BACKGROUND: Post-dural puncture headache (PDPH) is a common complication of lumbar puncture caused by cerebrospinal fluid leakage and intracranial hypotension. Epidural blood patch (EBP) is considered the gold standard treatment but remains invasive. Sphenopalatine ganglion block (SPGB), a non-invasive technique, has shown promising results in recent studies, but it was not evaluated as an alternative of EBP. This study aimed to evaluate the effectiveness of SPGB in PDPH by determining whether it could reduce the need for EBP in clinical practice.
METHODS: We conducted a prospective monocentr…

ObjectivesEpidural blood patch (EBP) is the current standard of care for postdural puncture headache (PDPH). However, when EBP fails to provide relief, patients may experience discomfort and functional impairments. This study reports the safety and efficacy of a novel approach that uses autologous platelet-rich fibrin (PRF) as an alternative epidural patch.MethodsSixty-seven patients with persistent PDPH symptoms after conservative medical treatment were screened. Among them, 12 (18%) patients underwent multiple EBP (range: 1-6), which failed to resolve the PDPH symptoms. As an alternative met

Post-dural puncture headache is one of the most undesirable complications of spinal anaesthesia. Previous pairwise meta-analyses have either compared groups of needles or ranked individual needles based on the pooled incidence of post-dural puncture headache. These analyses have suggested both the gauge and needle tip design as risk-factors, but failed to provide an unbiased comparison of individual needles. This network meta-analysis compared the odds of post-dural puncture headache with needles of varying gauge and tip design. We searched randomised controlled trials in medical databases. Th…

BACKGROUND: Post-dural puncture headache (PDPH) is one of the most common complications of neuraxial anaesthesia. It limits patients' general activity and increases the length of hospital stays and the use of care. It is particularly disabling during the postpartum period, when mothers have to take care of their child. Epidural blood patch is the standard treatment for PDPH. However, it is an invasive procedure that may result in rare but serious complications. Recent evidence has suggested that adrenocorticotropic hormone (ACTH) is effective in the management of PDPH. The aim of this study is…

OBJECTIVES: We evaluated post dural puncture headache (PDPH) cases and the contributing factors and monitored the treatment response with epidural blood patch (EBP), and other therapies.
METHODS: In this retrospective study, 77 PDPH cases treated with EBP were included. Patients were evaluated in terms of age, gender, type of surgery, type and diameter of the needle used for dural puncture, number(s) of dural puncture, onset of punctural headache, any conservative therapies for PDPH, number(s) of EBP application, analgesia quality at the 10th minute and 2nd hour after EBP application, and rad

INTRODUCTION: Post-dural puncture headache (PDPH) is a well-known consequence of neuraxial anesthesia that can impede patient recovery and delay early discharge. Traditional remedies include hydration and the administration of simple analgesics for symptom relief. When symptoms persist despite conservative interventions, an epidural blood patch (EBP) is typically recommended. However, this invasive procedure carries risks and complications. Our case report aims to explore a potential alternative treatment for PDPH.
CASE PRESENTATION: We present the case of a 22-year-old female who experienced…

PURPOSE: One of the most exhausting complications of spinal anesthesia, especially in parturients, is post-dural puncture headache (PDPH). This headache is not responsive to the usual pain killers. Ondansetron is a 5-HT3 receptor antagonist which is generally used for the prophylactic management of nausea and vomiting; however, studies have found that ondansetron might decrease the incidence of PDPH. Therefore, we aimed to evalute the effect of ondansetron on decreasing the incidence of PDPH.
METHODS: In this double-blind randomized placebo-controlled clinical trial, 210 parturients who under…

BACKGROUND: Post-dural puncture headache (PDPH) remains one of the most distressing complications of spinal anaesthesia, particularly relevant in obstetric anaesthesia where subarachnoid block is commonly employed for caesarean sections. This study aimed to determine the incidence of PDPH following subarachnoid block in patients undergoing lower segment caesarean section (LSCS) and identify associated risk factors in a tertiary care hospital in Eastern India.
METHODOLOGY: This prospective observational study was conducted over 18 months (January 2019 to June 2020) at Tata Main Hospital, Jamsh…

Background: Epidural blood patch (EBP) remains the standard procedural treatment for post-dural puncture headache (PDPH) persisting after conservative care; whereas, the evidence for greater occipital nerve block (GONB) is limited. This study describes pain trajectories during GONB-based management and factors associated with repeated GONB. Methods: Single-centre retrospective observational cohort of all consecutive patients treated for PDPH between January 2016 and January 2018. Eligible patients met the current ICHD-3 criteria after spinal anaesthesia, had a Numerical Rating Scale (NRS) scor…

Lumbar puncture (LP) to extract cerebral spinal fluid (CSF) to quantify analytes relevant to brain health can lead to post-dural puncture headache (PDPH). Incidences vary across studies. This prospective memory clinic study assessed the incidence and intensity of PDPH after protocolised extraction and post-procedural rest. Patient and procedural risk factors for PDPH were explored. In 724 patients or healthy volunteers we performed 1147 procedures with intravenous infusion of 500 ml of 0.9% saline, LP with 22-gauge non-cutting pencil point needles, CSF collection (13.5-15 ml) without negative …

We report a case of prolonged post-dural puncture headache (PDPH) in a patient with panic disorder. A 41-year-old woman received spinal anesthesia for interstitial cystitis. She noticed headache after surgery but did not report it to her doctor. As her headache worsened, she was readmitted to the hospital and diagnosed with PDPH 1 month after surgery. She had panic disorder, controlled by treatment with a selective serotonin reuptake inhibitor, sertraline. Conservative treatments were performed for 1 week, but her headache persisted. Successful resolution of PDPH was achieved following two epi…

OBJECTIVE: The aim of the study was to evaluate the incidence of side-effects in patients bearing a high risk of post dural puncture headache (PDPH) when a spinal anaesthesia was performed. This included outpatients, patients for sectio caesarea and patients younger than 40 years who were mobilized as soon as the surgeon agreed.
METHODS: Quality control without randomization. Spinal anaesthesia for sectio caesarea was applied with a G27 Whitacre needle with the patient in the right lateral decubitus position. Hyperbaric Bupivacain 2-2.2 ml (10-12 mg) was injected when spontaneous flow of spin

OBJECTIVE: This study aimed to assess magnetic resonance imaging (MRI) findings of the lumbar spine in patients with chronic post-dural-puncture headache.
BACKGROUND: Post-dural-puncture headache is a common complication after dural puncture usually resolving within 14 days. However, a chronic course (>14 days persistence) is increasingly being recognized and may lead to tremendous symptom burden. While current research shows that MRI of the head is mostly unremarkable in chronic post-dural-puncture headache, lumbar imaging signs have not yet been systematically studied.
METHODS: This is a r

Post-dural puncture headache (PDPH) following epidural anesthesia remains a frequent obstetric complication. While epidural blood patch (EBP) is the reference treatment, its efficacy assessment still relies on subjective pain reporting. This study explored whether changes in optic nerve sheath diameter (ONSD) measured by ultrasound could serve as an objective, real-time indicator of early EBP response. In this prospective, single-center observational study, 30 postpartum patients with PDPH undergoing EBP had ONSD measured before (H0) and 2 h after (H2) the procedure. Correlation between ΔONSD …

Spinal anesthesia using a pencil-point needle in older patients is a commonly performed procedure. Post-dural puncture headache (PDPH) is a complication that can happen following spinal anesthesia due to cerebrospinal fluid (CSF) leakage, but its incidence in elderly patients is rare. While PDPH presents with typical symptoms and resolves with conservative management, in older patients, it can happen along with intracranial subdural hematoma (SDH), a rare but potentially life-threatening complication due to the persistent CSF leak. We report a rare case of a 73-year-old woman who presented wit…

INTRODUCTION: Post-dural puncture headache (PDPH) is a common complication of diagnostic lumbar puncture (LP), often leading to extended hospitalization and additional medication use. Clinical studies have shown that anodal transcranial direct current stimulation (a-tDCS) is effective against migraine, and thus we decided to assess whether a-tDCS was also effective in treating and preventing PDPH.
METHODS: In two independent, randomized, monocentric controlled trials (RCTs), we enrolled 97 hospitalized participants who underwent LP for diagnostic purposes. Patients were randomized to receive …
