Evaluation of vaginal pessary management: a UK-based survey.

The use of intravaginal pessaries has been proven integral in the conservative treatment of pelvic organ prolapse (POP) and urinary incontinence (UI). Although there is no shortage of studies supporting the efficacy of intravaginal devices for conservative management of POP and UI and a large variety of pessaries are widely used in the UK, data on the clinical practice and recommended changing intervals are lacking. To evaluate the current clinical practice and management of patients with vaginal pessaries, self-administered questionnaires were mailed to all UK-based consultant obstetricians a…

[X-ray determination of the coincidence of intrauterine pessary and pregnancy].

It is reported on 21 pregnancies during contraception with IUD, among them one tubal pregnancy, one pregnancy with normal outcome and one pregnancy with a partly perforated IUD. By means of an extra- and intraamnial opaque matter injection one can find out exactly the position of the IUD on the roentgenogram before artificial abortion is carried out. The IUD can be seen above the small pelvis as a result of the changed position and the growth of the uterus during pregnancy. Furthermore it is possible to notice a glide down of the IUD into the cervix uteri or a perforation of the IUD into the u

The use of povidone-iodine ('Betadine') pessaries in the treatment of candidal and trichomonal vaginitis.

One hundred and thirty-five women suffering from trichomonal, candidal, or both infections simultameously, were treated with povidone-iodine pessaries, 2 pessaries being inserted nightly. Ninety-nine women were given a 7-day course of treatment, but the results obtained were disappointing, and the authors do not recommend such a regime for routine treatment. Better results were obtained with the recommended 14-day course. A further 36 women suffering from chronic trichomonal and/or canidal infections which had previously resisted orthodox treatment were given a prolonged 28-day course of pessa…

Cervical Pessary for Prevention of Preterm Birth: A Meta-Analysis.

To investigate the efficacy of cervical pessary placement in preventing preterm birth and perinatal morbidity and mortality in asymptomatic women with a singleton pregnancy and a short cervix, we searched literature in relevant databases. The meta-analysis of the 3 included trials (1412 women) showed cervical pessary placement did not reduce the risk of spontaneous preterm birth <34 weeks in these women (risk ratio (RR), 0.71; 95% confidence interval (CI), 0.21-2.43, P = 0.59; I2 = 90%). The sensitivity analyses by excluding one trial at one time showed the same results. This meta-analysis als

Delayed vesicovaginal fistula after ring pessary usage.

Vaginal pessaries are commonly used in the conservative management of pelvic organ prolapse, and are generally viewed as safe alternatives to surgery. Serious complications are rare, but can and do arise, typically as a result of the pessary not being fitted and maintained correctly. This case describes delayed development of a vesicovaginal fistula (VVF) 8 months after vaginal ulceration was noted and the ring pessary removed. The 82-year-old patient was managed with a urinary diversion via ileal conduit. This case highlights the importance of meticulous follow-up when a pessary is removed in…

A case of autoimmune progesterone dermatitis diagnosed by progesterone pessary.

Autoimmune progesterone dermatitis is a rare, cyclical eruption that occurs in the luteal phase of the menstrual cycle and during pregnancy. Many manifestations have been reported including cyclical eczema, urticaria, erythema multiforme, stomatitis and even anaphylaxis. The condition spontaneously resolves after menopause. As histopathology is non-specific, the diagnosis rests on history with precipitation of the eruption by a progesterone challenge, usually by the intradermal, intramuscular or oral route. We present the case of a 34-year-old woman with a premenstrual papular and eczematous e

Vesicovaginal fistula resulting from a well-cared-for pessary.

An 84-year-old vaginally grand multiparous woman presented with a vesicovaginal fistula (VVF) after appropriate use of a Gehrung pessary for the past 12 years for stage III pelvic organ prolapse. The patient reported strict adherence to removing her pessary nightly and replacing it in the morning for the last 12 years. One morning, she awoke and noted a sudden gush of urine through the vagina followed by continuous leakage. Given the complex nature of VVF repair with concurrent stage III pelvic organ prolapse, the patient was referred to urogynecologic care. A Latzko fistula repair and LeFort

Changes of intravaginal microbiota and inflammation after self-replacement ring pessary therapy compared to continuous ring pessary usage for pelvic organ prolapse.

AIM: Continuous usage of a ring pessary for pelvic organ prolapse may cause a disturbance of intravaginal microbiota and intravaginal mucosal damage. To avoid the side effects of continuous ring pessary therapy, daily self-replacement of the ring pessary is recommended. The purpose of this study is to evaluate the outcomes of self-replacement versus continuous ring usage, by analysing clinical findings and intravaginal microbiota. METHODS: Thirty seven patients who managed self-replacement ring pessary therapy and 13 patients with continuous ring pessary therapy participated in this study. Th

Is treatment with vaginal pessaries an option in patients with a sonographically detected short cervix?

OBJECTIVE: The purpose was to determine the effect of vaginal pessaries in patients at risk for spontaneous preterm birth (SPB). STUDY DESIGN: Transvaginal sonography (TVS) was longitudinally performed to measure cervical length (CL) in 258 singleton at risk for SPB and 282 twin pregnancies. Pairs with or without treatment were matched for gestational age and the CL at examination. RESULTS: In 4 singleton and 7 twin pregnancies the CL was < 15 mm before 24 weeks, the mean interval between pessary insertion and delivery was 13 + 2 and 12 + 5 weeks respectively. For the matched control analysi…

The use of pessary for pelvic organ prolapse in pregnancy: A case report.

Pelvic organ prolapse (POP) complicating pregnancy is rare, and evidence for its management remains limited. We report a case of advanced POP during pregnancy that was successfully managed with a ring pessary. A 40-year-old woman with longstanding prolapse presented with worsening symptoms during pregnancy. Conservative management with a vaginal pessary provided effective antepartum support and allowed continuation of pregnancy until spontaneous labor. After vaginal delivery, prolapse recurred and was successfully managed with pessary reinsertion during the puerperium after lochia had decrease

Unusual presentation of vaginal ring pessary incarceration in a young patient.

Vaginal pessaries are a simple and effective conservative treatment for pelvic organ prolapse. However, neglected devices can result in rare but serious complications, including incarceration and fistula formation. A woman in her early 30s with a history of uterine prolapse presented with 4 months of discomfort, lower abdominal pain and dyspareunia. She had a ring pessary inserted 1 year earlier, which had not been replaced or maintained. Examination revealed the pessary incarcerated in the posterior fornix with mucosal overgrowth. Histopathology excluded malignancy, routine tests were normal …
