Acute cardiogenic pulmonary oedema: reflecting on the management of an intensive care unit patient.

AIM: The aim of this paper is to reflect upon the management interventions of non-invasive ventilation (NIV) and diuretic therapy that were implemented for a patient admitted to an intensive care unit (ICU) with acute cardiogenic pulmonary oedema. BACKGROUND: Acute pulmonary oedema is a serious and life-threatening complication of acute heart failure, particularly if this results from an ischaemic event. Statistics highlight that of those patients treated for acute cardiogenic pulmonary oedema, approximately only one third were alive after 1 year. Many of these patients will require intensive

Critical care management of the patient with HIV infection who has Pneumocystis carinii pneumonia.

Pneumocystis carinii pneumonia (PCP) is the most frequently occurring opportunistic infection in individuals infected with the human immunodeficiency virus. Improved methods of diagnosing and treating PCP have resulted in increased survival rates. Nurses are more frequently faced with treatment of the critical care patient with PCP. Knowledge about the mechanisms and manifestations of PCP as well as its diagnosis and treatment provides a baseline for the nursing management of PCP. Nursing care for the critically ill adult patient with PCP focuses on the management of the human responses to PCP

Case management of a patient with botulism by an intensive care nurse practitioner team.

INTRODUCTION: People affected by conditions such as botulism often require complex case management, a core activity of nurse practitioners, in intensive care. This case report, coauthored by the patient, describes exemplary nurse practitioner-led complex case management including weaning from mechanical ventilation of a person affected by botulism. METHODS: The case management was person centred and included goal setting and collaborative multidisciplinary team meetings including the patient. Goals were set by the patient in consultation with the nurse practitioner and included timelines for

Patient-centered Care: Value Added by Breast Radiologists in the Management of Breast Pain.

With the shift of the healthcare system toward patient-centered, value-based care, the role of the breast radiologist is essential and increasingly multifaceted. Beyond sole image interpreters, breast radiologists serve as subject matter experts within multidisciplinary care teams, acting as advocates and initiators into the healthcare system and providing patient-centered care through effective communication and patient education. These vital roles are well demonstrated through the imaging evaluation and management of the most common breast symptom that affects the majority of the patient pop

Patient blood management in oncology in the Russian Federation: Resolution to improve oncology care.

There is accumulating evidence that anemia and iron deficiency, thrombocytopenia, blood loss and coagulopathy are independent risk factors for adverse patient outcomes in oncology and other settings. Patient blood management (PBM) aims to address these factors by managing and preserving a patient's blood. PBM improves patient health, but also reduces resource utilization, including use of allogeneic blood components, which is another risk factor for adverse outcomes. Supported by the World Health Organization and endorsed in WHA63.12, PBM is recommended by an increasing number of health author

The myasthenic patient in crisis: an update of the management in Neurointensive Care Unit.

Myasthenia gravis (MG) is an autoimmune disorder affecting neuromuscular transmission leading to generalized or localized muscle weakness due most frequently to the presence of autoantibodies against acetylcholine receptors in the postsynaptic motor end-plate. Myasthenic crisis (MC) is a complication of MG characterized by worsening muscle weakness, resulting in respiratory failure that requires intubation and mechanical ventilation. It also includes postsurgical patients, in whom exacerbation of muscle weakness from MG causes a delay in extubation. MC is a very important, serious, and reversi

Emergency and intensive care management of a comatose patient with intracranial hypertension, current concepts.

Altered mental status is a common occurrence in children with acute critical illness. The causes of non-traumatic coma are diverse ranging from neurological to systemic causes. Early appropriate supportive care is essential to avoid preventable secondary insults and optimize the neurological outcome. Evaluation and stabilization of the patient's airway, breathing and circulation (ABCs) must proceed simultaneously with assessments of the depth of coma and the presence of raised intracranial pressure (ICP). Any rapidly correctable cause of coma must be immediately corrected. Most patients with n

Problems in the care management of the aging patient: legal, financial and practical implications.

The graying of America poses many challenges to the dental professional. As the population ages, most dental practices are experiencing growth in percentage of older people served. The dental practitioner must be on the alert to special problems associated with the treatment of the older patient. Failure to identify these common concerns may result in patient non-compliance, failure to achieve treatment goals and heightened risk of malpractice. In addition, dental professionals who are aware of the multiplicity of factors that impact the aging process can better promote their own successful ag

[Management of palliative care patients facing emergencies in the community].

With an aging population, an increasing prevalence of chronic illnesses, and rising cancer cases, palliative emergencies in the community are becoming an ever-growing challenge. These situations generate new care needs, calling for general palliative care competencies among first-line healthcare providers and strong collaboration between the various actors involved in community-based care. But what exactly is a "palliative emergency"? Who intervenes, and how? Is it possible, even in the urgency of such situations, to foster patient autonomy and respect their care preferences? This article expl

[Management of vascular risk in patients with psoriasis in primary care].

Psoriasis is a chronic inflammatory disease associated with an increase in vascular risk, both for the higher prevalence of traditional risk factors as for the systemic inflammatory milieu. This risk is particularly increased in patients with moderate-to-severe psoriasis or psoriatic arthritis. Several studies have highlighted underdiagnosis and suboptimal control of hypertension, dyslipidaemia, diabetes and obesity in this population. Early and structured screening allows identification of patients with a high vascular risk eligible for intervention, being primary care the optimal area due to

Managing Chronic Pain in Ventilated Critical Care Patients.

Pain experienced by mechanically ventilated patients in intensive care units (ICUs) is often overlooked, but it is a significant issue. With over 5 million annual ICU admissions in the United States, the prevalence of pain during hospitalization and its aftermath is a pressing concern. Chronic pain in ICU survivors has been linked to post-traumatic stress disorder, poor quality of life, and long-term impairments known as postintensive care syndrome. Barriers to effective pain management include psychological factors, sedation restrictions, and inadequate use of valid assessment tools.

Critical care management of patients with lymphatic conduction disorders.

Lymphatic dysfunction in critical illness is complex. Primary complex lymphatic anomalies can lead to profound organ dysfunction, particularly respiratory failure and shock. Critical illness, the complications of critical illness, and the procedures and therapies used to treat critical illness, can lead to secondary lymphatic dysfunction. This is most often seen with congenital and acquired cardiovascular disease and respiratory disease. The critical care management of these patients requires an expert multidisciplinary team.

Management of adult patients with haematological malignancies in critical care.

There are a diverse range of haematological malignancies with varying clinical presentations and prognoses. Patients with haematological malignancy may require admission to critical care at the time of diagnosis or due to treatment related effects and complications. Although the prognosis for such patients requiring critical care has improved, there remain uncertainties in optimal clinical management. Identification of patients who will benefit from critical care admission is challenging and selective involvement of palliative care may help to reduce unnecessary and non-beneficial treatments.

Management of HIV-infected patients in the intensive care unit.

The widespread use of combination antiretroviral therapies (cART) has converted the prognosis of HIV infection from a rapidly progressive and ultimately fatal disease to a chronic condition with limited impact on life expectancy. Yet, HIV-infected patients remain at high risk for critical illness due to the occurrence of severe opportunistic infections in those with advanced immunosuppression (i.e., inaugural admissions or limited access to cART), a pronounced susceptibility to bacterial sepsis and tuberculosis at every stage of HIV infection, and a rising prevalence of underlying comorbiditie

Critical care management in patients with acute liver failure.

Acute liver failure (ALF) is defined as severe hepatic dysfunction (marked transaminases elevation, detoxification disorder (jaundice and coagulopathy with international normal ratio (INR) > 1.5), the presence of hepatic encephalopathy, and exclusion of underlying chronic liver disease, and a secondary cause like sepsis or cardiogenic shock. Reasons for ALF include paracetamol and warfarin toxicity, autoimmune and viral (mainly hepatitis B and E) hepatitis, and herbal and dietary supplements. Even in terms of meticulous and careful review of the patient, around 20-30% of the reasons remains un

Palliative Care in the Management of Patients with Advanced Heart Failure.

Globally, there are 18-million individuals living with heart failure, a disease that is responsible for 12-15 million office visits and 6.5 million inpatient hospitalizations each year. As HF becomes advanced or end-stage, patients often live in a cycle of frequent transitions between care settings, and with unmet needs, including distress from inadequately managed symptoms. Prognostication in patients with heart failure can be challenging due to the unpredictable exacerbating-remitting illness trajectory that is associated with this progressive disease. Recurrent hospitalizations, worsening f

Care of Patients at the End of Life: Management of Nonpain Symptoms.

Management of nonpain symptoms can improve quality of life for patients at the end of life and their family members. Constipation is the most common nonpain symptom. It can be related to opioid therapy and/or medical conditions. After abdominal examination to detect masses or evidence of bowel obstruction and rectal examination to exclude fecal impaction, constipation should be managed with a stimulant laxative (eg, senna) or an osmotic laxative (eg, sorbitol). Dyspnea also is common, and often improves with use of a fan to blow air into the face, as well with breathing and relaxation exercise
