A rare case of Whipple disease presenting as a hydrosalpinx and granulomatous peritonitis.

INTRODUCTION: Whipple disease is a rare infectious disease caused by the bacterium Tropheryma whipplei. The classic form affects gastrointestinal and musculoskeletal systems; but other forms may damage the heart, brain, or lungs. Due to non-specific and diverse clinical symptoms, diagnosis of Whipple disease is challenging and often late. Adequate and timely antibiotic treatment is essential for favorable outcome. CASE PRESENTATION: Here we present a case of a young woman admitted to the gynecological clinic for diagnostic laparoscopy for suspected haemato-/hydro- salpinx and peritoneal endom

[Diagnostic methods for peritoneal molecular residual disease in gastric cancer].

Peritoneal metastasis of gastric cancer serving as the most frequent form of metastasis, is one of the leading causes of death. A portion of surgically treated patients often suffer from small peritoneal residual metastasis, which will lead to recurrence and metastasis of gastric cancer patients after surgery. Given these, the prevention and treatment of peritoneal metastasis of gastric cancer deserves more attention. Molecular residual disease (MRD) refers to the molecular abnormalities of tumor origin that cannot be found by traditional imaging or other laboratory methods after treatment, bu

Current role for cytoreduction and HIPEC for gastric cancer with peritoneal disease.

Gastric cancer (GC) is an aggressive malignancy with a high burden of peritoneal disease. Evidence regarding the use of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) to improve outcomes has been growing. However, given multiple limitations, there remains a lack of international consensus regarding the optimal treatment paradigm. This review article discusses the burden of peritoneal disease in GC patients and the role of CRS + HIPEC in all treatment intents-curative, prophylactic, and palliative.

[Peritoneal tuberculosis. A rare form of presentation of an emerging disease in pediatrics].

Tuberculosis constitutes an important health problem, affecting one third of the world's population. The pulmonary localization is the most frequent one, being rare the peritoneal presentation. Clinical manifestations are non-specific so the diagnosis requires a high level of suspicion. The case of a 13-year-old teenager hospitalized for peritoneal tuberculosis is reported. The objective is to describe a rare form of extra-pulmonary manifestation of M. tuberculosis infection in the pediatric age and to sensitize the medical community, in the current epidemiological context, to the reemergence

Metastasis of colorectal carcinoma in the testis: first sign of peritoneal disease.

Metastases of colorectal carcinoma (CRC) in the testis are very rare and indicate an advanced stage of disease. In this case report, we present a patient with adenocarcinoma in the sigmoid colon with metastasis in the right testis. Testicular metastasis of CRC is mostly diagnosed late because of their low incidence rate. Patients with CRC and testicular metastasis have a poor prognosis. In this case, the patient turned out to have peritoneal metastasis and one should be aware that testicular metastasis could be the first sign of widespread disease.

Peritoneal Dialysis for Patients with Autosomal Dominant Polycystic Kidney Disease.

End-stage renal disease secondary to autosomal dominant polycystic kidney (ADPKD) is a common issue worldwide. Peritoneal dialysis (PD) is a reasonable option for renal replacement therapy for these patients and should not be withheld due to concerns that the patient may not tolerate the fluid volumes in the peritoneal cavity. This review covers the existing data on the outcomes and complications associated with the use of PD in the polycystic kidney disease patient. In general, PD is well tolerated and outcomes in ADPKD patients are equivalent to or better than other patient groups.

[Acute peritonitis complicating Crohn's disease: epidemiologic and physiopathological characteristics].

BACKGROUND: An uncommon event in the natural course of Crohn's disease is the spontaneous perforation. It's the most serious intestinal complication. AIM: To report the incidence of this complication and to discuss the mechanism of the perforation. OBSERVATIONS: We report, retrospectively, 4 cases (3 men and one woman; median age: 30 years) of peritonitis complicating Crohn's disease and study their clinical characteristics intra operatively findings and course evolution. Steroid therapy was administrated to all patients before the perforation. The perforation sites are on the ileum in three

Parathormone secretion in peritoneal dialysis patients with adynamic bone disease.

The prevalence of low-turnover lesions in patients undergoing peritoneal dialysis (PD) is high. Our aims are to evaluate the prevalence of adynamic bone disease (ABD) in PD patients, analyze risk factors, and define the association of serum parathyroid hormone (PTH) levels measured under different plasma calcium concentrations with this lesion. Fifty-seven patients were studied by bone biopsy (BB). ABD was found in 63.2%, and 36.8% showed high-turnover bone disease (HTBD). Patients with HTBD had a lower prevalence of diabetes, younger age, lower accumulated oral calcium salt intake, and greate

Prevalence of gastroesophageal reflux disease in peritoneal dialysis and hemodialysis patients.

The prevalence of gastrointestinal reflux disease (GERD) in hemodialysis (HD) and peritoneal dialysis (PD) patients was assessed at a single center with a self-administered questionnaire previously used in a general population. It defines (GERD as the presence of heartburn or acid regurgitation, or both. Risk factors for GERD and GERD-associated symptoms were also evaluated. In the studied population, 29.7% of patients had frequent GERD (heartburn, acid regurgitation, or both symptoms weekly). Frequent GERD was reported by 44.7% of PD patients versus the 18.9% reported by HD patients and the 1

Anomalous peritoneal folds of the duodenum--a normal variant simulating disease.

The double contrast barium meal appearance of four cases is presented where an anomalous peritoneal fold involves the duodenum. This fold results in a characteristic smooth, extrinsic indentation upon the antero-superior surface of the duodenal cap. This normal variant should be differentiated from distortion of this region due to pathology such as peptic ulceration. Anomalous peritoneal folds which involve the duodenum (cystoduodenal ligament, cysto-gastrocolic ligament) are not believed to cause symptoms (1). However, the recognition of the deformity produced by these folds is important, req

Cytologic findings in peritoneal washings associated with benign gynecologic disease.

In order to define the problems in the interpretation of peritoneal washing cytology specimens, 149 benign cases were examined retrospectively and the cytologies compared with the correlating histologies. There were problems in the cytologic interpretation on blind review in 18 cases (12.1%). Difficulties in interpretation of peritoneal washing cytology included conditions associated with (1) mesothelial reactions and adhesion formation, (2) rupture of cystic structures, (3) fallopian tube epithelium and (4) combinations of these factors. It is anticipated that familiarity with these problems

Peritoneal Dialysis Care for People with Diabetes, Polycystic Kidney Disease, or Advanced Liver Disease.

People treated with peritoneal dialysis (PD) often have complicating conditions that require careful management. Three such conditions are reviewed in this article-diabetes mellitus, polycystic kidney disease, and chronic liver disease. Each of these conditions requires an understanding of both its effect on the delivery of the PD and the effect of the PD on the condition itself. In diabetes, glucose absorption from the dialysate complicates metabolic control and affects salt and water management and patient outcome. There is particular benefit in clinical care being delivered through a multid

Prevention and treatment of peritoneal adhesions in patients affected by vascular diseases following surgery: a review of the literature.

Intra-abdominal adhesions are the most frequently occurring postoperative complication following abdomino-pelvic surgery. Abdominal and pelvic surgery can lead to peritoneal adhesion formation causing infertility, chronic pelvic pain, and intestinal obstruction. Laparoscopy today is considered the gold standard of care in the treatment of several abdominal pathologies as well as in a wide range of vascular diseases. Laparoscopy has several advantages in comparison to open surgery. These include rapid recovery times, shorter hospitalisation, reduced postoperative pain, as well as cosmetic benef

Elevated Sirtuin 1 Levels in Patients with Chronic Kidney Disease, Including on Peritoneal Dialysis: Associations with Cardiovascular Risk and Peritoneal Fibrosis.

Sirtuin 1 (SIRT1) is implicated in oxidative stress, inflammation, and fibrosis-processes central to chronic kidney disease (CKD) and cardiovascular complications. Increased serum levels of SIRT1 have been reported in dialysis patients, and its role in peritoneal fibrosis, a leading cause of peritoneal dialysis failure, is well established. This study evaluated serum SIRT1 levels in 165 participants: peritoneally dialyzed patients (CAPD), conservatively treated CKD patients (CT), and healthy controls. Serum SIRT1 was measured by ELISA and analyzed alongside clinical factors. SIRT1 concentratio

Transition to a combination of peritoneal dialysis and hemodialysis in chronic kidney disease patients receiving peritoneal dialysis treatment: a single-center study.

Peritoneal dialysis (PD) and hemodialysis (HD) are the two primary renal replacement therapies for patients with end-stage renal disease (ESRD). While PD is an effective and convenient modality, long-term use can lead to ultrafiltration failure, recurrent peritonitis, and progressive structural alterations in the peritoneal membrane, necessitating a transition to HD. In recent years, rather than a complete transition, the combined use of PD and HD has emerged as a viable alternative, offering potential advantages for selected patient populations. This retrospective study included patients with

Neisseria elongata -mediated peritonitis in an end-stage renal disease patient on automated peritoneal dialysis, A case report and literature review.

INTRODUCTION: Peritoneal dialysis (PD) can result in peritonitis, which frequently causes severe and near-fatal clinical implications if left untreated. Usually, Gram-positive bacteria are the most common organisms involved. Uncommonly recognized as the cause of peritonitis in PD patients, Neisseria Elongata is a gram-negative nasal and oropharyngeal normal flora organism. CASE PRESENTATION: We report a rare case of a 29-year-old man who had received automated peritoneal dialysis for six years and had Neisseria Elongata peritonitis. DISCUSSION: Several case Reports of niseria-related periton
