduced mobility means that effusion is present. This technique improves the quality of ears clinical examination and is particularly useful for the diagnosis of suppurative and serous otitis media.

Effect of middle-ear dysfunction and disease on hearing and language in high-risks infants.

The effect of middle-ear dysfunction and disease on hearing and language development at one year of age was evaluated for 143 high-risk infants. These infants were categorized as normal or abnormal based on otologic history, otoscopic examinations, and on tympanometric examinations. Language was significantly related to gestational age, being delayed by approximately the amount of prematurity. Language scores were therefore adjusted for gestational age. Speech-detection threshold was not related to gestational age, and was used as the measure of hearing. Hearing levels were negatively correlat

[Acoustic distortion products. Clinical values and limitations in the study of low frequencies].

Distortion products are otoemissions produced when the cochlea is stimulated by two pure, continuous sounds f1 and f2. The most ample distortion product in Man corresponds to a frequency of 2f1-f2. The aim of our work is to appreciate the possibilities to study the distortion products for the exploration of low frequencies in Man. Three populations of subjects have been studied: young subjects have been studied: young subjects with a normal hearing, subjects with pathological otoscopic findings in spite of a normal hearing, subjects with pathological otoscopic findings in spite of a normal aud

Evaluation of the canine tympanic membrane by positive contrast ear canalography.

Positive contrast ear canalography was described briefly in 1973 as a method for detecting rupture of the tympanic membrane in dogs with otitis media. The purpose of this study was to evaluate the sensitivity and usefulness of the technique. The ears of 10 normal canine cadavers and 31 dogs with clinical signs of ear disease were examined using otoscopy, radiography and contrast radiography after infusing 2 to 5 ml of positive contrast medium into the ear canals. These examinations were repeated in the cadavers after the tympanic membrane had been punctured with a Spreull needle. In the cadave

Aspects of diagnosis of acute otitis media.

BACKGROUND: Acute otitis media is a common disease, particularly among children. The importance of a correct diagnosis is crucial, especially as unjustified prescription of antibiotics has become a major problem in clinical praxis. OBJECTIVES: Our aim was to evaluate the predictive value of different otological findings in diagnostics and treatment of acute otitis media among GPs and ear specialists and to investigate if the diagnosis could be improved by the use of an ear microscope instead of an otoscope. Furthermore, we aimed to test the value of following an algorithm, METHODS: Thirty-on
