Triple therapy for Helicobacter pylori eradication: a comparison of pantoprazole once versus twice daily.

BACKGROUND: Proton pump inhibitor-based triple therapy is recommended as treatment for Helicobacter pylori eradication. The proton pump inhibitor may be given once or twice daily. However, little information is available on how these two treatment strategies compare. METHODS: H. pylori-positive patients (two positive test results) with endoscopy-proven healed duodenal ulcer or non-ulcer dyspesia were randomly allocated to 1 week of double-blind treatment with pantoprazole 40 mg once or twice daily, plus clarithromycin 250 mg and metronidazole 400 mg twice daily. Eradication was defined as a n…

pH-Sensitive acrylic acid grafted Abelmoschus esculentus composite for sustained release of pantoprazole sodium.

The research presents a pH-sensitive acrylic acid-grafted hydrogel (graft composite) derived from Abelmoschus esculentus mucilage (AEM) to improve therapeutic outcomes in Crohn's disease and ulcerative colitis. The FT-IR spectroscopy, scanning electron microscopy (SEM), and X-ray diffraction (XRD) confirm successful grafting with a porous structure favorable for drug loading. Drug release studies showed slower PAS release under acidic conditions and faster release under alkaline conditions, mimicking the pH variations of the gastrointestinal tract. Release kinetics confirmed the Korsmeyer-Pepp

A Very Rare Side Effect of a Very Commonly Used Drug: Pantoprazole-induced Seizure.

Pantoprazole is an extensively used proton pump inhibitor (PPI) for acid peptic disease. PPI rarely cause hypomagnesemia. Hypomagnesemia is commonly associated with hypokalemia and hypocalcemia. Severe hypomagnesemia and hypocalcemia can cause seizures. Here, we report a patient on long-term pantoprazole who presented with generalized tonic-clonic seizures and had severe hypomagnesemia, hypocalcemia, hypokalemia, and secondary hyperparathyroidism. When patients on long-term PPI present with seizures, hypomagnesemia/hypocalcemia has to be excluded.

Fabrication of pantoprazole-loaded microsphere clusters with enteric and mucoadhesive properties for enhanced ulcer targeting.

The present study aimed to formulate and evaluate, both in vitro and in vivo, enteric-coated mucoadhesive microspheres of pantoprazole using xanthan gum and Eudragit L100-55 in order to protect pantoprazole from gastric acid degradation and to create a temporary drug reservoir in the gastrointestinal tract, thereby achieving enhanced targeting. The goal was to assess their potential in preventing ketorolac-induced gastric ulcers in rats. Various formulations of pantoprazole microspheres were prepared and developed via an extrusion method using ionotropic gelation and dip coating. The formulati…

Evaluation of a pantoprazole and 4-desmethylpantoprazole-sulfate metabolic ratio as a novel CYP2C19 phenotyping method.

AIMS: CYP2C19 is one of the most important pharmacogenes, and its activity is highly variable due to factors such as genetics or drug-drug interactions. Due to the lack of an appropriate endogenous CYP2C19 biomarker, surrogate methods to assess its activity are warranted. The aim of this work was to validate pantoprazole as a CYP2C19 phenotyping drug. METHODS: We performed a single-dose pharmacokinetic study with 20 healthy participants in a randomized cross-over design. Participants received 10 mg omeprazole and 20 mg pantoprazole on two different study days with a washout phase of one week.…

A green three-ratio manipulating spectrophotometric approaches for the determination of a binary mixture of pantoprazole and domperidone.

BACKGROUND: Pantoprazole (PAN) is a proton pump inhibitor used to treat GERD and hyperacidity by suppressing gastric acid secretion, effectively relieving symptoms such as heartburn, acid regurgitation, and indigestion. Domperidone (DOM) is a prokinetic agent that enhances gastrointestinal motility, helping to alleviate nausea, vomiting, and bloating caused by motility disorders. Their combination (Pantosec-D) provides rapid and comprehensive relief from both acid-related and motility-related symptoms, significantly improving patient comfort and quality of life. OBJECTIVE: This study aims to …

The Effects of Pantoprazole on Kidney Outcomes: Post Hoc Observational Analysis from the COMPASS Trial.

KEY POINTS: In this post hoc analysis of a randomized controlled trial, the proton pump inhibitor pantoprazole led to a faster rate of eGFR decline as compared with placebo. Additional studies are needed to determine the effect of proton pump inhibitors on those at higher risk of adverse kidney outcomes. BACKGROUND: Observational studies have found an association between proton pump inhibitor use and worsening kidney function. It is unclear whether these associations are causal. We conducted post hoc analyses to determine the effect of pantoprazole on kidney function using data from the Cardi…

Effects of the Proton Pump Inhibitors Omeprazole and Pantoprazole on the Cytochrome P450-Mediated Metabolism of Venlafaxine.

UNLABELLED: BACKGROUND AND OBJECTIVE: An increasing trend in prescribing proton pump inhibitors (PPIs) inevitably increases the risk of unwanted drug-drug interactions (DDIs). The aim of this study was to uncover pharmacokinetic interactions between two PPIs-omeprazole and pantoprazole-and venlafaxine. METHODS: A therapeutic drug monitoring database contained plasma concentrations of venlafaxine and its active metabolite O-desmethylvenlafaxine. We considered three groups: a group of patients who received venlafaxine without confounding medications (non-PPI group, n = 906); a group of patients…
