PURPOSE: We describe a case and our experience with using plasmapheresis as a treatment for scleritis.
OBSERVATIONS: Treating relapsing autoimmune scleritis can be challenging when it inadequately responds to traditional therapy. Our patient could not receive non-steroidal anti-inflammatory therapy for her scleritis due to recent gastrointestinal surgery and previously failed multiple steroid sparing treatments due to intolerance. BACKGROUND: Hyperthyroidism is characterized by excess hormone secretion from the thyroid gland. Anti-thyroid drugs (ATDs), surgery, and radioactive iodine can be used in treatment. Plasmapheresis is a rapid and effective treatment option in cases where rapid euthyroidism is needed to be obtained due to complications of thyrotoxicosis and major adverse effects of ATDs.
MATERIAL AND METHOD: We present patients receiving plasmapheresis to provide immediate euthyroidism due to severe hyperthyroidism, adverse effects of ATDs, or non-thyroid surgery from January 2012 to December 2016.

The paper presents the results of plasmapheresis inclusion into a combined therapy of pyelonephritis. Out of 79 patients treated, 42 had urosepsis, 25 developed pyelonephritis in pregnancy, 12 had complicating chronic renal failure. Uroseptic patients were examined for hemostasis, the rest for immune status. There were symptoms of DIC syndrome in the former and immunity suppression in the latter. Twenty patients with rapidly progressing glomerulonephritis (RPGN) aged 16-50 underwent plasmapheresis (PA) as adjuvant to immunodepressants. Of these, 9 patients had lupus, 4 Bright RPGN, 4 and 3 patients combined glomerulonephritis with hemorrhagic vasculitis and Wegener granulomatosis, respectively. The authors studied the efficacy of plasmapheresis in preoperative management of patients with thyrotoxicosis in intolerance of and resistance to thyrostatic agents. Immunity status parameters were examined in 50 patients (26 male and 24 female ones) with severe forms of drug-induced toxicoderma whose treatment included traditional antiinflammatory therapy and plasmapheresis. The results evidence a high efficacy of plasmapheresis parallelled by traditional therapy showing immunocorrective effect.

Trials of plasmapheresis or plasma exchange (PE) in pemphigus and pemphigoid have been justified by the well demonstrated role of autoantibodies and the side effects of conventional therapy with high doses of steroids. Most open studies reported effectiveness of PE, mainly in steroid-resistant cases. However, a controlled study showed no benefit from PE in pemphigus when associated with a low steroid dose. Five patients with primary biliary cirrhosis and prolonged cholestasis underwent intensive plasmapheresis. The indications for plasmapheresis included intractable pruritus or hypercholesterolemia and xanthomatous neuropathy. Patients noted a rapid improvement of pruritus and fatigue which was sustained as long as plasmapheresis was continued. Cholesterol levels were lowered an average of 10.3 mmol/l and xanthomata were reduced in three of four patients.
