Effects of exposure to noise on permeability to potassium of the endolymph-perilymph barrier in guinea pigs.

Healthy guinea pigs were exposed to broadband noise at 115 dBA for 7 days. When perfusion of the scala vestibuli was carried with artificial perilymph containing 43K, the 43K concentrations in the perilymph of the scala tympani were very low in both control and noise-exposed guinea pigs. The transport rate constant for K+ was computed by utilizing compartmental analysis. The results indicate that exposure to noise at 115 dBA for 7 days does not alter the permeability to K+ of Reissner's membrane or the lateral wall of the cochlear duct but does decrease K+ conductance of the organ of Corti.

Syndrome of recurrent meninigitis due to congenital perilymph fistula with two different clinical presentations.

Recurrent meningitis secondary to a congenital labyrinthine anomaly is a rare clinical entity, diagnosis of which is dependent upon certain clinical, radiological and intraoperative features. In the following report we describe two children with congenital labyrinthine fistula and recurrent meningitis whose clinical presentation, radiological features and intraoperative findings were dissimilar and thus, illustrative of two different ways of presentation of this rare disorder. While one had a classical Mondini defect and unilateral hearing loss, the other had normal audiometric and radiographi

A dual wedge microneedle for sampling of perilymph solution via round window membrane.

Precision medicine for inner-ear disease is hampered by the absence of a methodology to sample inner-ear fluid atraumatically. The round window membrane (RWM) is an attractive portal for accessing cochlear fluids as it heals spontaneously. In this study, we report on the development of a microneedle for perilymph sampling that minimizes the size of RWM perforation, facilitates quick aspiration, and provides precise volume control. Here, considering the mechanical anisotropy of the RWM and hydrodynamics through a microneedle, a 31G stainless steel pipe was machined into wedge-shaped design via …

Cochlin-tomoprotein (CTP) detection test identified perilymph leakage preoperatively in revision stapes surgery.

Perilymphatic fistula (PLF) is defined as an abnormal leakage between perilymph from the labyrinth to the middle ear. Symptoms include hearing loss, tinnitus, and vertigo. The standard mode of PLF detection is intraoperative visualization of perilymph leakage and fistula, which ostensibly confirms the existence of PLF. Other possible methods of diagnosis include confirmation of pneumolabyrinth via diagnostic imaging. Recently, a cochlin-tomoprotein (CTP) detection test has been developed that allows definitive diagnosis of PLF-related hearing loss. We report the case of a 45-year-old man who p…

The cochlear modiolus as a functional gateway for fluid exchange between cerebrospinal flow and perilymph.

BACKGROUND: Long considered as the primary conduit to connect the inner ear and the subarachnoid space, the cochlear aqueduct has been shown obstructed in a paradoxically high percentage of population. This study aimed to elucidate the role of modiolus in perilymph-cerebrospinal fluid (CSF) communication. METHODS: A cohort of patients diagnosed with unilateral vestibular schwannoma (VS) were prospectively enrolled in this single-center observational study. On inner ear-specific magnetic resonance images, delayed enhancement (4 h after intravenous contrast administration) was quantitatively ev…

Utility of Perilymph microRNA Sampling for Identification of Active Gene Expression Pathways in Otosclerosis.

HYPOTHESIS: Profiling of microRNA (miRNA) within perilymph samples collected at the time of stapedectomy can be used to identify active gene expression pathways in otosclerosis as compared with controls. BACKGROUND: miRNAs are small non-coding RNAs that effect gene expression by post-transcription regulation and silencing. Perilymph sampling allows for a novel way to collect material actively involved in the disease process. METHODS: Perilymph was collected at time of stapedectomy, underwent a microarray analysis, and significantly expressed miRNAs were correlated to known bone morphology pa…

Comparative perilymph permeability of cephalosporins and its significance in the treatment and prevention of suppurative labyrinthitis.

Cephalosporins are nonototoxic antibiotics that provide excellent coverage for almost all bacteria that can cause suppurative labyrinthitis. In this study we performed comparative perilymph permeability determinations of the three cephalosporins that we deemed to have the most clinical potential in these varied situations. Perilymph pharmacokinetic profiles were established for ceftazidime, cefuroxime, and cefotaxime and its metabolite desacetylcefotaxime in 36 guinea pigs by using the technique of high-performance liquid chromatography. At 1, 2, 3, 4, and 6 hours after intravenous administrat…

High concentrations of thiosulfate in scala tympani perilymph after systemic administration in the guinea pig.

CONCLUSION: High concentrations of the antioxidant thiosulfate reach scala tympani perilymph after i.v. administration in the guinea pig. Thiosulfate concentrations in perilymph remain elevated longer than in blood. This warrants further studies on the possibility of obtaining otoprotection by thiosulfate administration several hours before that of cisplatin without compromising the anticancer effect caused by cisplatin inactivation in the blood compartment. OBJECTIVE: Thiosulfate may reduce cisplatin-induced ototoxicity, presumably by oxidative stress relief and formation of inactivate plati…

Dexamethasone Dosing of Human Perilymph Compared for Common Delivery Protocols Using Inner Ear Simulations.

UNLABELLED: <p>Introduction: Locally applied dexamethasone therapy of the inner ear is commonly used to treat disorders such as Meniere's disease and idiopathic sudden sensorineural hearing loss. Dexamethasone is also being used in conjunction with cochlear implantation to reduce electrode impedances and fibrous tissue formation around the implant. The variety of formulations currently in use makes it difficult to compare the dosing levels resulting from different therapies. METHODS: We have used the FluidSIM cochlear fluid simulation program to calculate perilymph and plasma dexamethasone le…

Vestibule-Middle Ear Dehiscence Tested With Perilymph-Specific Protein Cochlin-Tomoprotein (CTP) Detection Test.

An 8-year-old boy was referred to the ENT department for further evaluation of right-sided conductive hearing loss. A small cyst anterior to the oval window and fixation of the stapes footplate were observed during an exploratory tympanotomy. The concentration of a perilymph-specific protein, cochlin-tomoprotein (CTP), in the middle ear lavage fluid was measured with an ELISA-based CTP detection kit. The level of CTP in the middle ear lavage fluid before fenestration of the cyst was 0.26 ng/ml (negative), and its level after fenestration was 2.98 ng/ml (positive), confirming the presence of pe…
