Arthroscopic inspection after the surgical treatment of patella fractures.

After the treatment of patella fractures the only way to evaluate healing at the articular surface before implant removal is through arthroscopy. The purpose of this study was to examine the healing potential of the cartilage. Arthroscopy was performed in 18 patients at the time of implant removal. The mean age of the patients was 42.1 years. The time elapsed from the index surgery to the arthroscopy and implant removal surgery was 12.9 months. During the arthroscopy, we inspected articular step-off, cartilage loss, and joint surface irregularities. Cartilage irregularities were observed in 13…

Arthroscopic-assisted reduction and stabilization of transverse fractures of the patella.

Most patellar fractures are transverse involving the central third. Open reduction and stabilization of transverse patellar fractures is indicated if there is more than 2-3 mm of fragment separation and/or articular incongruity. Arthroscopic-assisted reduction and stabilization of displaced fractures of the patella had been reported as an alternative to open technique. The aim of this study was to report the results of arthroscopic-assisted reduction and fixation of displaced transverse fractures of the patella. Fourteen patients with 14 displaced transverse fractures of the patella were inclu

Arthroscopic-assisted surgical technique for treating patella fractures.

The purpose of this study was to describe and report preliminary results of an arthroscopic-assisted surgical technique for the treatment of displaced transverse patella fractures with percutaneous internal fixation. We applied this technique in 5 patients who presented a displaced transverse patella fracture and were followed-up for an average of 18 months. Under an image-intensifier fluoroscope, the fracture was closely reduced with a percutaneously reduction clamp while the congruence of the articular surface was monitored arthroscopically. Two Kirschner wires were inserted perpendicular to…

Open fractures of the patella: long-term functional outcome.

Seventy-nine open patella fractures in 76 patients were treated between 1986 through 1994, with an 80% incidence of multiple injuries. All were treated with irrigation and debridement, open reduction, internal fixation, and reconstruction of the extensor mechanism. In no case was a primary patellectomy performed, even with severe comminution. There were three failures of initial fixation and one asymptomatic nonunion. Average range of motion for all groups was 112 degrees, at an average follow-up of 21 months. Secondary surgical procedures were performed in 65% of knees, the majority for sympt…

Early development of patella infera after knee fractures.

The condition referred to as patella infera is characterized by a permanent shortening of the patellar ligament and is associated with a severely limited range of motion of the knee joint. Patella infera is a common complication of injury or surgery to the knee joint, and it is generally considered a condition that presents after a variable, albeit considerable time after injury. A review of the knee roentgenograms of patients with arthrofibrosis and patella infera showed this condition to be an immediate complication of knee fractures. Therefore, in order to better define the development of p

Incidence, mechanism of injury, and treatment of fractures of the patella in children.

Fractures of the patella in skeletally immature patients are rare. The charts of 185 patients treated for patella fractures at the University of Kentucky Medical Center between 1976 and 1988 were retrospectively reviewed. The 12 patients of these 185 aged 8 to 16 years were included in this study. The incidence was calculated to be 6.5% of all patella fractures. All patients studied were male with an average age of 12.7 years. Sleeve fractures were the most common type of patella fracture observed (five), followed by transverse fractures (four). Ten of the 12 cases required operative managemen…

Chondral and osteochondral fractures after luxation of the patella and their treatment.

The traumatic luxation of the patella is the most frequent cause of osteochondral or chondral fragments in the knee joint. A precise case history most often provides the necessary reference concerning the occurrence of a luxation. The hemarthrosis and the painful reduction of the mobility after a closed spontaneous reposition hint towards the possibility of a chondral or osteochondral fracture requiring an emergency arthrotomy. If large chondral or osteochondral fragments are found intraoperatively, then a refixation of these fragments should be attempted. The following methods can be used: --

Outcomes of Variable-Angle Locking Anterior Patella Plating for the Treatment of OTA 34-C3 Patella Fractures.

The gold standard for patella fracture fixation is tension band wiring; however, achieving stable anatomic fixation can be challenging in comminuted patterns. The "star" variable-angle locking patella plate is an alternative fixation construct that is meant to address these limitations by providing multiple fixation points and dorsal cortical stability. The purpose of this study was to analyze the outcomes of patients treated with the variable-angle locking "star" patella plate. A total of 358 patients who underwent repair of a displaced patella fracture over a 10-year period were reviewed. Pa…

Management of patella fractures associated with central third bone-patella tendon-bone autograft ACL reconstructions.

Patella fractures are an acknowledged complication of cruciate ligament reconstructions that use bone-tendon-bone autografts. Two fracture patterns are seen: a nondisplaced longitudinal patellar fissure fracture recognized interoperatively and the classic displaced transverse fracture that disrupts the extensor mechanism. Rigid fixation techniques are described for each fracture type that minimally affect rehabilitative efforts.

Revision Patella Tendon Repair With Hamstring Tendon Autograft Augmentation Following Failed Inferior Pole Patella Fracture Open Reduction and Internal Fixation.

A 46-year-old man presents with left knee bucking 3 months after open reduction and internal fixation of a left knee inferior pole patella fracture. Radiograph and magnetic resonance imaging evaluation reveal displacement and comminution of the inferior pole of the patella confirming failure of the repair. The purpose of this video is to demonstrate the surgical management of failed inferior pole patella fractures using suture anchors for repair of the patella tendon and augmentation with ipsilateral autologous hamstring grafts.

Open lateral femoral condyle fracture concomitant with a vertical patella fracture in an adolescent: A case report.

We present the rare case of an open femoral condyle fracture (Gustilla IIIA) combined with a vertical patella fracture due to sharp injury after a traffic collision in a 17-year-old female. During subsequent operation, a locking compression plate was used for definitive fixation of the distal femur and the patella was reduced and fixated with two screws. Four months after surgery, the patient had practically full range of motion again and no secondary complications occurred. On control X-ray, there was consolidation of the fracture.

Surgical Outcomes Following Patella Fracture Repair: A Single-Center Retrospective Cohort Study.

Introduction Patella fractures are an uncommon yet complex injury, accounting for approximately 1% of presentations to orthopedic departments. Surgical management is indicated for unstable, displaced, or comminuted fracture patterns, with the aim of restoring the extensor mechanism and preserving knee function. Various surgical techniques are available, each with differing complication profiles. This study evaluated surgical outcomes following patella fractures over a 15-year period within a single-center orthopedic department. Methods A retrospective cohort study was conducted. A single-cente…

Open Reduction and Internal Fixation of Patella Fracture With Contoured Dorsal Plating.

In this article, we present a novel patella fixation technique using a low-profile highly contoured dorsal mini-fragment locking plate. This procedure is ideally indicated in patients with fracture patterns that are transverse without significant comminution. Long-term clinical studies are being performed to evaluate the effectiveness of this surgical technique.

Patella fracture fixation with suture and wire: you reap what you sew.

INTRODUCTION: Operative fixation of displaced inferior pole patella fractures has now become the standard of care. This study aims to quantify clinical, radiographic and functional outcomes, as well as identify complications in a cohort of patients treated with non-absorbable braided suture fixation for inferior pole patellar fractures. These patients were then compared to a control group of patients treated for mid-pole fractures with K-wires or cannulated screws with tension band wiring. METHODS: In this IRB approved study, we identified a cohort of patients who were diagnosed and treated s…

Atraumatic Bilateral Patella Fracture in Middle-Aged Female: A Rare Case Report.

The patella is the largest sesamoid bone in the body and an important structure of the extensor apparatus which is under undue stress during flexion and extension of the knee. Bilateral fracture of the patella without trauma is a very rare event and may be multifactorial without a single cause. A repetitive stress reaction in a previously predisposed bone can be an important cause. We report a case of a 45-year-old female with a nontraumatic bilateral transverse patella fracture with loss of extensor mechanism. A stable surgical fixation for such a displaced fracture with a good rehabilitation…

Non-operative management of a patella fracture: environmental considerations in the Subantarctic.

Injuries occurring in the remote environment present particular challenges to healthcare professionals, and decisions need to be carefully made on an individual basis. This report describes the successful management of a patella fracture sustained on the Subantarctic island of South Georgia. A 36-year-old boating officer presented to the island surgery after sustaining an isolated closed injury to his left knee. On physical examination there was a large effusion palpable within the joint, and on subsequent radiography an undisplaced transverse fracture of the patella was apparent. The patient …
