Four patients with chronic or smouldering type adult T cell leukaemia (ATL) were treated by extracorporeal photochemotherapy (photopheresis). From 4 to 8 months after starting photo-pheresis, skin lesions with ATL cell infiltration began to disappear. Cell surface markers in three patients showed improvement. In one patient, a serial decrease of soluble interleukin-2 receptor levels (950 U/ml to 620) in the serum was observed after 4 months. Six patients with extensive pityriasis alba were treated with oral methoxsalen, followed by exposure to midday summer sun or exposure to long-wave ultraviolet radiation in a psoralens plus ultraviolet A (PUVA) cabinet. Complete clearing or marked improvement was obtained in five patients in less than 4 weeks of treatment. The patient with the most extensive skin involvement and the longest duration of the disease achieved a marked degree of improvement after 15 weeks of therapy. A long-term follow-up is still required to define the maintenance treatment and its efficacy.

Oral photochemotherapy has been employed in the treatment of mycosis fungoides for more than 13 years. The Authors report their 9 year experience in a series of 47 patients affected by mycosis fungoides and treated by means of PUVA-therapy. Eight patients with mycosis fungoides limited to the skin have been treated with either topical nitrogen mustard alone or in combination with photochemotherapy. This regime does not prevent contact sensitization to nitrogen mustard but will clear sanctuary sites of disease which may develop when photochemotherapy is used alone.

This study was undertaken to assess the amount of glycogen present in psoriasis vulgaris (PV), psoriasis erythrodermica (PE) and psoriasis pustulosa generalisata (PPG) lesions before and after photochemotherapy (PUVA). There were more deposits of glycogen in PPG than in PE and PV. With PUVA treatment, glycogen almost completely disappeared in PV and PE, but traces were still observed in PPG. After treatment, the epidermal layers returned to their normal structure but there was no significant improvement in the dilated and tortuous capillaries in the upper corium.

Chronic graft-versus-host disease (GVHD) is a major limitation of successful allogeneic hematopoietic stem cell transplantation (HSCT). Extracorporeal photochemotherapy (ECP) has been tested extensively in small cohorts of patients with chronic GVHD. In this study, we retrospectively evaluated 71 patients with severe chronic GVHD treated with ECP. Response rate was 61% (n = 43), and 14 patients had complete responses (CRs).
