'I am not your friend': contract in counselling and psychotherapy.

The largest professional body of counsellors in Australia defines the relationship between counsellor and client as contractual 'and nothing else'. While contract law has been traditionally associated with 'rational', arms-length, commercial relationships, counselling and psychotherapy have been associated with intimate, emotional relationships, work, and exchange. What is the significance, therefore, of these seemingly dissonant discourses coming together in the counselling and therapeutic relationship? Is there a place for formal and distant contract law in the flexible and intimate therapy

Relationship-undermining statements by psychotherapists with clients who present with marital or couple problems.

This study examined the prevalence of relationship undermining statements by psychotherapists as reported by clients in individual therapy who presented with relationship problems, and whether these statements were associated with worse outcomes for client relationships. Participants (n = 101) reported on recollections of whether their therapist had suggested that their partner would never change, had a diagnosable personality/mental health disorder, had negative motives, that the relationship was doomed from the start or beyond repair now, or that divorce/breakup was their best option. Managing Romantic and Sexual Feelings Towards Clients in the Psychotherapy Room in Flanders (Belgium).

It is important that therapists manage adequately their romantic and sexual feelings toward clients as it can negatively affect the psychotherapeutic relationship and may even pose a risk of sexual abuse. This study explores how psychotherapists in Flanders (Belgium) manage such feelings, by conducting both a survey (using 105 of 786 respondents for analyses, as they reported romantic feelings) and focus groups (with a total of 36 participants). Results show that most therapists never consider starting a romantic relationship with a client. They reflect profoundly on their feelings, dwell on p

Introduction: The transformative powers of aesthetic experiences in psychotherapy.

This issue of the Journal of Clinical Psychology: In Session contains seven essays that give expression to three basic convictions. The first is that therapists who are consistently able to help their patients make constructive changes in their lives practice psychotherapy creatively, whether or not they conceive of therapy as an applied science or as an art form. The second is that cultivating an aesthetic perspective on the communicative exchanges that take place in therapy can enhance a therapist's capacity to serve creatively as an agent of change. The third is that therapists can make bet

Pain catastrophizing in dental patients: implications for treatment management.

BACKGROUND: "Pain catastrophizing" refers to an exaggerated negative mental set brought to bear during an actual or anticipated painful experience. A patient's perception of a dental care experience as catastrophic can result not only in poor satisfaction with the therapy but also in avoidance of necessary treatments, resulting in the deterioration of oral health. METHODS: The author reviewed literature regarding pain catastrophizing regarding dental treatment as well as behavioral models related to catastrophizing. RESULTS: People who catastrophize show excessive attention to pain (ruminati

Aphasia therapists' stories of ending the therapeutic relationship.

This article uses the medium of clinicians' comments and stories to explore their perceptions of therapeutic relationships and how these relationships come to a close at discharge from aphasia therapy. These narratives are drawn from a qualitative, grounded theory study carried out in South Australia and Northern Territory involving semi-structured interviews with 30 speech pathologists. The stories reported here shed light on the complexities of negotiating boundaries and endings to therapeutic relationships. Interviewees discussed the special nature of their relationships with their clients

Therapists and quasi-therapists in a therapy analogue situation.

Evaluated an assumption from previous psychotherapy analogue studies, namely, that trained psychotherapists and untrained quasi-therapists with personality similarities will respond similarly to patient presentations. Ss were 24 male psychotherapists and 24 male undergraduate students, chosen partially on the basis of their responses to an A-B personality scale derived from the work of Whitehorn and Betz (1954). The three-factor experimental design included S personality type (A or B), S professional status (psychotherapist vs. student), and videotaped patient simulations (neurotic vs. schizoi

Shared decision-making, the working alliance, and patient-centered care: A simultaneous concept analysis and review of the literature.

OBJECTIVE: To identify the conceptual similarities, differences, and interrelationships between shared decision-making (SDM), the working alliance, and patient-centered care (PCC) in primary care. METHODS: This study is a simultaneous concept analysis based on the method of Walker and Avant (2005) and Haase et al. (1992). First, a systematic search was conducted for articles published between 2012 and 2022 to identify those describing definitions, antecedents, attributes, consequences, and empirical referents of any of the three concepts. Then, data was extracted and organized into a validity

Communicating about obesity and weight-related topics with children with a physical disability and their families: spina bifida as an example.

PURPOSE: The purpose of this study was to explore the experiences of children with spina bifida (SB), their families and healthcare professionals (HCPs) when discussing weight-related topics. METHOD: In-depth qualitative interviews were conducted with HCPs from Canadian outpatient SB clinics (n = 13), children aged 6-18 years with SB (n = 17) and their parents (n = 20). Data were analyzed using a phenomenological approach within an interpretative paradigm. RESULTS: Many HCPs were not confident talking about weight, concerned that they would damage relationships with children and families. Pa

Unpacking the concept of patient satisfaction: a feminist analysis.

AIM: The aim of this paper is to present a feminist critique of the concept of patient satisfaction. BACKGROUND: Fiscal restraint, health care restructuring, shifting demographics, biomedical technological advances, and a significant shortage of health care professionals are stretching health care systems across North America to the breaking point. A simultaneous focus on consumerism and health service accountability is placing additional pressure on the system. The concept of patient satisfaction, with roots in the consumer movement of the 1960s, has both practical and political relevance in

[Abuse in healthcare - lessons learned during two decades of research].

Patients' experience of abuse in health care (AHC) is common, and may cause long-lasting suffering. In Sweden, lifetime prevalence is estimated at 20 per cent among female patients and 8 per cent among male patients, and a background of other abuse is a risk factor. Most health care staff have experience of patients who have been abused, but the topic is surrounded by silence from both patients and caregivers. Models for training staff in groups have been evaluated and a more proactive stance can be fostered. Department heads and health care authorities are responsible for providing staff with
