Cutaneous and mucosal mucormycosis mimicking pancreatic panniculitis and gouty panniculitis.

BACKGROUND: Histopathologic study of lesions of cutaneous mucormycosis usually shows suppurative granulomas involving the deep dermis and subcutaneous fat. Large, broad and non-septate fungal hyphae are easily identified within the necrotic areas. OBJECTIVE: The main goal of our study is to describe the histopathologic features of 13 cases of cutaneo-mucous mucormycosis, which mimicked the findings of pancreatic and/or gouty panniculitis and discuss the histopathologic differential diagnosis among these 3 disorders. METHODS: Histopathologic examination of sections stained with hematoxylin-eo…

Panniculitis of pancreatic disease masquerading as systemic lupus erythematosus panniculitis.

We describe a patient with systemic lupus erythematosus (SLE) who presented with severe refractory pannicular lesions diffusely involving the buttocks and lower extremities. Due to the severity of these lesions, a biopsy was performed, which implicated panniculitis associated with pancreatic disease, rather than lupus panniculitis. Serum amylase was normal, but the serum lipase was markedly elevated. An abdominal computerized tomographic scan demonstrated a pancreatic mass, which upon laparotomy was found to be an acinar cell carcinoma. After resection of the mass, her symptoms improved, where…

Panniculitis in Children.

Panniculitides form a heterogenous group of inflammatory diseases that involve the subcutaneous adipose tissue. These disorders are rare in children and have many aetiologies. As in adults, the panniculitis can be the primary process in a systemic disorder or a secondary process that results from infection, trauma or exposure to medication. Some types of panniculitis are seen more commonly or exclusively in children, and several new entities have been described in recent years. Most types of panniculitis have the same clinical presentation (regardless of the aetiology), with tender, erythemato…

Panniculitis: A summary.

The diagnosis of panniculitis is felt to be a confusing topic by some pathologists. This summarical article presents inflammatory diseases of the subcutis in a systematic fashion, based on whether they are centered on fibrovascular septa or the adipose lobules, and whether morphologic vasculitis is present or not. Septocentric, non-vasculitis disorders include erythema nodosum, panniculitis that follows the use of "biological" therapeutic agents, lipodermatosclerosis, post-irradiation panniculitis, morphea profunda, and necrobiosis lipodica profunda. Polyarteritis nodosa and Behçet's disease a…

Panniculitis in children.

This paper will give a comprehensive view of the most frequent panniculitides seen in childhood, with emphasis on the types exclusively found in infancy, and for all other types of panniculitides also found in adults. Aim of this paper is also to analyze the clinical differences between panniculitis in childhood and in adulthood, and to give reliable histopathologic criteria for a specific diagnosis. A review of the literature is here integrated by authors' personal contribution. Panniculitides in children is a heterogeneous group of diseases, as well as in adult life, characterized by inflamm…

Panniculitis in childhood.

Panniculitis refers to disorders with inflammation of the subcutaneous fat. Such inflammation can be primary or can be a reaction pattern induced by a systemic process. Some types of panniculitis are seen more commonly or exclusively in children. These include erythema nodosum, subcutaneous fat necrosis of the newborn, sclerema neonatorum, poststeroid panniculitis, and cold panniculitis. The most typical clinical finding is tender, erythematous subcutaneous nodules. Clinical clues can aid in the diagnosis of the panniculitides, but pathology is often necessary to confirm the diagnosis. In gene…

Panniculitis in children.

The panniculitides include a group of disorders of varied etiology that manifest as inflamed nodules in the subcutaneous tissue. They are rarely seen in infants and children. The panniculitides of the newborn represent a unique response of the infant's fat to different injuries, and are a specific type of panniculitis that is only seen in neonates and very young infants. These specific panniculitides of children include subcutaneous fat of the newborn, poststeroid panniculitis, sclerema neonatorum, and cold panniculitis. This article reviews in detail the specific types of panniculitis of the …

Update on panniculitis.

There is considerable confusion regarding the pathogenesis, nosology and treatment of panniculitis. This paper examines newer concepts in five types of panniculitis: i.e., histiocytic cytophagic panniculitis, erythema induratum, lipodermatosclerosis, pancreatic panniculitis and alpha-1-antitrypsin deficiency panniculitis. Recent developments in etiology, pathogenesis, molecular techniques, and therapy are discussed.

Lupus erythematosus panniculitis.

Lupus erythematosus panniculitis is an uncommon variant of lupus erythematosus characterized by a specific involvement of the subcutaneous fat. It is a panniculitis with peculiar clinical features and histopathologically characterized by a mostly lobular panniculitis. It may appear in patients with discoid lupus erythematosus and systemic lupus erythematosus, but also as the unique manifestation of lupus erythematosus, and in the latter cases the diagnosis may be problematic. Histopathologic differential diagnosis with subcutaneous panniculitis-like T-cell lymphoma may also be extremely diffic…

Sclerosing postirradiation panniculitis.

Sclerosing postirradiation panniculitis is an unusual variant of panniculitis that appears as a complication of megavoltage radiotherapy. Most patient are women with a history of breast cancer who develop nodular lesions on the anterior chest skin several months after receiving megavoltage therapy for breast carcinoma. Sclerosing postirradiation panniculitis is an unusual cutaneous complication of megavoltage radiotherapy that should be distinguished from subcutaneous metastatic disease, cellulitis, and connective tissue diseases involving the subcutaneous fat. The differential diagnosis can b…

Lupus erythematosus panniculitis.

Lupus erythematosus panniculitis is a quite rare clinical entity characterised by one or several firm, asymptomatic, often fairly large subcutaneous nodules, as a manifestation of SLE or DLE. Two cases of lupus panniculitis, both female, are described here. Both of them had absence of typical lesions of SLE or DLE elsewhere in the body. Systemic symptoms were not present, antinuclear factor was positive in one case. Both showed typical lobular panniculitis on biospy and responded favourably to systemic chloroquine therapy.
