Pulmonary embolism masquerading as ST elevation myocardial infarction and ventricular tachycardia.

A middle-aged man with metastatic lung adenocarcinoma presented with acute dyspnoea and chest tightness, triggering a primary percutaneous coronary intervention alert for suspected myocardial infarction. He had a left-sided deep vein thrombosis diagnosed 1 week prior, treated with rivaroxaban and was on day 3 of entrectinib chemotherapy for lung cancer. Examination revealed haemodynamic compromise. Electrocardiography revealed widespread ST elevation (V2-V6, II, III, aVF), but coronary angiography showed unobstructed coronary arteries. Echocardiography and elevated troponin levels suggested ri

Pulmonary Embolism Presenting as ST-Elevation Myocardial Infarction: A Diagnostic Trap.

BACKGROUND The clinical presentation of pulmonary embolism (PE) is highly variable, ranging from no symptoms to shock or sudden death, often making the diagnosis a challenge. An electrocardiogram (EKG) is not a definitive diagnostic tool; however, it can alter the clinical suspicion of acute PE. PE has nonspecific electrocardiographic patterns ranging from a normal EKG in almost 33% of patients to sinus tachycardia, S1Q3T3 pattern (McGinn-White Sign), right axis deviation, and incomplete right bundle branch block (RBBB). ST-segment elevation associated with PE is exceedingly rare, and to date,

[Three cases of right-sided active endocarditis with multiple pulmonary infarction].

We have experienced three patients with right-sided active endocarditis combined with multiple pulmonary infarction. Ventricular septal defect (VSD), aortic regurgitation (AR), tricuspid regurgitation (TR) and congestive heart failure were present in case 1. TR was present in case 2. VSD, TR and patent ductus arteriosus were present in case 3. alpha-Streptococcus caused endocarditis in case 1 and 3; Candida albicans caused endocarditis in case 2. Antibotic therapy had no effect in case 2 and 3. Case 1 and 3 developed pulmonary hemorrhage, which resolved before the operation in case 1, but not …

[Renal infarction following pulmonary surgery in a 13-year-old boy].

We report a case of renal polar infarction in a 13-year old boy following surgical excision of contralateral congenital lobar emphysema. Patient history was unremarkable and yielded no data suggestive of any other underlying pathology or systemic disease. The diagnosis was made on the CT, arteriography and DMSA scan findings. The etiology could not be determined. The patient was treated conservatively with prophylactic antibiotic therapy and followed closely. There were no complications; the patient was asymptomatic one month after the episode. The control IVP showed residual segmental atrophy

Cerebellar infarct with neurogenic pulmonary edema following viper bite.

Russell's viper (Daboia russelli) bites are well known to cause bleeding complications. However, thrombotic complications are rare. We present the case details of a female who was bitten by a Russell's viper (Daboia russelli) in her village. She then developed features of envenomation in the form of hemorrhagic episodes. She received 27 vials of polyvalent anti-snake venom to which the hemorrhagic complications responded. After about 48 h of the bite she developed features of cerebellar infarct along with pulmonary edema which was in all probability neurogenic in origin. She was managed with m

Pulmonary embolism and splenic infarction in a patient with sickle cell trait.

A 43 year-old black man with sickle cell trait documented by hemoglobin electrophoresis presented with severe pleuritic chest pain and hypoxemia three weeks after discharge following abdominal surgery. A pulmonary embolus was diagnosed by angiography and he was treated with heparin; the minimum arterial pO2 was 55 torr while O2 was being administered at a rate of 3 L/min. During this therapy, he developed abdominal pain. Computerized tomography suggested splenic infarction, which was documented by radionuclide liver-spleen scan and magnetic resonance imaging (MRI); the patient's spleen had bee

[Differential diagnosis of pneumonia and pulmonary infarct based on lung scintigraphy].

The etiology of radiologic signs of circumscript parenchymatous lesions is multivarious. In some instances infarction of the lung has to be distinguished from pneumonia. The differential diagnosis has important therapeutic consequences for the patient. An important criterion for primary pneumonic infiltration is Xe-trapping within the area lacking perfusion, which can only be demonstrated by perfusion scintigraphy with Xe-133-NaCl. However, infarction of the lung lacks this pathologic feature. Pathophysiologically the various changes may occur as the result of obstruction of the vessel i.e. du

Spectral CT-derived peripheral iodine concentration in acute pulmonary embolism: association with subsegmental emboli and pulmonary infarction.

OBJECTIVE: Pulmonary infarction (PI) is a recognised complication of acute pulmonary embolism (PE). While computed tomography pulmonary angiography (CTPA) provides high-resolution morphological imaging, it does not permit direct functional assessment of regional lung perfusion. Spectral CT pulmonary angiography (SCTPA) enables quantitative perfusion analysis using iodine maps. However, the relationship between peripheral perfusion impairment, subsegmental emboli (SSE), and PI remains incompletely understood. This study investigated their association using SCTPA. MATERIALS AND METHODS: This re…

Characteristics of pulmonary infarction in patients with acute pulmonary embolism in China: a single-center retrospective observational study.

BACKGROUND: Pulmonary infarction (PI) is an uncommon complication of pulmonary embolism (PE). The risk factors of PI are still relatively unclear. METHODS: This was a single-center retrospective review conducted on 500 patients with PE. After applying the inclusion and exclusion criteria, 386 patients diagnosed with PE were enrolled in our study. These patients were then categorized into the PI group (n=64) and the non-PI group (n=322). A comparison was conducted between the two groups regarding the clinical characteristics. RESULTS: The occurrence of PI secondary to PE was 16.58%. In univar…

Cavitary Pulmonary Infarction Mimicking Malignancy: A Diagnostic Challenge Resolved by Conservative Management.

Cavitating pulmonary infarction is an uncommon complication of pulmonary embolism (PE) that can closely mimic lung malignancy or necrotising infection on imaging. We report the case of a 72-year-old man with a history of heavy smoking who presented with subacute cough, dyspnoea, anorexia, and weight loss. Imaging revealed bilateral PE with right heart strain and a large, thick-walled cavitary lesion in the right lower lobe. Extensive microbiological, cytological, and autoimmune testing did not identify infection, vasculitis, or malignancy. Following multidisciplinary team discussion, the patie…

Acute bilateral renal infarction after left upper pulmonary lobectomy: a case report.

BACKGROUND: Left upper lobectomy (LUL) is considered a risk factor for thrombus formation in the pulmonary vein stump compared with other lobectomies, and it occurs in 11.7-13.5% of cases. Although cerebral infarction after LUL has been reported in many articles, reports of bilateral renal infarction after lobectomy are rare. CASE PRESENTATION: A 70-year-old male patient was admitted to our hospital with a diagnosis of lung cancer. The nodule's diameter was 2.5 cm on computed tomography, and it was diagnosed as a non-small cell lung carcinoma by a bronchoscopic biopsy. The clinical stage was

Post-COVID-19 Pulmonary Infarction Complicated by Spontaneous Pneumothorax: A Case Report.

Pulmonary cavitation is an atypical finding in COVID-19 patients. In this rare case report, a 63-year-old woman (35 days from COVID-19 sympt … ical examination of the resected lung specimen showed a bland (aseptic) cavitary pulmonary infarct. Pulmonary infarction is a rare cause of cavitation in COVID-19 patients, nonetheless, something that should be considered in those presenting with respiratory symptoms or complications during or post-COVID-19.

Neurogenic pulmonary oedema misdiagnosed as acute myocardial infarction in a comatose patient.

INTRODUCTION: We report a case of neurogenic pulmonary oedema (NPO) following massive left cerebral infarct, which was initially misdiagnosed as acute myocardial infarction (AMI). CLINICAL PICTURE: This 52-year-old man presented with acute loss of consciousness with normal brain computed tomography (CT). He was treated as non-ST-elevation AMI complicated with pulmonary oedema based on findings of chest radiograph (bilateral pulmonary oedema), electrocardiogram (marked ST-T changes in leads V3 to V6), and cardiac enzymes [elevated creatinine kinase (CK) and CK-MB]. However, coronary angiogram

Disseminated Nocardia transvalensis infection resembling pulmonary infarction in a liver transplant recipient.

Infections due to Nocardia transvalensis are extremely rare: only four disseminated infections with this pathogen have been reported, three of which ended fatally. This is the first report of a liver transplant recipient with Nocardia transvalensis infection. The patient had disseminated infection with pulmonary involvement, which presented as pulmonary infarction. Despite a ten-day delay in the administration of correct therapy, he responded rapidly to trimethoprim-sulfamethoxazole. The pitfalls of differentiating nocardial infection from pulmonary thromboembolism in solid organ transplant re…

Pulmonary-artery cineangiocardiography to demonstrate cardiac thrombi in patients with cerebral infarction.

The usefulness of pulmonary-artery cineangiocardiography (PACAC) to demonstrate intracardiac thrombi in patients with cerebral infarctions of possible embolic origin has been explored. PACAC was performed in 60 patients (mean age 74 years) with sudden onset of permanent neurological deficits in whom CT scan and CSF analyses had excluded intracerebral hemorrhage. Opacification of the left atrium was excellent in 52 (87%) investigations, acceptable in 7 and poor in 1. Among 33 patients with chronic atrial fibrillation, an atrial thrombus was demonstrated in 8 (24%) and a suspected thrombus in 3.

Simultaneous Onset of Deep Vein Thrombosis, Pulmonary Embolism, Cerebral Infarction, and Myocardial Infarction in a Patient with Patent Foramen Ovale.

Paradoxical embolism is a known complication with intra-cardiac shunts. It should be considered in the differential as the pathophysiologic mechanism of simultaneous thromboembolism in the venous and systemic vasculature. We present a case of simultaneous deep venous thrombosis, pulmonary embolism and myocardial infarction in the presence of a confirmed patent foramen ovale on echocardiography. Thrombolytic therapy was administered. Subsequent concerns of intracranial hemorrhage on imaging of the brain complicated the management and added to the challenge of co-managing the clot burden in our
