The evolving understanding of prurigo nodularis.

Prurigo nodularis (PN) is a chronic, highly pruritic inflammatory skin disease that, until recently, was poorly understood. PN was first described in the late 1800s, but awareness of the disease remained low, probably because of its low prevalence and resemblance to other diseases. Interest in the research and therapeutic advancement of PN has grown in recent years, leading to a greater understanding by the medical community of its natural history, pathophysiology, diagnosis and treatment. We describe how our understanding of PN has evolved in recent years and how current therapeutic advanceme…

Immunotargets and Therapy for Prurigo Nodularis.

Prurigo nodularis is a chronic inflammatory skin disease consisting of severely pruritic nodules that can be very debilitating for patients. The basis of this skin condition is immunological dysregulation and neural amplification, driven by T-lymphocytes, mast cells, eosinophilic granulocytes, macrophages, and cytokines mediating itchy processes. Further complicating this already taxing diagnosis is the lack of approved treatment and consensus on management; although there are off-label treatments utilized as therapy. Immunomodulators are the cornerstone of treatment for PN, and additional nov…

Prevalence of Prurigo Nodularis in Poland.

Prurigo nodularis is a chronic skin condition that presents significant challenges for treatment and quality of life, with little known about disease prevalence. This study assessed the prevalence of prurigo nodularis in Poland using publicly maintained database case records and diagnostic codes for prurigo nodularis (L28.1) and other prurigo (L28.2). For the period 2016-18, the prevalence of prurigo nodularis increased from 5.82 to 6.52 cases per 100,000 population. The total prevalence of all prurigo diagnoses over the same period remained relatively stable, with a small increase from 9.04 t…

Treatment of prurigo nodularis with lenalidomide.

Prurigo nodularis (PN) is an intensely pruriginous dermatological disorder whose treatment is challenging for dermatologists. It is characterized by eruptions of papules and hyperkeratotic nodules, some of which are eroded, on the extensor surfaces of the limbs. The most commonly used treatments for this condition are oral antihistamines and topical or systemic steroids. Thalidomide is an effective treatment option in cases of recalcitrant PN; however, its most frequent adverse effect is neurotoxicity, which often results in its discontinuation. Lenalidomide is an analogue of thalidomide that …

The conjunctivitis of solar (actinic) prurigo.

Solar (actinic) prurigo (SP) is an abnormal reaction to sunlight that affects mostly the Indian and mestizo populations of America, and is well known in Canada and the United States. It is quite common in Mexico, Central, and South America, although rare in Europe. It usually starts in childhood in both sexes and in about 30-50% of cases involves the oral and ocular mucosa. In this study we present the clinical and histopathologic conjunctival findings in 11 of 105 children with SP. Clinically the most important features were photophobia, some degree of pterygium, and pinguecula, hyperemia, Tr…

The epidemiology of prurigo nodularis in Belgium.

Prurigo nodularis (PN) is a rare, severely pruritic chronic skin disease, which seriously reduces patients' quality of life. PN prevalence in Belgium is unknown. To investigate PN prevalence, patient profile, diagnosis, and treatment strategies in Belgium. These insights will better support patient identification and treatments choice to address their specific needs. An anonymous questionnaire was sent to 800 dermatologists for data collection; 65 dermatologists (8%) completed the survey. Also, a search query in the electronic medical records system of a Belgian university hospital was perform…

Dupilumab for the Treatment of Prurigo Nodularis.

Prurigo nodularis (PN) is a chronic inflammatory skin condition characterized by the presence of pruritic nodules. Dupilumab was approved by the US Food and Drug Administration in September 2022 and Health Canada in July 2023 for the treatment of PN. Dupilumab is a human monoclonal immunoglobulin G4 antibody that binds the interleukin (IL)-4 receptor alpha subunit, blocking intercellular signalling of IL-4 and IL-13. Inhibition of these cytokines downregulates the inflammatory response and improves disease severity and pruritus. Two randomized controlled trials have shown dupilumab to be effec…

[Update on the treatment of chronic prurigo].

Chronic prurigo is an inflammatory dermatosis defined by the presence of chronic pruritus and single to multiple symmetrically distributed pruriginous lesions such as nodules, papules, and plaques. Various dermatological, systemic, neurological, and/or psychiatric diseases are associated with chronic prurigo. The care of these patients is very complex due to the multifactorial character and also because of the sometimes very pronounced consequences such as an impairment of quality of life with sleep disorders. Furthermore, there are no approved therapies. The current guideline-based treatment …

Treatment of prurigo nodularis with pregabalin.

WHAT IS KNOWN AND OBJECTIVE: Prurigo nodularis (PN) is a chronic skin condition that is difficult to treat. Pregabalin is one of the possible treatments for PN but its safety and efficacy are not well defined. We aimed to assess the efficacy of pregabalin in patients with PN. METHODS: Thirty patients (10 men, 20 women; mean age 51.6 ± 9.39 years) were treated with pregabalin (75 mg/day) for 3 months. Efficacy was classified as (i) successful (disappearance of the pruritus and reduction of nodules); (ii) slight improvement/reduction of the nodules, that is, number and/or flattening, no disappe…
