[Effect of birth timing on perinatal mortality].

Several reports show increased risk of perinatal/neonatal mortality for night time, weekend and holiday births. Intrapartum-related deaths were attributed to physical and mental fatigue, reduced availability of skilled and experienced staff and inadequate supervision by senior staff. The incidence of intrapartum and neonatal deaths related to intrapartum asphyxia is regarded to be a sensitive indicator of the quality of care during labor. Continuity of care is regarded as a major element in providing quality medical care. Several reports indicate the occurrences of potentially preventable adve

Effect of prolongation of pregnancy on perinatal mortality.

OBJECTIVES: (1) To determine the distribution of singleton and twin births according to gestational age in a Nigerian obstetric population; and (2) to compare their perinatal outcomes according to gestational age. METHODS: A 10-year retrospective comparative study of twin and singleton births at a tertiary care center in Enugu, Nigeria. The variables analyzed were: the proportion of deliveries occurring at each gestational age, the gestational age-specific rates for stillbirths, cesarean section, babies with 1-min Apgar scores less than 4 and those whose birthweights were below the 10th perce

Perinatal and neonatal mortality and morbidity in Iraq.

Perinatal mortality rates in Iraq have been extremely high during the last 10 years, although detailed data are not available due to inadequate collection of health information during the Gulf conflict and the period of sanction which has continued since August 1990 until the present. The average perinatal mortality rate for Iraq was estimated to be 28 per 1000 live births during the period 1980-90 (the period before sanctions). This rate is similar to the rate in other countries in the region in the same period. The average perinatal mortality rate during the period 1990-1999 is estimated to …

Perinatal mortality in the Federation of Bosnia and Herzegovina.

AIM: The aim of the study was to investigate the most frequent causes of perinatal mortality in Bosnia and Herzegovina. METHODS: We analyzed in a retrospective study over a one year period (1999) the following cantons: Sarajevo, Mostar, Tuzla, Bihać, Gorazde, Travnik, Zenica, which represent about one half of the population of Bosnia and Herzegovina. RESULTS: Perinatal mortality in the analyzed regions within Bosnia and Herzegovina was 19.55@1000, which is unacceptable in comparison with developed countries. Early neonatal mortality (9@1000) was lower than late fetal mortality 10.55@1000. Th…

Determinants of perinatal and infant mortality in Italy.

Determinants of stillbirths, perinatal and infant mortality in Italy have been analyzed using information collected routinely by the Italian Central Institute of Statistics on more than 2,400,000 births and 33,000 infant deaths in the period 1980-1983. Individual records included data on maternal (i.e. age, education, obstetric history) and fetal characteristics (sex, birth weight, gestational week at birth). The Italian stillbirth, perinatal and infant (1st-365th day of life) mortality rates in the period considered were respectively 7.7/1000 births, 16.4/1000 births and 13.5/1000 livebirths.

Perinatal mortality and morbidity--the Scottish perspective.

Although perinatal mortality has continued to fall in Scotland during the last decade, there has been little change during the last three years i.e. 8.9, 8.9, 8.7 per thousand births respectively. Many deaths still result from antepartum haemorrhage and unknown causes so the Scottish Perinatal Mortality group is undertaking a confidential enquiry into the deaths of all normally formed infants of birthweight greater than 2500 g. The other major cause of prenatal death i.e. preterm delivery is also associated with a significant risk of neurological impairment. Results from the Scottish Low Birth…

Perinatal morbidity and mortality in twin pregnancies.

We reviewed 488 twin pregnancies that resulted in the birth of neonates weighing 1000 g or more. Complications were present in 56.7% of all pregnancies; most common were threat of premature labor (17.4%) and preeclampsia (14.5%). Perinatal mortality was 31.8 per thousand. Eighty percent of neonatal deaths in twin pregnancies involved newborns weighing less than 2000 g. Twin pregnancies are high risk pregnancies and benefit from early diagnosis, prevention and treatment of the threat of premature birth.

Perinatal mortality in a Saudi maternity hospital.

Perinatal mortality was analyzed in infants to mothers who delivered at the main Maternity hospital in Riyadh and who represented a large proportion of the obstetric population in the city. The crude perinatal mortality rate was 39.8 per 1000 births. The birth weight-specific mortality rates were compared with the corresponding Swedish rates. The ratio between the birth weight-specific mortality rates in Riyadh and Sweden increased from the lightest to the heaviest birth weight category. Thus the perinatal mortality rate was 2.3 times higher in infants less than 1500 g, and 8.8 times higher in…

Perinatal factors in twin mortality in Nigeria.

The results of a retrospective study involving 622 twin-pairs born over an 18-month period among 17,726 births at the University of Ilorin Teaching Hospital, Ilorin, Nigeria, are presented with particular reference to four variables: birthweight, presentation, parity, and intertwin delivery time interval. The twinning incidence was 35.1 per 1000. Monozygous and dizygous rates are 7 and 28 per 1000, respectively. Overall perinatal mortality (PNM) was 15.5%. Mortality was higher in second than in first twin (19.5% vs. 11.6%), and consistently higher when divided into birthweight groups. Correcte…

The pathology in perinatal mortality in Nairobi, Kenya.

The clinico-pathological causes of death in 497 stillbirths and 1st 24 hour neonatal deaths in Nairobi are described. The major factors in stillbirth (303 cases) were cord compression and prolapse, birth trauma, and prolonged labour. Hyaline membrane disease and intraventricular haemorrhage caused death in 52% of the neonatal cases. The importance of intra-uterine infection is discussed.

Causes of perinatal mortality in an African city.

Little has been published about the causes of most of the deaths responsible for the high perinatal mortality rates characteristic of preindustrial, urban societies. The present study searched for answers in one such society by identifying the causes of death in a large-scale analysis of perinatal mortality in Addis Ababa, Ethiopia. A 72% autopsy rate was achieved when 1019 postmortem examinations were performed for 1424 consecutive perinatal deaths. The overall perinatal mortality rate was 65.3 per thousand live births. The ratio of stillbirths to neonatal deaths was 2.7:1, indicating that ma…

Contribution of preterm delivery to perinatal mortality.

A detailed retrospective analysis was made of the records of 486 preterm infants, who accounted for 5-1% of all births during 1973 and 1974. Whereas preterm delivery did not contribute to perinatal mortality in terms of stillbirth, it outweighed all other causes in terms of early neonatal deaths. Preterm birth was responsible for 85% of the early neonatal deaths not due to lethal congenital deformities. Early neonatal mortality rates were closely linked both to gestational age and birth weight and to the reason for preterm birth. Early neonatal mortality was high (97 per 1000) when preterm lab…

The high-risk perinatal registry. A systematic approach for reducing perinatal mortality.

A systematic and comprehensive review of all 320 perinatal deaths occurring in Nassau County in 1973 reveals that one-quarter of these death … udes that rigorous application of currently available medical knowledge, the establishment of local perinatal mortality review committees, and vigorous outreach to practitioners are urgently needed to bridge the time gap between the development of new modalities of care and their application.
