Penicillamine-induced polymyositis with complete heart block.

Complete heart block with severe Adams-Stokes attacks and polymyositis was observed in a 69-year-old man following 6 weeks penicillamine treatment for suspected rheumatoid arthritis. After drug withdrawal the muscle enzymes and the cardiac rhythm gradually normalized, only leaving a right bundle branch block. Although this is the first report to show that severe cardiac damage associated with penicillamine-induced polymyositis is a potentially reversible condition, the severity of the three cases reported so far warrants close observation for cardiac involvement in patients with penicillamine-…

Liver abnormalities in penicillamine treated rheumatoid arthritis.

Liver enzymes were followed in 99 patients treated with D-penicillamine for rheumatoid arthritis. In six abnormalities were found which consisted of elevated levels of lactic dehydrogenase. ALAT/ASAT, alkaline phosphatases or combinations of these. The changes were reversible on stopping the drug with one possible exception. No evidence of biliary cirrhosis, chronic active hepatitis or HBag-associated hepatitis was found. Liver biopsy was performed in 4 cases--one was taken 2 months after the treatment was discontinued, and was normal. One biopsy showed mild inflammatory changes, whereas in tw…

D-penicillamine therapy of acute arsenic poisoning.

Severe poisoning resulting from single ingestions of rodenticides, herbicides, or insecticides containing arsenic have been frequently recognized. We record three cases of solubilized arsenic trioxide poisoning in Navajo Indian children and one case of sodium arsenate ingestion in an infant. One fatality occurred during dimercaprol therapy prior to initiation of therapy with D-penicillamine. Three survivors were treated with 2.3-dimercaprol intramuscularly and with oral D-penicillamine. The use of D-penicillamine in arsenic poisoning has not been generally appreciated. Excretion data from the …

Diffuse alveolitis on a small dose of penicillamine.

A 60-year-old man with seropositive rheumatoid arthritis developed rapidly progressive dyspnoea and bilateral pulmonary infiltrates on short exposure to 50 mg penicillamine daily. He made a satisfactory recovery following cessation of penicillamine therapy and the addition of prednisolone. This case has been reported to the Committee on Safety of Medicines and we would like to emphasize that the possibility of penicillamine-induced lung disease should be recognized, even on a small dose of short duration.

[Morphology and pathogenesis of penicillamine-induced nephropathy].

Histological, immunohistological and electron microscopical investigations on the glomerular renal lesions in penicillamine-induced nephropathy were carried out by studying biopsy material of eight patients with rheumatoid arthritis (4 cases), allergic alveolitis (3 cases) and systemic scleroderma (1 case) under the treatment with D-penicillamine. Summarizing all the findings, the characteristic pattern of immune complex nephritis as membranous type glomerulonephritis on the one hand and proliferative type glomerulonephritis (mesangioproliferative GN, focal proliferative GN or minimal prolifer…

Immunohistochemical findings in a patient with penicillamine pemphigus.

This report describes the immunofluorescence (IF) findings in sequential skin biopsies and serum obtained during a follow-up study of a patient who developed pemphigus lesions after D-penicillamine treatment of arthritis. The distribution in the skin of IgG, IgM and C3 varied with the date, type and site of the biopsy sample. Thus IF patterns showed features of (a) pemphigus vulgaris, (b) pemphigus foliaceus and (c) pemphigus erythematosus. Indirect IF examination indicated the presence of circulating intercellular epithelial antibody of both the IgG and IgM class.

D-Penicillamine Induced Myelotoxicity: A Unique Case.

Purpose: A case of D-penicillamine-related myelotoxicity in a patient with Wilson's disease is reported. Summary: There is a paucity of literature regarding D-penicillamine (DPA) induced myelotoxicity in the setting of Wilson's disease (WD). A 22-year-old male presented with a 1-week history of bleeding gums and dizziness. Four months prior, he had been diagnosed with Wilson's disease and started on a regimen of DPA. His blood counts demonstrated profound pancytopenia. Due to concern for suspected drug-induced myelotoxicity, DPA was discontinued. Parvovirus B19, Epstein-Barr virus, cytomegalov…

D-penicillamine-induced cleft palate in mice.

The teratogenic potential of the lathyrogen, D-penicillamine (DP), was assessed in pregnant mice, especially with respect to its ability to produce cleft palate. The dosage and the duration of treatment as they relate to the induction of cleft palate were also studied. Two different doses of DP were administered orally for either 5 or 4 consecutive days during the critical period of palatal closure. D-penicillamine (DP) at a dose level which does not have any apparent maternal toxic effects produced cleft palate in the offspring, and this teratogenic effect depended more upon the duration of t…

Yellow nail syndrome associated with penicillamine therapy.

A patient is described in whom the nail changes of the yellow nail syndrome developed whilst she was taking penicillamine. Stopping the drug was associated with resolution of the nail changes. The yellow nail syndrome was first described in 1964 (Samman and White), but the cause is still unknown. Impaired lymphatic drainage is thought to be a factor in the pathogenesis, and lymphoedema is often an accompanying feature. We describe a case in which nail changes, typical of those described in the yellow nail syndrome, occurred in a patient treated with d-penicillamine, and in whom the nails rever…
