Clinical significance of monitoring plasma levels of psychotropic drugs.

The possible clinical significance of plasma level monitoring of psychotropic drugs in psychotic patients is described. In particular, data obtained with children and 'resistant' psychotic patients are presented. The results in children indicate the existence of a good relationship between side-effects and haloperidol plasma levels as well as between adverse effects and chlorimipramine plasma levels. Furthermore, an age effect on haloperidol clearance could be shown. Relationship between haloperidol and chlorimipramine concentration and effects could be observed for tics and enuresis respectiv…

[Fibromyalgia ans psycho-traumatic stress: psychotropic drugs and psychotherapies].

Fibromyalgia and post-traumatic stress : role of psychotropic drugs and psychotherapies. The literature shows a high frequency of post-traumatic stress disorder (PTSD) in fibromyalgia syndrome and unexplained chronic pain as a comorbidity of PTSD. On the therapeutic level, the care of patients must be done in the benevolence and respect for the suffering of the patient with a therapeutic education for an adapted nutrition, the maintenance of the maximum possible physical activity and a drug management, physiotherapy and psychotherapy such as behavioral and cognitive therapy and hypnosis. Early…

[Bariatric surgery and psychotropic drugs : where are we now ?].

Bariatric surgery is considered an effective treatment against obesity. This intervention and the subsequent weight loss can affect several pharmacokinetic parameters leading to a possible need of dosing adjustment. Despite an increasing number of bariatric interventions, clinical recommendations are often based on theoretical principles and expert opinions. Because reliable clinical and biological parameters for adjusting psychotropic drug are not available, the use of therapeutic drug monitoring needs to be advised as a useful approach. However, it is not currently possible to predict the ef

Psychiatric Disorders and TRP Channels: Focus on Psychotropic Drugs.

Psychiatric and neurological disorders are mostly associated with the changes in neural calcium ion signaling pathways required for activity … tasis in BDs. The TRPV1 channel also plays a function in morphine tolerance and hyperalgesia. Among psychotropic drugs, amitriptyline and capsazepine seem to have protective effects on psychiatric disorders via the TRP channels. Some drugs such as cocaine and methamphetamine also seem to have an important role in alcohol addiction and substance abuse via activation …

Impact of psychotropic drugs on suicide and suicidal behaviors.

OBJECTIVE: To examine the impact of psychotropic drugs on suicide and suicidal behaviors in bipolar disorders. METHODS: A Medline search of articles published from January 1960 to January 2013 was performed using relevant keywords to identify studies examining the relationship of psychotropic drugs to suicidal behaviors. The publications were further reviewed for relevant references and information. Additionally, the US Food and Drug Administration Center for Drug Evaluation Research website was searched. RESULTS: The available studies used differing methodologies, making interpretation of t…

Use of psychotropic drugs in patients with HIV infection.

Psychotropic drugs are frequently employed to treat the wide range of neuropsychiatric syndromes that patients infected with the human immunodeficiency virus (HIV) may develop. In order to administer these agents properly, physicians should take certain factors into account: the central nervous systems of these patients are often impaired, the patients tend to suffer from medical illnesses, and they may be taking various other drugs. The possible interactions between substances taken by these patients may sometimes make it necessary to adjust the dosage of psychotropic agents administered. In …

Basic pharmacokinetic principles and their application to psychotropic drugs.

The application of pharmacokinetic principles to the administration of psychotropic medications provides a rational approach to understanding factors influencing the time course and intensity of drug action. When plasma concentrations of a given medication can be linked to levels at receptors in the brain, the pharmacodynamic effects of that drug can then be predicted. The pharmacokinetic parameter of most interest to clinicians is clearance, which describes the rate of drug removal per unit of plasma concentration. For any given drug, this parameter varies widely from person to person and may

Cytochrome P-450 monooxygenases and interactions of psychotropic drugs.

The authors review drug biotransformation. Phase I reactions are preconjugation processes that take place through cytochrome P-450--a hepatic mixed function oxidase (MFO) enzyme system with multiple isozymes to metabolize foreign substances. Phase II reactions are conjugation processes of the substrates. Based on the characteristics of the cytochrome P-450 microsomal enzyme systems, the authors explain the clinical phenomena of induction and inhibition of these enzymes when psychotropic agents interact. This article describes both MFO inducers (barbiturates) and inhibitors (cimeditine, fluoxet

Bioavailability of psychotropic drugs: historical perspective and pharmacokinetic overview.

The evolution of the federal government's role in the regulation and evaluation of generic psychotropic medications is described. To place many of the methodologic bioequivalence issues for antipsychotic agents into perspective, the pharmacokinetics of these drugs are reviewed. Appropriate methodologies for studying the pharmacokinetics and pharmacodynamics of psychotropic drugs are in early developmental stages. Many of the issues relating to bioequivalence of generic products will not be resolved until a better understanding of these factors is developed.

[Interaction between antihypertensive agents and psychotropic drugs (author's transl)].

The present review paper is dealing with the interaction between antihypertensive agents and various psychotropic drugs. Various psychotropic drugs enhance certain-side-effects of the antihypertensive substances, like sedation, extrapyramidal disorders and orthostatic hypotension. On the other hand, the blood-pressure lowering effect of clonidine, guanethidine and related drugs, and possibly also that of alpha-methy-dopa is reduced by phenothiazine-neuroleptics and tricyclic antidepressants (thymoleptics), but not by benzodiazepine tranquilizers or by butyrophenone-like neuroleptics. The pharm…

Effect of psychotropic drugs on caudate spindle in cats.

To ascertain whether neuroleptics act on the caudate nucleus itself, the effects of these compounds as well as other centrally acting drugs were examined in relation to caudate spindle and EEG arousal responses (sciatic nerve stimulation) in gallamine-immobilized cats. Haloperidol and chlorpromazine enhanced the caudate spindle at a dose which had no effect on the EEG arousal response. On the other hand, clozapine and a higher dose of chlorpromazine enhanced the caudate spindle, but depressed the arousal response. High frequency stimulation of the sciatic nerve suppressed the caudate spindle.

New drug developments in the Netherlands. "Tetracyclic" psychotropic drugs.

Based on qualitative and quantitative EEG findings, the psychotropic properties of GB-94, GC-46, and GC-94 were predicted. According to the EEG model, GB-94 was predicted to be similar to amitriptyline and GC-46 t( imipramine. GC-94 was found to be much less effective on the CNS than GB-94 and GC-46 and was predicted to have "stimulatory" clinical effects similar to desipramine. The clinical trials demonstrated that GB-94 is indeed an antidepressive psychotropic compound similar to amitriptyline. GB-94 in lower dosages was also found to be effective in anxiety syndromes. In some schizophrenics
