Neuromuscular Complications of HIV-1 Infection.

Neuromuscular disorders are the most frequent neurologic complications that occur in patients with HIV infection. The distinction among the different forms of peripheral neuropathy (ie, distal symmetrical polyneuropathy, polyradiculopathy, mononeuritis multiplex) is crucial in determining their potential etiology and treatment. Distal symmetrical polyneuropathy is most common in HIV-infected patients with advanced immunosuppression and may also result from neurotoxicity of several antiretroviral agents. Myopathy may occur at any stage of HIV disease, and has also been described as a toxic side…

Guillain-Barré Syndrome Preceded by Bell's Palsy After Acute Infectious Gastroenteritis: A Case Report.

Guillain-Barré Syndrome (GBS) is an acute immune-mediated inflammatory demyelinating polyradiculopathy of the peripheral nerves often provoked by a preceding upper respiratory or gastrointestinal infection. Guillain-Barré Syndrome usually presents with symmetrical lower limb ascending weakness and decreased deep tendon reflexes. Here, we describe a case of an uncommon presentation of GBS presenting with upper extremity neuropathy and cranial nerve palsy in a 36-year-old Caucasian Army pilot. This case highlights various ways GBS may present and the importance of early identification and treatm…

Rehabilitation outcomes after combined acute disseminated encephalomyelitis and Guillain-Barré syndrome in a child: a case report.

A 5-year old female presented with acute tetraparesis and areflexia. Initial imaging and cerebrospinal fluid analysis were suggestive of acute disseminated encephalomyelitis (ADEM). Minimal clinical response with intravenous steroids prompted further work up. Limited nerve conduction studies suggested possible acute motor-sensory axonal neuropathy, a rare variant of Guillain-Barré syndrome (GBS). Repeat imaging was compatible with polyradiculopathy indicating concomitance of ADEM and GBS. The patient suffered severe motor deficits and neuropathic pain. Slow but significant functional recovery …

Cytomegalovirus polyradiculoneuropathy in acquired immune deficiency syndrome.

A 34-year-old homosexual man with acquired immune deficiency syndrome developed extraocular muscle deficits, chorioretinitis, and paraplegia without sensory symptoms. EMG showed severe diffuse denervation, but only mildly slowed nerve conduction velocities, in both legs. Meningitis persisted for 6 weeks and was exacerbated prior to the patient's death. Necropsy revealed subpial and subependymal cytomegalovirus (CMV) infection. Histology of ventral roots demonstrated proximal CMV infection and massive fiber loss. In this immunosuppressed patient, CMV caused a severe motor polyradiculopathy by s…

"A girdle-like tightening sensation" misapprehended as abdominal splanchnopathy in a sarcoidosis patient.

We describe a 53-year-old man with the isolated manifestation of girdle-like tightening sensation of the trunk due to polyradiculopathy at the beginning of sarcoidosis which was first misapprehended as abdominal splanchnopathy. Late development of other neurological and systemic symptoms led to the final diagnosis of sarcoidosis. Segmental dysesthesia at the trunk in neurosarcoidosis is unique and may mimic a splanchnic pain. Such a dysesthesia may be solely manifested at the beginning of sarcoidosis and may continue for days without other symptoms. When patients complain of a girdle-like tigh…

A Rare Case of Neurosarcoidosis With Symmetric Multilevel Dorsal Root Ganglia Uptake on 18 F-FDG PET/CT.

Approximately 17% of patients with neurosarcoidosis develop peripheral neuropathy, whereas polyradiculopathy is rare, occurring in ∼2% of patients with neurosarcoidosis. Radiculopathy may be associated with dorsal root ganglion (DRG) involvement. We report a case of a 28-year-old man who presented with limb weakness and sensory disturbances. 18 F-FDG PET/CT showed bilateral symmetric hypermetabolic foci in multiple cervical and lumbar DRGs. 18 F-FDG PET/CT was more informative in detecting DRG lesions and mapping their distribution than MRI. Few reports have presented cases of DRG and asymmetr…

Guillain-Barré Syndrome Following Chickenpox With Multiple Cranial Nerve Palsies and Cerebrospinal Fluid Pleocytosis.

Guillain-Barré syndrome (GBS) is a rare complication of chickenpox. All cases of post-varicella GBS published in the literature have been associated with the classical albuminocytological dissociation. We report the case of a 48-year-old male with flaccid areflexic quadriparesis and bilateral seventh, ninth, tenth, and twelfth cranial nerve palsies 10 days after chickenpox. Cerebrospinal fluid (CSF) analysis done in the second week showed marked lymphocytic pleocytosis. Electroneurographic studies were suggestive of acute inflammatory demyelinating polyradiculopathy. He had near-total neurolog…

Visual loss with papilledema in Guillain-Barre syndrome.

Papilledema and raised intracranial pressure have been reported in association with Guillain-Barre syndrome. Papilledema is usually asympotomatic or associated with mild visual field defects, without any visual loss. The cerebrospinal fluid protein is usually reported to be high. A case of a 35 year old lady is reported, who presented with headache, diplopia and progressive visual loss in both eyes and limb weakness with hyporeflexia. Optic fundus examination showed bilateral papilledema. She had features of pseudotumor cerebri. Nerve conduction studies were suggestive of polyradiculopathy. Th…

Disseminated Infection With Mycobacterium genavense in the Setting of HIV Infection Misdiagnosed as Sarcoidosis.

Mycobacterium genavense was first identified in a patient with HIV. Here, we describe a 40-year-old man with prolonged fever and mediastinal … ency disorder, which ultimately resulted in an HIV diagnosis. Concurrently, the patient experienced polyradiculopathy caused by cytomegalovirus. This case highlights that after identifying a granuloma in tissue, a comprehensive investigation to exclude infectious causes using microbiological and molecular methods is crucial.

Guillain-Barre Syndrome Amid Osteosarcoma Treatment: A Therapeutic Dilemma and Literature Review.

Guillain-Barre syndrome (GBS) is a clinical syndrome with multiple variants. GBS is defined as an acute demyelinating polyneuropathy commonly preceded by infection (bacterial or viral), trauma, or inflammatory processes, which triggers an autoimmune response that affects the peripheral nervous system. This case report describes a patient with high-grade osteosarcoma that completed neoadjuvant chemotherapy and underwent surgical resection with no immediate complications. Fourteen days after the surgery, the patient developed an acute inflammatory demyelinating polyradiculopathy consistent with …

Disorders of the cauda equina.

PURPOSE OF REVIEW: Cauda equina syndrome is an important neurologic disorder characterized by lower back pain, sciatica, perineal numbness, and sphincter dysfunction. This article reviews the anatomy, clinical presentation, evaluation, and treatment of cauda equina dysfunction, focusing on diskogenic cauda equina syndrome. RECENT FINDINGS: Assessment of suspected cauda equina syndrome is hampered by modest diagnostic accuracy of any one clinical feature. Although urgent operation for diskogenic cauda equina syndrome is standard practice, most data about timing of intervention comes from small
