Routine preprocedural ultrasound in palpation versus ultrasound guided radial access for cardiac catheterization.

BACKGROUND: The radial first approach in cardiac catheterization is preferred for its benefits in patient comfort and recovery time. Yet, challenges persist due to characteristics like small, deep, calcified, and mobile radial arteries. Utilizing ultrasound before and during procedures can improve success rates. However, the adoption of its use is still limited and subject to debate. AIM: To utilize routine preprocedural ultrasound (US) and compare US guided with palpation guided radial access, focusing on operator efficiency and outcomes. METHODS AND RESULTS: Consenting adult patients under…

Prediction model of contact forces and IOP during digital palpation of porcine eyes.

Frequent intraocular pressure (IOP) measurements are desirable in the diagnosis and management of glaucoma. Most current tonometers utilize some form of corneal deformation to estimate the IOP, since trans-scleral tonometry suffers from loss of sensitivity. Tran-scleral and trans-palpebral tonometry, however, offer a pathway towards a non-invasive home tonometry. This article presents a mathematical model capturing the relationship between the IOP and the displacements imposed onto the sclera by externally applied forces. Similar to manual digital palpation tonometry, trans-scleral mechanical …

Force-Sensorless Identification and Classification of Tissue Biomechanical Parameters for Robot-Assisted Palpation.

The implementation of robotic systems for minimally invasive surgery and medical procedures is an active topic of research in recent years. One of the most common procedures is the palpation of soft tissues to identify their mechanical characteristics. In particular, it is very useful to identify the tissue's stiffness or equivalently its elasticity coefficient. However, this identification relies on the existence of a force sensor or a tactile sensor mounted at the tip of the robot, as well as on measuring the robot velocity. For some applications it would be desirable to identify the biomech…

Performance of pulse palpation compared to one-lead ECG in atrial fibrillation screening.

BACKGROUND: The 2020 European Society of Cardiology atrial fibrillation guidelines recommend opportunistic screening for atrial fibrillation by pulse taking or ECG rhythm strip in those aged over 65 years. HYPOTHESIS: We aimed to compare the diagnostic accuracy of pulse palpation to ECG rhythm strip when screening for atrial fibrillation. A secondary aim was to investigate whether participants with palpitations were more likely to be diagnosed with new atrial fibrillation. METHODS: The study population were 75/76 year old individuals that participated in the STROKESTOP II study, a Swedish sc…

Resting electromyographic activity of deep thoracic transversospinalis muscles identified as abnormal with palpation.

CONTEXT: In the 1940s, osteopathic researchers suggested that paraspinal tissue abnormality was associated with spontaneous muscle activity, but few studies have since re-examined these reports. OBJECTIVE: To determine whether abnormal motor activity plays a role in deep paraspinal tissues that appear abnormal to palpation. METHODS: Using an observational study design, the PVG of participants with thoracic pain were palpated by two examiners for consensus on the most marked level of tissue abnormality. Dual fine-wire, intramuscular electrodes were inserted into the deep transversospinalis (m…

[Optimization of sentinel lymph node biopsy in breast cancer by intraoperative axillary palpation].

INTRODUCTION: Sentinel node biopsy (SNB) by radioisotopes is a widely accepted and reliable surgical method for staging breast cancer in patients with unknown positive axillary lymph nodes involvement. The main limitation of this method is due to the appearance of false negatives that may be caused by tumor lymph node blockage of the sentinel lymph node and uptake in the neighboring lymph nodes. Infiltered sentinel nodes are generally increased in size and firm. Thus, they can be detected by intraoperative palpation, even when there is no uptake by the radiotracer. AIM: To reduce the false ne…

Spinal palpation for lumbar segmental mobility and pain provocation: an interexaminer reliability study.

OBJECTIVE: This study determined the degree of interexaminer reliability using 2 experienced clinicians performing 3 palpation procedures over the lumbar facet joints and sacroiliac joints. METHODS: The sample consisted of 39 patients with low back pain who had a recent history of low back pain. Two doctors of chiropractic independently examined each of these patients in the prone position with 3 different procedures: (1) springing palpation for pain provocation, (2) springing palpation for segmental mobility testing, and (3) the prone instability test. The doctors were blinded to each other'…

Femoral pulse palpation before and after exercise: a guide to occult iliac stenosis?

Iliac artery stenosis may produce significant haemodynamic impairment in the absence of any visible iliac narrowing on uniplanar arteriography. Such stenoses may be unmasked by the use of biplanar angiography or by direct femoral pressure measurements before and after papaverine. The aim of this study is to determine whether simple femoral pulse palpation is a reliable guide to haemodynamic impairment in the radiologically normal aorto-iliac segment, and in particular whether occult iliac stenosis can be unmasked by noting a reduction in the palpable pulse after exercise. Twenty-nine limbs wit…

A model to estimate spleen volume from palpated spleen length in patients with myelofibrosis.

Spleen volume is commonly assessed in clinical trials involving patients with myelofibrosis (MF), but direct measurement requires radiologic imaging. In routine clinical settings, spleen length measured by palpation is frequently used as an alternative measure. As the relationship between spleen length and spleen volume is not strictly proportional, interpretation of palpation-based measurements may be variable. In this post hoc analysis, we examined the relationship between palpable spleen length and spleen volume in patients with MF using pooled data from two phase 3 clinical studies. Palpab…

Comparison of the outcomes of dynamic/static tests and palpation tests in TMD-pain patients.

UNLABELLED: In addition to palpation tests, dynamic/static tests have been proposed to complement temporomandibular disorders (TMD) patients' evaluation in the clinical setting. The aim of this study was to assess the intra- and inter-observer reliability of the palpation tests and of the dynamic/static tests, and to determine whether those tests would yield comparable outcomes in terms of pain diagnoses. Ninety-eight (N = 98) consecutive adult patients were examined during 2 clinical sessions by 2 independent examiners, based on muscle and joint palpation techniques described in the Research …

Reliability of palpation of the radial artery compared with auscultation of the brachial artery in measuring SBP.

BACKGROUND: Systolic blood pressure contributes more to cardiovascular disease than DBP, especially in elderly persons. Palpation of the radial artery to assess SBP - Riva-Rocci's technique - may be an attractive alternative for auscultatory SBP in these patients. Therefore, we investigated the difference between SBP determined by palpation of the radial artery (pSBP) and SBP assessed by auscultation of the brachial artery (aSBP). METHODS: Patients were included from the waiting room of a hypertension outpatient clinic. In each patient eight simultaneous pSBP and aSBP measurements were assess…

Accuracy and reliability of palpation and percussion for detecting hepatomegaly: a rural hospital-based study.

BACKGROUND: Palpation and percussion are standard bedside techniques used to diagnose hepatomegaly. Ultrasonography is a noninvasive and accurate method for measurement of liver size, but many patients in developing countries have limited access to it. We compared the accuracy of palpation and percussion in a rural population in central India, using ultrasonography as a reference standard. METHODS: The study design was a blinded, cross-sectional analysis of a hospital-based case series. Three physicians, blind to clinical data and to each other's results, independently used palpation and perc…

[Value of palpation and mammography in primary breast cancer. A retrospective study 1973-1982].

The value of palpation findings and mammography in 596 patients with primary breast cancer was investigated. The most frequent symptom (86.1%) was the lump. The primary tentative diagnosis came from the patient in 63.4% of the cases, from the physician in 28.9% of the cases, and only after mammography in 7.7% of the cases. False-negative palpation findings and false-negative mammography findings were observed in 9.9% and 5.0% of cases respectively. There was a clear dependence on the tumor stage. Carcinomas that were first diagnosed by means of mammography manifested a five-year survival rate …

Comparative Investigation of Laryngeal Palpation Scale Between Primary and Secondary Muscle Tension Dysphonia.

OBJECTIVES: Muscle tension dysphonia (MTD) is a condition that is often assessed through palpation examination. The laryngeal palpation scale (LPS) is a reliable and valid tool developed to evaluate patients with MTD. MTD is classified into primary (without tissue reactions) and secondary (with tissue reactions). Primary MTD is caused by muscle hyperfunction, while maladaptive compensation typically leads to secondary MTD. The objective of this study was to compare the LPS scores between primary and secondary MTD. METHODS: This study is a cross-sectional analytical comparative study. It inclu…
