A comparison of the frequency and severity of poisoning cases for ingestion of acetaminophen, aspirin, and ibuprofen.

During 1984, 62,837 cases of human exposure to analgesics were reported to the National Data Collection System of the American Association of Poison Control Centers. We undertook a comparative study of 10,134 cases of human exposure to adult formulations of aspirin, acetaminophen, and ibuprofen from which we found that human exposures to analgesics are a common problem reported to poison control centers. All three analgesics studied appear to have minimal toxicity in children who accidentally ingest the product and report the exposure to a poison control center. Most children were managed with

Fatalities involving acetaminophen combination products reported to United States poison centers.

CONTEXT: Deaths from overdose of acetaminophen (APAP) combination medications are reported, yet the individual ingredients are not well examined as individual putative causes of death. OBJECTIVE: To examine the individual contribution of APAP or other ingredient(s) to fatalities resulting from ingestion of APAP combination products reported to poison centers. MATERIALS AND METHODS: A search in the United States (US) National Poison Data System between 1 January 2000 and 31 December 2009 (10 years) was conducted. Only fatal cases determined by American Association of Poison Control Centers Fa

Contaminant identification: the importance of thorough interviewing techniques.

The exact name of a contaminant is of extreme importance to the poison information specialist (SPI) in order to make a correct assessment of the poisoning exposure and to recommend proper treatment modalities. Incorrect contaminant information could lead to a serious and possibly fatal mistake on the part of the SPI. Thorough and effective interviewing techniques must be employed to avoid errors. We conducted a prospective study to determine how often misinformation is provided to the SPI at the time of the initial contact with the poison center. Over a 6-mo period 159 calls were identified in

A descriptive study of an outbreak of clenbuterol-containing heroin.

STUDY OBJECTIVE: Illicit drugs may be adulterated with substances other than the sought-after substance of abuse. Although the true incidence and clinical effects of this practice are unknown, geographically disparate outbreaks of clinically significant adulteration continue to occur. We report on a recent outbreak of clenbuterol-adulterated heroin occurring along the East Coast of the United States. METHODS: After identification of index cases, 5 US poison centers collaborated with state and territorial health departments to alert the public of clenbuterol-tainted heroin. A case definition o

Ingestions considered nontoxic.

We have compiled a list of common household products and drugs that are frequently ingested by children and may be considered nontoxic unless taken deliberately or in large amounts. An understanding of the nontoxic ingestion should prevent overtreatment, decrease emergency room visits, and allow physicians and poison control centers the opportunity to practice poison prevention. The reporting of all ingestions is encouraged to obtain information on the human experiment that occurs when a non-edible material is ingested. Only as we accumulate this knowledge will we be able to advise with a degr

A profile of unintentional poisoning caused by household cleaning products, disinfectants and pesticides.

Unintentional poisoning occurred mainly among children. The leading cause of such poisoning in Brazil, among consumer products was household cleaning products. For this study 2810 calls made to two poison control centers in the State of Rio de Janeiro between January 2000 and December 2002 were analyzed. Children under five were the most vulnerable group. More boys under 10 suffered accidental poisoning than girls, although above this age, the distribution was inverted. The calls received by poison control centers were mainly from health services within the first two hours following poisoning.

Home syrup of ipecac use does not reduce emergency department use or improve outcome.

BACKGROUND: The usefulness of syrup of ipecac as a home treatment for poisoning and the need to keep it in the home has been increasingly challenged. Many poison centers do not recommend any use of syrup of ipecac. OBJECTIVE: To determine if use of syrup of ipecac in children at home is associated with reduced utilization of emergency department (ED) resources or improved outcome after unintended exposure to a pharmaceutical. DESIGN: Cohort comparison. SETTING: American Association of Poison Control Centers' Toxic Exposure Surveillance System Database. PATIENTS: Blinded data for each of th

Camphor Poisoning: an evidence-based practice guideline for out-of-hospital management.

A review of national poison center data from 1990 through 2003 showed approximately 10,000 annual ingestion exposures to camphor-containing products. A guideline that determines the threshold dose for emergency department referral and need for pre-hospital decontamination could potentially avoid unnecessary emergency department visits, reduce health care costs, optimize patient outcome, and reduce life disruption for patients and caregivers. An evidence-based expert consensus process was used to create the guideline. Relevant articles were abstracted by a trained physician researcher. The firs

Hydrogen cyanamide exposure: a case series from Pavia Poison Control Centre.

BACKGROUND: Hydrogen cyanamide is a plant growth regulator introduced in Italy as Dormex in 2000, but recalled from the market in 2008. It's currently not authorized in Europe. Inhalation/dermal contact may cause irritation/caustic burns, ingestion of severe organ damage and concomitant alcohol consumption disulfiram-like reaction due to aldehyde-dehydrogenase inhibition by hydrogen cyanamide. AIMS: To study all exposure cases referred to our centre, evaluating temporal and geographic distribution and analysing clinical manifestations, including the ones after alcohol consumption. [Experience of the Toxicology Information Center in Prague with pesticides].

The Toxicological Information Centre replied in 1988 and 1989 some 5500 inquiries. This number comprised 5-7% concerning treatment of acute intoxications with pesticides. More than half of them concerned children who ingested pesticides. In adults in half of inquiries possible intoxication after ingestion was involved, in half intoxication was a result of inhalation or percutaneous intoxication. 10.5% and 12.5% of the inquiries concerning pesticide intoxications in adults were attempted suicide. The number of inquiries regarding occupational diseases during spraying was small (some 5 and 3 cas

Hypothermia: an unusual side effect of paracetamol.

We report 4 cases of children who presented hypothermia after normal therapeutic doses of paracetamol. Possible mechanisms of the antipyretic action of paracetamol are reviewed in an attempt to explain this rather exceptional description. Comparison is made with other cases in the literature and the experiences of other Poison Control Centers.

Envenomations during pregnancy reported to the national poison data system, 2009-2018.

Envenomations during pregnancy have consequences affecting both maternal and fetal outcomes. U.S. poison center data on envenomations offers a comparative view of envenomations in pregnant and non-pregnant women. The National Poison Data System of the American Association of Poison Control Centers was searched for cases of envenomation during pregnancy between January 1, 2009 and December 31, 2018 and compared with exposures to non-pregnant females of childbearing age. Odds ratios and descriptive statistics were used where appropriate. There were a total of 3,555 venomous animal exposures in p

Salicylate poisoning: an evidence-based consensus guideline for out-of-hospital management.

A review of U.S. poison center data for 2004 showed over 40,000 exposures to salicylate-containing products. A guideline that determines the conditions for emergency department referral and pre-hospital care could potentially optimize patient outcome, avoid unnecessary emergency department visits, reduce health care costs, and reduce life disruption for patients and caregivers. An evidence-based expert consensus process was used to create the guideline. Relevant articles were abstracted by a trained physician researcher. The first draft of the guideline was created by the lead author. The enti

Acute intoxications during work.

Information on acute intoxications during work in the Netherlands has been gathered from 4 sources during a 1-year period: 1. The registration of occupational accidents of the Directorate-General of Labour, by reviewing reports on accidents involving chemicals; 2. The Netherlands Poison Control Centre of the National Institute for Public Health and Environmental Hygiene, by tracing back consulting physicians; 3. The Dutch Centre for Health Care Information, by searching their data base for hospital admissions with relevant admission diagnoses and 4. A large Occupational Health Service, which f

Management of patients with suspected but unidentified poisoning in the emergency department: a joint Royal College of Emergency Medicine and National Poisons Information Service best practice guideline.

The management of patients suspected, but not confirmed, as being poisoned is challenging. The Royal College of Emergency Medicine and National Poisons Information Service have produced this guidance to provide a generalised clinical approach to any poisoned patient in the emergency department. This guideline provides a clinical approach to support the initial assessment of a patient, identification of potential toxic agents and emergency management. A toxidromic approach is used, with emphasis on consideration of the toxicokinetics of potential poisons and how the patient's clinical condition
