Altered motor activity is a frequently observed symptom in many psychiatric disorders. It is an important diagnostic parameter in mania, depression, ADHD and others. The method which allows measuring motor activity precisely and objectively is actometry. An actometer is a small, electronic device measuring body movements. This paper is a review of psychiatric research in which actometry was used: sleep disorders, ADHD, schizophrenia, anorexia nervosa, affective disorders and also in chronobiology and psychopharmacology. Methodology of actometric research and limitations of the method are discu

This paper discusses the current practice of using single word labels for emotional states that ignore the nature of the incentive, cognitive representations, physiologic profile, and especially the developmental stage of the agent. The universal cognitive advances that are derivatives of brain maturation over the first dozen years are accompanied by significant emotional states that infants and young children cannot experience. The cognitive transitions at the end of the first year, the middle of the second year, 5-7 years and early adolescence have special implications for affect states.

Orbitofrontal syndrome is a variant of frontal lobe syndrome in which behavioural disturbances are prevailing. It results from bilateral lesions of the orbitofrontal cortex and the medial face of frontal lobe. Patients present disorganized hyperactivity. They are distractable, impulsive, euphoric and unable to abide by social rules. They often have instinctive disinhibition (hypersexuality, hyperphagia and urinary behaviour disorders). In spite of severe behavioural disturbances cognitive functions are often intact so that orbitofrontal syndrome may be confounded with two psychiatric disorders

This paper discusses the problems of Japanese under- and postgraduate education of psychiatry, and indicates several important points for psychiatric education. Most medical schools give lectures on psychiatry to fourth and fifth graders. Lecturing hours average 2.3% of the total lecture time for medical students. However, the hours have tended to decrease recently, and lectures by the Department of Psychiatry are spreading to the first and second graders. These lectures include behavioral science, psychology and science for human relationships. The following problems should be solved urgently

The classifications currently used in psychiatry have different aims: to facilitate communication between researchers and clinicians at national and international levels through the use of a common language, or at least a clearly and precisely defined nomenclature; to provide a nosographical reference system which can be used in practice (diagnosis, prognosis, treatment); to optimize research by ensuring that sample cases are as homogeneous as possible; to facilitate statistical records for public health institutions. A classification is of practical interest only if it is reliable, valid and

The accidental separation of PTSD and the dissociative disorders, based on the unfortunate fact that the PTSD and the dissociative disorder workgroups of the DSM-III never met to compare data, and the lack of knowledge within the workgroups of existing research connecting these disorders, will continue in the DSM-IV. That separation, however, is slowly being eroded by a solid body of research that shows that these conditions rarely occur independently. Thus, contemporary research is beginning to show that the original concept of "hysteria," formulated 150 years ago to capture a group of patien

Benzodiazepines are generally well tolerated (compared to barbiturates or antidepressants, their side-effects are milder). They may be used safely, their toxicity is low. Benzodiazepine overdosage may be lethal only if the drug is taken simultaneously with other drugs or alcohol. They act primarily through inhibiting the GABA system, their anxiolytic and sedative effects are of primary importance from the psychiatric aspect. Their classification is based on the difference in their receptor affinity (potency) and kinetics. Derivatives of low, medium and high potency are known. The introduction

A 20-year experience of an ophthalmologist at a mental hospital, and the results of examination of over 2500 patients show an important role of ophthalmological examination in psychiatry. It is of definite help in the diagnosis of organic pathology of the brain, an indirect assessment of the status of the cerebral vessels, the conduction of clinico-genetic investigations, therapeutic and rehabilitation help to patients and control of side effects of psychotropic therapy on the eye. The psychic status of patients and the frequency of functional visual disturbances in them require the employment

The stigma associated with psychiatry is the most critical problem facing the profession, the author contends. He identifies the major sources of stigma as psychiatrists' internal conflicts over treatment ideologies and methods and the hostility between psychiatrists and other physicians that has led to disparagement of psychiatry as a medical specialty. To combat the stigma he recommends that highest priority be given to efforts to unify the profession and to increase psychiatry's participation in organized medicine. He also urges psychiatrists to limit self-revelation and self-exploration in

Neurochemical postmortem examination of brain tissue may never be completely replaced as a research tool in psychiatry. This method has already provided support for the hypotheses relating norepinephrine, dopamine, serotonin, peptides, and hemisphere asymmetries to psychiatric syndromes.

Although they have a long history of use, standardized or structured interviews for the gathering of clinical data in psychiatry have become increasingly popular in the last 10 years. Some definitions of structured interviews and a brief historical review of their use are presented. In addition to enhancing the reliability of symptoms and diagnoses in research, there is evidence that structured interviews improve the comprehensiveness of the data base, and reduce examiner bias. Even in the routine clinical setting, completeness of data collection and accuracy of diagnosis would appear to outwe

The complex presentation of the older patient in crisis places unusual demands on psychiatrists. The patients themselves may be unable or unwilling to seek psychiatric evaluation. For various reasons psychiatrists and other physicians may stereotype older patients and their problems. These problems are frequently complex and require sophistication on the part of the examiner. The psychiatrist's approach must be unhurried and flexible. The routine mental status examination must be expanded to include brief psychological testing. As the cases demonstrate, emergencies may present in unexpected co

Recertification raises a number of issues for the subspecialty of child psychiatry. The author discusses double jeopardy (i.e., examination in both the main and subspecialty areas), the use of multiple methodologies for achieving certification, and the problems of attaining evaluation and validity of the recertification process.

An account is given of the first year's experience in providing psychogeriatric service to a large urban community. Patients were attended in their homes, in nursing homes and in Homes for the Aged. There was a tendency for the consultant's help to be sought mainly in cases of "disposal" of difficult patients rather than for treatment of the many who could be expected to benefit from it. However, the advantage has been demonstrated of continuity of service in the variety of settings and, as a result, the more proper placement of patients. Acceptance of the help the psychogeriatrician can contr
