Completion pneumonectomy for bronchiectasis: morbidity, mortality and management.

BACKGROUND: Completion pneumonectomy performed for benign causes is associated with a high morbidity and mortality. We analyzed the patients who underwent completion pneumonectomy procedure for bronchiectasis, which constitutes a specific benign condition, together with the indications for surgery, the difficulties encountered during operations and the postoperative results. METHODS: Records of all patients who underwent completion pneumonectomy for the diagnosis of brochiectasis between January 1991 and April 2006 at the thoracic surgery clinic of a training and research hospital specializin…

[Pneumonectomy with extracorporeal circulation to treat pulmonary metastasis].

Surgical resection of pulmonary metastasis is appropriate provided the general principals of oncological surgery are followed. Complete excision of the metastatic tumor is associated with long-term survival and low perioperative mortality. We present 2 cases of single pulmonary metastasis from osteosarcoma with cardiac involvement. In both cases, complete excision required left pneumonectomy by sternotomy with extracorporeal circulation. The outcomes were favorable, no perioperative complications were reported, and the patients remained free of disease at 14 and 17 months after surgery. In con…

[Clinical analysis of completion pneumonectomy for pulmonary disease].

OBJECTIVE: Completion pneumonectomy (CP) is widely known to be associated with a high morbidity and mortality. However, in certain instances, CP offers the only chance for a cure. Now to explore the indications, prevention and management of complications as well as late outcomes of CP. METHODS: During a period of 21 years from January 1985 to August 2006, 24 patients received CP, representing 2.3% of 1026 patients who had undergone pneumonectomy in the same period. There were 17 right and 7 left CPs done in 20 male and 4 female patients with an average age of 58 years (range from 42 to 67 yea…

[A case of pneumonectomy for lung cancer associated with scleroderma].

A 60-year-old female, diagnosed as scleroderma, was referred to our hospital because of symptoms of common cold and abnormal findings on a chest X-ray. The chest X-ray and CT scan revealed a mass in the left upper field, suspected to invade left anterior chest wall. Moreover, fibrotic changes were observed in the lung field. Cytology by bronchofiberscopy showed squamous cell carcinoma. Evaluation of pulmonary function, including unilateral pulmonary artery occlusion test (UPAO), revealed possibility of lung resection. Subsequently, pneumonectomy with combined resection of left 1st rib was perf…

Effect of left pneumonectomy on lung mechanics in rabbits.

Left pneumonectomy was performed in nine rabbits at 10 wk of age. Detailed pulmonary mechanics studies were performed at 14 wk of age in these rabbits and in nine littermate controls. Lung weight, total lung capacity, and static lung pressure-volume curves of the pneumonectomized rabbits were not different from those of the controls, but the forced vital capacity was smaller (P less than 0.05). Tantalum bronchograms of excised right lungs revealed no difference between controls and pneumonectomized rabbits in terms of bronchial diameter or pressure-area behavior of central or peripheral airway…

[Functional state of the remaining lung following pneumonectomy].

The investigation performed has revealed moderate pulmonary hypertension associated with adaptation of the organism to changed conditions of blood circulation in the lungs. Greatly elevated pressure in the lung artery in response to physical exercise allowed to make a conclusion of practically complete mobilization of compensatory reserves of the right portion of the heart. The changes in the functional state of the remaining lung are evaluated as compensatory ones aimed at the maintenance of a sufficient square surface for gas exchange.

Robotic-assisted Left Pneumonectomy For Vanishing Lung Syndrome.

Vanishing Lung Syndrome (VLS) is a rare, severe form of bullous pulmonary disease characterized by giant bullae occupying more than 30% of the hemithorax, which can easily be misdiagnosed as a pneumothorax. While cigarette smoking is the primary cause, other factors such as Ehlers-Danlos Syndrome and alpha-1 antitrypsin deficiency have also been associated with the condition. Surgical management, including pneumonectomy, remains the gold standard for severe VLS cases. This case report aims to highlight the effectiveness of the robotic approach and the benefits of robotic-assisted left pneumone…

Trauma Pneumonectomy: Has Survival Improved Over Two Decades?

IntroductionThe hypothesis of this study is that recent advances in mechanical cardiopulmonary support and operative management have improved survival in patients requiring a trauma pneumonectomy.MethodsRetrospective, single center study from January 2003 to December 2023 of all patients who underwent a pneumonectomy for trauma. Data collected included demographics, admission physiology, use of venovenous extracorporeal membrane oxygenation (VV-ECMO), and mortality, defined as early (within 24 hours of surgery) and late (>24 hours after surgery). Outcomes were compared between decades, the fir…

Post-pneumonectomy Cardiac Herniation Management: A Rare Case.

Cardiac herniation is an uncommon complication of pneumonectomy. It usually occurs when a defect is made in the pericardium during the surgical resection of the lung. It is a life-threatening complication; even after surgical correction, morbidity and mortality rates remain high. We present a case of right-sided cardiac herniation after extrapleural pneumonectomy. This case report describes the difficulty in diagnosing and managing this rare complication. It discusses the causes and pathophysiology of cardiac herniation. We have described the clinical presentation and tools to diagnose such ca…

Transsternal occlusion of main bronchi fistulae after pneumonectomy.

OBJECTIVE: To summarize our experience in transsternal occlusion of main bronchus fistula. MATERIAL AND METHODS: We have performed 146 transsternal occlusions of the main bronchi for the period from 1979 to 2018. There were 123 (84.2%) men and 23 (15.7%) women. Mean age of patients was 63 years. Lung tuberculosis was diagnosed in 36 (24.7%) patients, lung cancer - 91 (62.3%) patients, non-specific suppurative lung diseases - 14 (9.6%) patients, trauma - 5 (3.4%) patients. All patients underwent transsternal occlusion of the main bronchi fistulae with or without pericardial cavity dissection.

Balanced Chest Drainage Prevents Post-Pneumonectomy Pulmonary Oedema.

BACKGROUND: Pneumonectomy in the adult patient is associated with a mortality of 1-9%. Death is often due to post pneumonectomy pulmonary oedema (PPPO). The use of balanced chest drainage system (BCD) in the setting of post pneumonectomy has been reported to be of benefit in the prevention of PPPO. This study seeks to compare the incidence of PPPO in patients who underwent pneumonectomy and whose empty pleural space was managed either with CRD or BCD. METHODS: This retrospective observational cohort study involved 98 patients who were operated on by one surgeon at Liverpool Hospital, Sydney, …

Extrapleural pneumonectomy (EPP) vs. pleurectomy decortication (P/D).

Surgical techniques for treatment of malignant pleural mesothelioma (MPM) have improved over the decades. The main surgical principle was accepted as macroscopic complete resection. This principle was achieved with extrapleural pneumonectomy (EPP) and extended pleurectomy/decortication (P/D). Mortality and morbidity are higher following EPP with supraventricular arrhythmias, pulmonary embolism, bronchopleural fistula and pulmonary failure being the most common, while prolonged air leak is frequent following P/D. Completion of multimodality treatment was also shown to be a prognostic factor. Ma…

Unidirectionally progressive left pneumonectomy & mediastinal lymph node dissection.

The patient has lower left lung tumor and adenocarcinoma at the openings of both upper and lower left lung. Preoperative bronchoscopic biopsy has confirmed the diagnosis. The surgical approach is unidirectionally progressive left pneumonectomy + mediastinal lymph node dissection. The layers of structure are treated one after another until the fissure from a single direction through the working port. Hence, the resecting order should be left superior pulmonary vein-left lower pulmonary vein-left main bronchus-left pulmonary artery. The vessels and bronchi are cut using an endoscopic linear stap…

Video: Single-incision video-assisted thoracoscopic right pneumonectomy.

BACKGROUND: The most common approach for Video-assisted thoracoscopic (VATS) lobectomy is undertaken with three or four incisions, including a utility incision of about 3-5 cm. However, major pulmonary resections are amenable by using only a single utility incision. This video shows the technical procedure of a right pneumonectomy by single-incision approach with no rib spreading. METHODS: A 52-year-old woman was proposed for single-incision VATS resection of a 5-cm right lower lobe adenocarcinoma. A 4-cm incision was made in the fifth intercostal space. We placed a 30-degree, high-definition…

[Early surgical treatment of bronchopleural fistula after pneumonectomy].

OBJECTIVE: To investigate the method of early surgical treatment of bronchopleural fistula after pneumonectomy. METHODS: Twelve patients (9 males and 3 females with a mean age of 58.6-/+5.7 years) with bronchopleural fistula after pneumonectomy received a reoperation within 72 h after a definite diagnosis. Empyema was found in none of the 12 cases. Fistula occurred within 4 to 17 days (8 days in average) after the operation. The fistula of the residual main bronchus was resected, and the thoracic cavity was asepticized by flushing. RESULTS: Ten patients were discharged with complete healing.…
