Malaria is a major cause of death in low-income countries. Malaria relapses are caused by Plasmodium vivax-induced latent liver stage hypnozoites, and relapses contribute significantly to the total disease burden. Pneumocystis pneumonia is an increasingly recognized threat in non-HIV immunosuppressed patients and is associated with worse outcomes compared to HIV-infected patients. The preferred first line treatment is trimethoprim-sulfamethoxazole; however, second line treatments for those intolerant of this regimen have been primarily studied in patients with acquired immunodeficiency syndrome (AIDS). We found a program of intravenous and subsequent oral clindamycin, combined with oral primaquine, to be effective for Pneumocystis carinii pneumonia in nine patients with AIDS. The pneumonias were either primary or recurrent and sometimes severe, with cavity formation and/or pneumothorax. In this study, we built on the known inhibitory potential of diaminoquinazolines (DAQs) against different stages of Plasmodium development and designed a convenient two-step synthesis to combine DAQ with primaquine (PQ) pharmacophore.
