BACKGROUND AND AIM OF THE STUDY: Fibroblast growth factor-2 (FGF-2) is a multifunctional protein which plays a role in smooth muscle cell growth, wound healing, tissue repair and angiogenesis. FGF-2 is also released by mechanically wounded cells. Herein, the importance of FGF-2 release from periannular tissue in the mechanism of pannus formation in obstructed mechanical prostheses was investigated.
METHODS: Between January 1993 and December 2002, 35 patients with an obstructed bileaflet prosthetic mitral valve were classified according to the nature of obstruction as either thrombus or pannus…

A case of mitral stenosis following mitral valve replacement in a patient with endocardial fibroelastosis is reported. A 14-year-old boy pre … al fibroelastosis with severe inflow obstruction due to the formation of a complete fibrous ring of pannus/fibrosis around the prosthetic margin on the ventricular aspect of the left ventricle. This complication has not previously been described in children after mitral valve replacement.

We experienced a case of surgical aortic valve re-replacement due to structural valve deterioration caused by pannus formation 4 years after transcatheter aortic valve replacement( TAVR). The patient underwent surgery because the mean transvalvular pressure gradient increased to 48 mmHg on echocardiography. Contrast-enhanced computed tomography (CT) was useful for predicting the site of adhesion to surrounding tissue preoperatively and exploring the presence of the pannus. Intraoperative findings showed the TAVR valve was covered with neointima except around the origins of the left and right c…

PURPOSE: The prevalence of trachomatous inflammation-follicular (TF) in Papua New Guinea (PNG) suggests antibiotic mass drug administration (MDA) is needed to eliminate trachoma as a public health problem but the burden of trichiasis is low. As a result, WHO issued bespoke recommendations for the region. If ≥ 20% of 10-14-year-olds have both any conjunctival scarring (C1 or C2 or C3) and corneal pannus and/or Herbert's pits, MDA should be continued. Equally, if ≥ 5% of that group have both moderate/severe conjunctival scarring (C2 or C3) and corneal pannus and/or Herbert's pits, MDA should be …

BACKGROUND AND AIM OF THE STUDY: The authors' clinical experience is presented of non-structural valvular dysfunction of the prosthetic aortic valve caused by pannus ingrowth during the late postoperative period after previous heart valve surgery.
METHODS: Between January 1999 and April 2012, at the authors' institution, a total of 33 patients underwent reoperation for increased mean pressure gradient of the prosthetic aortic valve. All patients were shown to have pannus ingrowth. The mean interval from the previous operation was 16.7 +/- 4.3 years, and the most common etiology for the previo…

Rheumatoid arthritis (RA) is a debilitating inflammatory condition characterised by joint damage that affects the cervical spine most commonly at the atlantoaxial joint resulting in neck pain and myelopathy. The pathogenesis of RA involves the formation of a hyperplastic synovial tissue, termed pannus, which invades the local bone and causes osseous erosion. Here, we describe a case of rapid onset quadriparesis due to spinal cord compression at C5-C6 secondary to vertebral subluxation and mass effect from a large inflammatory pannus in the subaxial spine. Surgical decompression and resection o…

BACKGROUND: The clinical significance of pannus in the prosthetic mitral valve (MV) is not well documented.
OBJECTIVES: To investigate the clinical significance of pannus on cardiac computed tomography (CT) in patients with a prosthetic MV.
METHODS: A total of 130 patients with previous MV replacement who underwent cardiac CT were retrospectively included in this study. The presence of pannus, paravalvular leak (PVL) around the prosthetic MV and limitation of motion (LOM) of the MV were analyzed using CT. Between patients with MV pannus and those without pannus, CT, echocardiographic, and re…

We studied an enucleated postmortem eye from an 82-year-old white donor who had been implanted with a Pannu "universal" intraocular lens (IOL) in the anterior chamber approximately 20 years earlier. This IOL has design features characteristic of a 1-piece, C-loop posterior chamber IOL. Magnetic resonance imaging showed a relatively well-centered IOL in the anterior chamber with haptics impinging on the iris. Gross and light microscopic analyses of the eye and the IOL showed peripheral anterior synechiae enclaving one haptic, areas of angle widening, significant attenuation of the corneal endot

The atlantoaxial subluxation and the formation of a synovial periodontoid pannus are associated with rheumatoid arthritis causing mechanical compression of the spinal cord and cervical myelopathy. Atlantoaxial subluxation is very rare in psoriatic arthritis (PsA). Even more rare is the formation of a periodontoid synovial pannus associated with PsA and signs of myelopathy. In this report, cervical myelopathy caused by periodontoid synovial pannus in PsA is described.

The significance of immune responses is examined in relation to clinicopathological changes in the rheumatoid joint and the concept of a dual site of action is developed. In the joint cavity interaction between immune complexes and complement is central to the phenomena observed, which depend on the generation of chemotactic factors and the ingress of polymorphs. Other chemical mediators--e.g. prostaglandins and lymphokines--are envisaged as playing a secondary or augmentory role. At the pannus-cartilage (and bone) junction the possibility is considered that changes are due to activation of a …

In this article, we report the case of a 58-year-old man who was admitted to our institution with acute dyspnea and poor hemodynamic condition. He had undergone double mitral-aortic valve replacement five years earlier for rheumatic heart disease. Transthoracic echocardiography (TTE), cinefluoroscopy, and transesophageal echocardiography (TEE) revealed a significant reduction in the opening of the mechanical mitral valve leaflets. An urgent redo surgery was performed, during which the pannus was removed, ensuring the restoration of normal leaflet motion. This clinical case presents two rare ci…

Rheumatoid arthritis (RA) presents a significant challenge among autoimmune diseases, affecting millions globally with diverse clinical manifestations, primarily targeting joints and inducing inflammation, pain, and deformities. In some cases, it leads to the formation of Bakers cysts, fluid-filled bursae in the popliteal region, resulting from chronic inflammation of the synovial membrane and subsequent pannus growth. Our case, a young female patient with RA, presenting with calf mass, highlights this complex interplay. She presented with large swelling and pain in her left calf region which …

BACKGROUND: Acute dysfunction of mechanical aortic valve prostheses is a life-threatening adverse event. Pannus overgrowth, which is fibroelastic hyperplasia originating from the periannular area, is one cause of dysfunction. The aim of this study was to determine the annual incidence of readmittance resulting from acute obstruction caused by pannus during 30 years of observation in patients with Medtronic-Hall aortic valve prostheses and to analyze the risk factors associated with pannus development.
METHODS: From 1982 to 2004, 1,187 patients in our department underwent aortic valve replacem…

BACKGROUND: Coronary obstruction after transcatheter aortic valve implantation (TAVI) is a rare complication typically occurring acutely. Delayed coronary obstruction (DCO) due to early pannus formation is exceptionally uncommon.
CASE SUMMARY: A 74-year-old woman with no coronary artery disease underwent TAVI with a self-expandable 26-mm Evolut Pro + (Medtronic) transcatheter heart valve (THV) six months prior. She presented with recurrent non-ST-segment elevation myocardial infarctions (NSTEMI), despite no angiographic evidence of acute coronary lesions. Multimodality imaging revealed early …
