[Emphasize preoperative imaging interpretation and surgical planning for total pelvic exenteration].

Total pelvic exenteration (TPE) is a critical surgical procedure for treating locally advanced rectal cancer (LARC) at stage T4b and locally recurrent rectal cancer (LRRC), where the resectability of pelvic tumors depends on precise preoperative imaging evaluation. Laparotomy primarily aims to exclude abdominopelvic peritoneal metastases. Preoperative assessment involves contrast-enhanced chest/abdominal CT, contrast-enhanced liver MRI (as a supplement when CT findings are unclear), contrast-enhanced pelvic MRI (the preferred modality for evaluating soft tissue planes, organ involvement, and r

Quality-of-life outcomes following pelvic exenteration for primary rectal cancer.

BACKGROUND: For patients with locally advanced tumours and contiguous organ involvement, pelvic exenteration (PE) can offer cure with relatively low mortality. The literature surrounding quality of life (QoL) in patients undergoing PE is limited. Furthermore, there are no matched comparisons of QoL between abdominoperineal resection (APR) and PE. The aim of this study was to compare differences in long-term QoL for patients with primary rectal cancer undergoing APR versus PE. METHODS: All patients who underwent either APR or PE between January 2011 and December 2012 were identified. Patients

Passage of peritoneal fluid into lumbar epidural catheter during pelvic exenteration.

Migration of an epidural catheter into either paravertebral tissues or pleural cavity has already been described. Up to now, no case of passage of peritoneal fluid into the epidural space has been reported. Here we report such an event in a patient submitted to pelvic exenteration, and discuss its diagnosis and management.

Pelvic exenteration with low rectal anastomosis: survival, complications, and prognostic factors.

Between July 1979 and January 1989 there were 31 patients who underwent pelvic exenteration with low rectal anastomosis (PE and LRA) at the University of Alabama at Birmingham (UAB). There were no operative deaths; however, 32% of the patients experienced significant postoperative morbidity. Survival (Kaplan-Meier) at 1 year is 86%. Overall survival is 68% with follow-up ranging from 10 months to greater than 9 years. For those patients with disease confined to the cervix and/or vagina (n = 18) the survival is 89% while it is 38% for patients with spread to the bladder, rectum, or parametrium

Survival Outcomes in Patients Undergoing Pelvic Exenteration for Pelvic Mucosal Melanomas: Retrospective Single Institution Australian Study.

BACKGROUND: Pelvic mucosal melanomas, including anorectal and urogenital melanomas, are rare and aggressive, with a median overall survival of up to 20 months. Pelvic mucosal melanomas behave differently from their cutaneous counterparts and present late with locoregional disease, making pelvic exenteration its only curative surgical option. OBJECTIVE: This study aimed to evaluate the survival outcomes after pelvic exenteration in pelvic mucosal melanomas at Royal Prince Alfred Hospital. DESIGN: Retrospective case series from a prospectively collected pelvic exenteration database from Octobe

[Feasibility and safety of a fascial space priority approach to total pelvic exenteration in patients with pelvic malignancy].

Objective: To evaluate the feasibility and safety of a fascial space priority approach to total pelvic exenteration (TPE) in patients with pelvic malignancy. Methods: This was a descriptive case series. Relevant clinical data of patients who had undergone TPE via a fascial space priority approach at Tianjin Union Medical Center from September 2017 to March 2025 were retrospectively collected. All operations had been performed via a fascial space priority approach, the guiding principle of which is separating the avascular pelvic spaces first and then transecting the vessels and nerves of the p

Using Bakri balloon as a visceral replacement for occupying pelvic cavity in pelvic exenteration, a case report.

INTRODUCTION AND IMPORTANCE: Total pelvic exenteration is the choice treatment for locally advanced or recurrent cervical cancers. However, the procedure is usually associated with serious complications. One of the most common complications is "empty pelvic syndrome". In this case report, we described a novel method to investigate its efficacy in prevention of empty pelvic syndrome. CASE PRESENTATION: A 51-year-old woman presented with recurrent cervical cancer underwent TPE after chemoradiotherapy. After removing the organs of the pelvic cavity, a silicone-made Bakri balloon was placed in th

Urological outcomes following pelvic exenteration for advanced pelvic cancer are not inferior to those following radical cystectomy.

BACKGROUND: Pelvic exenteration (PE) for locally advanced pelvic malignancy requires a multi-disciplinary approach and is associated with significant morbidity. Urinary reconstruction forms a major component of this procedure. The aim of the study is to review the urological outcomes following PE in a newly established pelvic oncology unit, to compare with those following radical cystectomy (RC) for bladder cancer. METHODS: Patients were identified from prospectively maintained PE and bladder cancer databases, inclusive of all cases performed between January 2012 and December 2016. Those with

A hundred patients with vertical rectus abdominis myocutaneous (VRAM) flap for pelvic reconstruction after total pelvic exenteration.

PURPOSE: We analysed the outcomes of a series of 100 consecutive patients with anorectal cancer with neoadjuvant radiochemotherapy and abdominoperineal exstirpation or total pelvic exenteration, who received a transpelvic vertical rectus abdominis myocutaneous (VRAM) flap for pelvic, vaginal and/or perineal reconstruction and compare a cohort to patients without VRAM flaps. METHODS: Within a 10-year period (2003-2013) in our institution 924 patients with rectal cancer stage y0 to y IV were surgically treated. Data of those 100 consecutive patients who received a transpelvic VRAM flap were col

Colonic flap with mucosa removed: a novel technique for pelvic reconstruction after exenteration of advanced pelvic malignancy.

BACKGROUND: Immediate reconstruction after pelvic exenteration is challenging. Several flap techniques, such as the vertical rectus abdominis musculocutaneous flap and the gracilis flap, have been reported. However, flap-specific complications have been documented. Instead of harvesting the myocutaneous flap from the abdomen or legs, our group has proposed a colonic flap for neovaginal reconstruction especially for rectal cancer with vaginal invasion. Nevertheless, the application of a colonic flap for individuals needing only vascularized tissue to fill up the pelvic dead space is problematic

Pelvic Exenteration: Oncological and Surgical Outcomes From a Single Tertiary Statewide Service.

BACKGROUND: Advancements in the multidisciplinary management of locally advanced and recurrent rectal cancer, as well as advanced pelvic malignancies, have significantly improved oncological outcomes. Pelvic exenteration is a complex procedure involving en bloc resection of the involved organs with the aim of achieving a microscopic negative margin (R0)1. Since 2012, the Royal Brisbane and Women's Hospital (RBWH) has been performing pelvic exenteration surgery and is now the statewide referral centre in Queensland. This study aims to present our oncological and surgical outcomes since establis

Pelvic Exenteration for Cervical Cancer: Oncological, Morbidity and Quality of Life Outcomes.

BACKGROUND AND OBJECTIVES: One third of patients who undergo definitive chemoradiotherapy for locally advanced cervical cancer develop disease recurrence. Salvage surgery in the form of pelvic exenteration (PE) may offer improved survival and possibly cure in carefully selected patients, however robust data is scarce. This study aimed to describe margin status, morbidity, survival and quality of life (QOL) outcomes following PE for cervical cancer at a high-volume center. Primary outcome was histological margin clearance; secondary outcomes were postoperative complications, overall survival (O

The Gamboa-Hoil Point for Primary Perineal Closure During Total Pelvic Exenteration.

Introduction Total pelvic exenteration (TPE) is a radical procedure for selected patients with advanced pelvic malignancies and is associated with significant perineal morbidity. Despite multiple reconstructive strategies, no consensus exists regarding the optimal strategy for perineal closure. The Gamboa-Hoil point is a standardized primary closure technique proposed by the authors, designed to reinforce perineal wound closure using a simple suture-based approach. Methods We conducted a single-center, retrospective descriptive study including patients with colorectal cancer (rectal, sigmoid c

Genitourinary Outcomes in Patients Undergoing Pelvic Exenteration in an Australian Quaternary Centre.

BACKGROUND: Pelvic exenteration (PE) including en-block resection of two or more adjacent pelvic organs, regional lymph nodes, and pelvic side wall is a major surgical undertaking with associated morbidity. This study aims to assess the rate of urological intervention and complications of PE at an Australian quaternary centre. METHODS: Patients undergoing PE with a genitourinary component between January 2003 and July 2021 were included. Data were collected prospectively and analyzed retrospectively. Complications were defined as early (< 30 days) or late (≥ 30 days) using the Clavien-Dindo c

Short- and Intermediate-Term Morbidity Following Total Pelvic Exenteration in Colorectal Cancer.

INTRODUCTION: Total pelvic exenteration (TPE) for clinical T4b colorectal cancer (CRC) is associated with significant morbidity. Short (0-30 days)- and intermediate (31-90 days)-term temporal analysis of complication onset is not well described, yet needed, to better counsel patients considering TPE. METHODS: A retrospective cohort study of consecutive patients with primary or recurrent clinical T4b pelvic CRC undergoing open TPE between 2014 and 2023 was conducted. Clinicopathologic variables were collected for each patient. Postoperative morbidity was classified according to the Clavien-Din

Urological complications following pelvic exenteration are comparable to those following radical cystectomy.

INTRODUCTION: Radical cystectomy and urinary diversion is required for both primary muscle-invasive bladder cancer and in the setting of pelvic exenteration for advanced malignancy of pelvic organs. Acute and chronic complications following radical cystectomy can be a significant cause of morbidity. We compared the rates of urological complications following these two procedures at our tertiary referral centre. METHODS: Patients who underwent radical cystectomy and urinary diversion either alone or as part of pelvic exenteration between June 2017 and April 2024 at our hospital were included.

Pelvic exenteration without external urinary or fecal diversion in gynecological cancer patients.

In 1993, the authors introduced an orthotopic bladder replacement technique in the treatment of gynecological cancer patients. In this series, they report their long-term experience with orthotopic urinary reconstruction in case of primary- and radiotherapy-treated recurrent gynecological malignancies. Between 1993 and 2003, in 29 patients orthotopic ileocecal ascending colon reservoirs (Budapest pouch) were created. Twenty-one of the 29 patients received radiotherapy prior to their operation. In 12 out of these 29 cases, the fecal stream was reconstructed with the use of low rectal anastomosi
