Employment, income, and education and risk of postpartum depression: the Osaka Maternal and Child Health Study.

BACKGROUND: Epidemiological evidence regarding the associations of employment, income, and education with the risk of postpartum depression is inconsistent. METHODS: Subjects were 771 Japanese women. Postpartum depression was defined as present when subjects had an Edinburgh Postnatal Depression Scale score of 9 or higher between 3 and 4 months postpartum. Adjustment was made for age, gestation, parity, cigarette smoking, family st

[The healthcare professional's encounter with the seriously ill and dying patient in the hospital].

Seriously ill patients often fear not death but dying in pain and solitude. This review emphasises setting treatment ceilings and prioritising palliation over unnecessary interventions. Such discussions are best held in calm settings but can be challenging in acute situations. Effective care addresses the physical, psychological, social, and existential needs of individuals. Family members play a key role and require clear communication and empathy. Pharmacological relief primarily includes opioids and benzodiazepines, aiming to ensure a dignified end of life and support for loved ones.

Drawing attention to difference: dilemmas in discussing dysmorphism with parents.

Dysmorphism is a sensitive topic to discuss with parents. Discussing dysmorphism requires careful thought, awareness of parent reactions, practice and reflection. Clinical practice suggests that barriers exist for both parents and health professionals, which make these discussions challenging. Here, we explored the challenges inherent in dialogue about dysmorphism and offer suggestions for communication strategies that are grounded in both theory and practice.

Fostering health in the foster care maze.

The fastest growing population of children in foster care today is quite young, and many of these children have significant health care needs. The General Accounting Office (GAO) reported that children in foster care "are among the most vulnerable individuals in the welfare population" (GAO, 1995, p. 1). Poverty, increased homelessness, substance abuse, and a rise in the incidence of persons with HIV all contribute to the problems faced by these children. The Caring Communities for Children in Foster Care Project, funded by the Maternal Child Health Bureau Integrated Services Medical Home Init

Shades of truth: cultural and psychological factors affecting communication in pediatric palliative care.

Communication with children who have life-threatening illnesses is a major challenge. Communication practices are greatly influenced by factors such as the child's age, the parents' wishes, and the cultural norms. This article presents the case of a 12-year-old Japanese boy with advanced hepatoblastoma. The patient also was diagnosed with Asperger's syndrome, which impairs interpersonal communication. The case is discussed from the perspective of clinical ethics, especially with regard to truth telling. The health care team faced an ethical dilemma because of the complications involved. Physic

Chronic sorrow in families of disabled children.

Among professionals who work with disabled children and their families, the need has arisen for better articulation of the concept of chronic sorrow, a bereavement process parents experience in response to life with a disabled child. This article presents information and concepts about the nature of chronic sorrow that permit clarification and understanding of the meaning of what is otherwise often confusing parent behavior. Implications for the professional in assisting the parent's movement through the bereavement process will be discussed.

Communication and end-of-life care in dementia: insights from a close family member working in healthcare.

There is very little documented evidence on the experiences of healthcare professionals caring for their own family outside of their role. This article explores the differences in the care received as the author shares her perspectives of caring for her mother, who was diagnosed with dementia. The author reflects on the care received and the impact of communication, as well as the ethical limitations of caring for a member of her own family. Significant references to communication barriers already exist within the literature, relating to end-of-life conversations. The presence of a relative wh

Enhancing the health professional's role in requesting transplant organs.

The shortfall in organs for transplant continues in the UK. To address this problem, methods of organ procurement are continuously widening with the recent development of protocols in elective ventilation and non-heart beating donors. Until recently, the nurse's role in the success of organ procurement was largely limited to those working in intensive care units involved in cadaveric transplant and community-based nurses working with patients on kidney dialysis who may become involved with live related transplant. Involvement in organ procurement has now extended to nurses working in general w
