Oxygen delivery and carbon dioxide removal on venovenous extracorporeal membrane oxygenation.

PURPOSE OF REVIEW: Venovenous extracorporeal membrane oxygenation (VV-ECMO) provides gas exchange for patients with advanced respiratory failure who cannot maintain adequate oxygenation or carbon dioxide (CO 2 ) clearance through conventional mechanical ventilation. This review examines clinical applications of VV-ECMO with a focus on optimizing oxygen delivery and CO 2 removal. RECENT FINDINGS: Over the past two decades, VV-ECMO utilization has expanded, now serving as a bridge to recovery in cases of severe hypoxemic and hypercapnic respiratory failure, as procedural support, and as a bridg

Topical oxygen therapy and singlet oxygen in wound healing: A scoping review.

The aim of this scoping review was to provide an overview of current research into topical oxygen therapies including the under-researched singlet oxygen for wound healing. A scoping review was undertaken using five databases. After duplicates and ineligible studies were excluded, 49 studies were included for a narrative review. Out of the included 49 studies, 45 (91.8%) were published in the past 10 years (2013-2023) with 32 (65.3%) published in the past 5 years (2018-2023). Eight of the studies were systematic reviews and/or meta-analysis and 18 were RCTs. The search identified zero human RC

Oxygen Saturation or Tissue Oxygen Determinations on Skin Whose Viability is at Risk.

The triad of ischaemia, neuropathy, and infection are among the principal causes of lower extremity wounds that are commonly prevalent in patients with diabetic foot (DF) a condition in which peripheral arterial disease commonly co-exists. The prevalence of this condition is increasing globally and with it, the mounting costs of its management. One aspect of management is saving limbs and or digits, a crucial part of this process is assessing tissue viability of skin which is a focus of this review: there are other aspects which are well described in the literature. Amputations are offered to

Apnoeic oxygenation with high-flow oxygen for tracheal resection and reconstruction surgery.

BACKGROUND: Tracheal resection and reconstruction are the most effective treatments for tracheal stenosis, but the difficulties are surgery and maintaining ventilation performed on the patient's same airway. High-flow oxygen has begun to be applied to prolong the apnoea time in the tracheal anastomosis period for tracheal resection and reconstruction. This study aims to evaluate the effectiveness of apneic conditions with high-flow oxygen as the sole method of gas exchange during anastomosis construction. METHODS: A prospective study was performed on 16 patients with tracheal stenosis, with a

Effects of changes in inspired oxygen fraction on urinary oxygen tension measurements.

BACKGROUND: Continuous measurement of urinary PO2 (PuO2) is being applied to indirectly monitor renal medullary PO2. However, when applied to critically ill patients with shock, its measurement may be affected by changes in FiO2 and PaO2 and potential associated O2 diffusion between urine and ureteric or bladder tissue. We aimed to investigate PuO2 measurements in septic shock patients with a fiberoptic luminescence optode inserted into the urinary catheter lumen in relation to episodes of FiO2 change. We also evaluated medullary and urinary oxygen tension values in Merino ewes at two differen

Hyperbaric Oxygen Therapy and Oxygen Compatibility of Skin and Wound Care Products.

Objective: Use test methods to assess the oxygen compatibility of various wound care products. Approach: There are currently no standard test methods specifically for evaluating the oxygen compatibility and safety of materials under hyperbaric oxygen (HBO) conditions. However, tests such as the oxygen index (OI), oxygen exposure (OE), and autogenous ignition temperature (AIT) can provide useful information. Results: The OI test measures the minimum oxygen concentration that will support candle-like burning, and it was used to test 44 materials. All but two exhibited an OI equal to or greater (

Neonatal Intermittent Hypoxia, Reactive Oxygen Species, and Oxygen-Induced Retinopathy.

Most of the major morbidities in the preterm newborn are caused by or are associated with oxygen-induced injuries and are aptly called "oxygen radical diseases in neonatology or ORDIN". These include bronchopulmonary dysplasia, retinopathy of prematurity, periventricular leukomalacia, intraventricular hemorrhage, necrotizing enterocolitis and others. Relative hyperoxia immediately after birth, immature antioxidant systems, biomolecular events favoring oxidative stress such as iron availability and the role of hydrogen peroxide as a key molecular mediator of these events are reviewed. Potential

On the pulmonary toxicity of oxygen. 5. Electronic structure and the paramagnetic property of oxygen.

Oxygen uptake by the pulmonary circulation is a chemical reaction. The physicochemical attributes of oxygen are critical when studying pulmonary oxygen toxicity. Extent of lung injury depends on the percentage of oxygen in an oxygen-nitrogen mix in polybaric circumstances (Shanklin, 1969). Further change in extent of lesion follows when other gases are used in the inhalant mix instead of nitrogen (Shanklin and Lester, 1972), with oxygen at 21-100% of the mix. Comparative subatmospheric oxygen levels down to 3% in hydrogen, helium, nitrogen, argon, or sulfur hexafluoride, were run with and with

The impact of arterial oxygen tension on venous oxygen saturation in circulatory failure.

Central and mixed venous oxygen saturations have been used to guide resuscitation in circulatory failure, but the impact of arterial oxygen tension on venous oxygen saturation has not been thoroughly evaluated. This observational study investigated the impact of arterial oxygen tension on venous oxygen saturation in circulatory failure. Twenty critically ill patients with circulatory failure requiring mechanical ventilation and a pulmonary artery catheter in an intensive care unit in a tertiary hospital in Western Australia were recruited. Samples of arterial blood, central venous blood, and m

The influence of arterial oxygenation on cerebral venous oxygen saturation during hyperventilation.

Cerebral venous oxygen desaturation may occur when hyperventilation is employed during neurosurgical procedures. In this study, we examined the effect of arterial hyperoxia (PaO2 > 200 mmHg) on jugular bulb venous oxygen tension (PjvO2), saturation (SjvO2) and content (CjvO2) in 12 patients undergoing anaesthesia for neurosurgical procedures. Under stable anaesthetic conditions, the inspired oxygen fraction (FIO2) was varied to give four different levels of arterial oxygen tension (PaO2 100-200, 201-300, 301-400, and > 400 mmHg), at two levels of controlled hyperventilation (PaCO2(25) and 30 m

Oxygen consumption-oxygen delivery dependency in adult respiratory distress syndrome.

Considerable attention has been directed toward the study of oxygen transport principles in the adult respiratory distress syndrome (ARDS). In particular, concerns have been voiced regarding disturbed tissue oxygen extraction creating a pathologic dependency of oxygen consumption on oxygen delivery. This article discusses oxygen transport principles, data relevant to the oxygen extraction controversy, and implications regarding the pathogenesis and potential therapies for ARDS.

Domiciliary oxygen--by liquid or concentrator? Working Group on Oxygen Therapy of IUATLD.

A few hypoxaemic and even less non-hypoxaemic patients with chronic obstructive pulmonary disease (COPD) benefit from portable oxygen. For these few, selected by double-blind tests against compressed air, liquid oxygen would seem more convenient and efficient. The extra cost of treating all hypoxaemic COPD patients with liquid oxygen by displacing the oxygen concentrator is not justified on the basis of current knowledge. The application of differing oxygen delivery systems to COPD patients requires more evaluation. The selection of patients for long-term oxygen therapy (LTOT) still presents m

Relationship between oxygen uptake and oxygen delivery in patients with pulmonary hypertension.

Dependency of oxygen consumption (VO2) on oxygen delivery (DO2) in patients with adult respiratory distress syndrome and with congestive heart failure has been reported previously. We evaluated this relationship in 11 patients with pulmonary hypertension, 8 with primary pulmonary hypertension (PPH), and 3 with pulmonary hypertension secondary to chronic obstructive lung disease (SPH) at baseline and during treatment with vasodilating calcium channel antagonists. The mean baseline DO2 and VO2 were 11.0 +/- 4.2 and 3.9 +/- 1.1 ml/min/kg, respectively. After increasing cardiac output via vasodila

Brain tissue oxygen tension is more indicative of oxygen diffusion than oxygen delivery and metabolism in patients with traumatic brain injury.

OBJECTIVES: Despite the growing clinical use of brain tissue oxygen monitoring, the specific determinants of low brain tissue oxygen tension (P(bt)O2) following severe traumatic brain injury (TBI) remain poorly defined. The objective of this study was to evaluate whether P(bt)O2 more closely reflects variables related to cerebral oxygen diffusion or reflects cerebral oxygen delivery and metabolism. DESIGN: Prospective observational study. SETTING: Level I trauma center. PATIENTS: Fourteen TBI patients with advanced neuromonitoring underwent an oxygen challenge (increase in FiO2 to 1.0) to a

High flow nasal oxygen vs. conventional oxygen therapy over respiratory oxygenation index after esophagectomy: an observational study.

BACKGROUND: Postoperative pulmonary complications after esophagectomy still represent a matter of concern. High flow nasal cannula (HFNC) early after major abdominal and thoracic surgery has demonstrated some advantages over conventional oxygen therapy. Data about respiratory effect of HFNC after esophagectomy is scarce. The primary aim of this study is to investigate if the early use of HFNC after esophagectomy could enhance patients' postoperative respiratory oxygenation (ROX) index and, ultimately, reduce postoperative pneumonia. METHODS: In this single center retrospective study all patie
