Sleep time and efficiency in patients undergoing laboratory-based polysomnography.

STUDY OBJECTIVES: Sleep quality in patients studied with laboratory-based polysomnography may differ from sleep quality in patients studied at home but remains clinically relevant and important to describe. We assessed objective sleep quality and explored factors associated with poor sleep in patients undergoing laboratory-based polysomnography. METHODS: We reviewed diagnostic polysomnography studies from a 10-year period at a single sleep center. Total sleep time (TST) and sleep efficiency (SE) were assessed as markers of sleep quality. Poor sleep was defined as TST ≤ 4 hours or SE ≤ 50%. Mu…

Polysomnography Outcomes after Supraglottoplasty in Children with Obstructive Sleep Apnea.

OBJECTIVE: Laryngomalacia has been reported to contribute to the severity of obstructive sleep apnea (OSA) in children. It is unclear if surgical treatment of laryngomalacia improves polysomnography (PSG) outcomes in these patients. The objective of this study is to report the impact of supraglottoplasty on PSG parameters in children with laryngomalacia-related OSA. STUDY DESIGN: Retrospective case series. SETTING: Tertiary care medical center. SUBJECTS AND METHODS: Historical cohort study of consecutive children with laryngomalacia who underwent supraglottoplasty and who had undergone over…

Quantifying the ventilatory control contribution to sleep apnoea using polysomnography.

Elevated loop gain, consequent to hypersensitive ventilatory control, is a primary nonanatomical cause of obstructive sleep apnoea (OSA) but it is not possible to quantify this in the clinic. Here we provide a novel method to estimate loop gain in OSA patients using routine clinical polysomnography alone. We use the concept that spontaneous ventilatory fluctuations due to apnoeas/hypopnoeas (disturbance) result in opposing changes in ventilatory drive (response) as determined by loop gain (response/disturbance). Fitting a simple ventilatory control model (including chemical and arousal contrib…

Polysomnography vs self-reported measures in patients with sleep apnea.

BACKGROUND: While obstructive sleep apnea syndrome is defined by both polysomnographic (PSG) abnormalities and symptoms, severity is quantified primarily by the apnea-hypopnea index (AHI) alone. OBJECTIVE: To determine the correlation between standard PSG indices (AHI and others) and self-reported sleepiness, mental health status, and general health in patients with sleep apnea. DESIGN: Cross-sectional study. SETTING: University-affiliated outpatient sleep laboratory. PATIENTS: Ninety-six consecutive patients with PSG-confirmed sleep apnea (AHI >or=5). MEASUREMENTS: Patients completed a q

[Validation of half-night polysomnography for sleep apnea/hypoapnea syndrome].

UNLABELLED: A diagnosis of sleep apnea/hypopnea syndrome (SAHS) is based on clinical signs and nighttime polysomnograms. Brief polysomnography has been proposed as an alternative to all-night recording. OBJECTIVES: The aim of this study was to determine whether a polysomnograms obtained during the first half of the night is sufficient for establishing a diagnosis of SAHS and to determine the correlation between polysomnographic variables recorded during the first four hours (half the study time) with those recorded over the full eight hours (full study time), as well as to determine diagnosti…

Social Determinants of Preoperative Polysomnography and Surgical Outcomes in Pediatric Tonsillectomy.

OBJECTIVE: To evaluate how sociodemographic and neighborhood factors are associated with use of preoperative polysomnography (PSG) in children undergoing tonsillectomy, and to assess whether these factors are associated with return for postoperative PSG, surgical success, or postoperative complications. METHODS: We analyzed 940 children undergoing tonsillectomy at a tertiary children's hospital (2016-2024). Multivariable logistic regression estimated associations between sociodemographic factors (race, ethnicity, insurance, language), neighborhood opportunity (Child Opportunity Index [COI]), …

Role of Polysomnography in Tracheostomy Decannulation in Neuromuscular Disease: A Case Report.

Tracheostomy decannulation in patients with neuromuscular disease presents significant challenges due to impaired airway clearance, pharyngolaryngeal hypotonia, and chronic hypoventilation. Polysomnography (PSG) may provide an objective assessment of ventilatory stability and support safe decision-making. We describe a 54-year-old woman with sequelae of Arnold-Chiari malformation type I, syringomyelia, and tetraplegia (AIS D, neurological level C1), who developed chronic respiratory insufficiency requiring tracheostomy. Despite achieving nocturnal normocapnia with non-invasive ventilation, she…

Tolerance of polysomnography in children with neurodevelopmental disorders compared to neurotypical peers.

STUDY OBJECTIVES: Diagnostic polysomnography (PSG) is the gold standard test to evaluate sleep-disordered breathing in children. Little is known about how children with neurodevelopmental disorders (NDD) tolerate electrodes and sensors in PSG compared to neurotypical children. METHODS: In this retrospective cohort study of children > 12 months of age who underwent diagnostic PSG at our center from 01/01/2021-30/06/2021, we used sleep technician and physician reports to determine how PSG was tolerated in children with NDD compared to neurotypical children. Subanalyses included tolerance of ind…

Is camera recording crucial for the correct diagnosis of sleep bruxism in polysomnography?

Due to potential issues, several sleep laboratories conduct sleep bruxism diagnosis without the use of a camera, instead relying mostly on e … he bruxism episode index value in the analysis of noncamera recordings. In conclusion, camera-based polysomnography still appears to be the most reliable way to assess the severity of sleep bruxism, particularly in patients who also report other sleep disturbances. Predictors of in-laboratory polysomnography attendance in a cohort of patients with stroke or TIA.

STUDY OBJECTIVES: After cerebrovascular events, obstructive sleep apnea (OSA) is associated with poor functional outcomes, an increased risk of recurrent stroke, and mortality. Although the significant under-diagnosis of OSA is likely multifactorial in nature, factors associated with attendance at in-laboratory polysomnography (iPSG) are poorly understood. We evaluated demographic, lifestyle, and clinical variables that predicted attendance at iPSG in a cohort of patients with stroke and transient ischemic attack (TIA) enrolled in a clinical research study. METHODS: Demographic, lifestyle, an…

Polysomnography-Detected Bruxism in Children is Associated With Somatic Complaints But Not Anxiety.

STUDY OBJECTIVES: Sleep bruxism (SB) is common in children and is associated with somatic symptoms and sleep disturbance. Etiological theories posit the role of anxiety, suggesting youth with anxiety disorders may be at high risk for SB, but empirical data are lacking. Furthermore, parent report rather than polysomnography (PSG) has been used to examine SB-anxiety relationships in children. We examined rates of PSG-detected compared to parent-reported SB in children with generalized anxiety disorder (GAD) and healthy controls. Associations among SB, somatic complaints, and sleep disturbance we…

A 6-Month-Old Infant With Different Capnography Values in Polysomnography.

A 6-month-old infant with a past medical history of hypoxic ischemic encephalopathy was referred for evaluation of snoring. She was born at 41 weeks' gestational age to a 25-year-old gravida 1, para 1 mother via vacuum-assisted delivery due to cardiac decelerations. The infant's Apgar scores were 1, 4, and 6 with nuchal cord and meconium at delivery. She was started on positive-pressure ventilation but eventually required intubation at approximately 40 minutes of life. Brain MRI showed abnormal areas of restricted diffusion, involving the corpus callosum, bilateral posterior limb of the intern
