Since 1991, quinidine gluconate, a class 1a anti-arrhythmic agent, has been the only parenteral antimalarial available for use in the United States (1). It is indicated for the treatment of patients with life-threatening Plasmodium falciparum malaria (2), including those who cannot tolerate oral therapy, have high-grade parasitemia, or have complications (e.g., cerebral malaria or acute renal failure).

CASE SUMMARY: P. falciparum malaria was diagnosed in a woman visiting from Nigeria. Treatment was instituted with intravenous quinidine given the high level of parasitemia. The ability of quinidine retard of restoring sinus rhythm and further maintenance after successful regulation were checked in 42 patients with flutter and fibrillation in an open clinical trial. Quinidine retard restores the sinus rhythm in less patients (60.0%) than quinidine sulphate (81,8%). At the same time, it maintains sinus rhythm in 91,4 per cent of the cases in anti-recurrence treatment. That makes quinidine retard adequate for long-term administration.

The plasma protein binding of salicylate, a representative acidic drug, and of quinidine, a representative basic drug, has been studied in patients with multiple myeloma, a disease characterized by a profound alteration of the plasma protein pattern. Abnormal binding of salicylate was detected only in patients with high values of circulating abnormal protein. Plasma protein binding of quinidine was not altered in any of the patients studied.

We report an adolescent with a malignant form of Brugada syndrome who presented with 15 episodes of ventricular fibrillation (VF) over 10 days, shortly after implantation of an implantable cardioverter defibrillator. Quinidine was combined with propranolol to restore sinus rhythm in cases of paroxysmal atrial fibrillation (PAF) in 42 patients.
