Radiation meets inflammation: NLRP3 inflammasome at the core of radiation-induced cardiac injury.

Radiation therapy is a widely used and effective treatment for various types of cancer; however, it is associated with several side effects, including an increased risk of cardiovascular disease. While the molecular mechanisms underlying RT-induced cardiovascular damage remain unclear, the NLRP3 inflammasome has emerged as a central player in its pathogenesis. This review focuses on the current understanding of the involvement of NLRP3 in radiation-induced cardiotoxicity, highlighting emerging evidence reported in the literature. Recent findings shown that the NLRP3 inflammasome may play a rol

Radiation injury: imaging findings in the chest, abdomen and pelvis after therapeutic radiation.

Radiation may be used as adjuvant or primary therapy in a variety of tumors in the chest, abdomen and pelvis. Therapeutic radiation affects not only malignant tumors but also surrounding normal tissues. The risk of injury depends on the size, number and frequency of radiation fractions, volume of irradiated tissue, duration of treatment, and method of radiation delivery. Concomitant chemotherapy can act synergistically to produce injury. Other predisposing factors include infection, prior surgery and chronic illness like hypertension, diabetes mellitus and atherosclerosis. Radiation changes va

Single-shot bevacizumab for cerebral radiation injury.

BACKGROUND: Cerebral radiation injury, including subacute radiation reactions and later stage radiation necrosis, is a severe side effect of brain tumor radiotherapy. A protocol of four infusions of the monoclonal antibody bevacizumab has been shown to be a highly effective treatment. However, bevacizumab is costly and can cause severe complications including thrombosis, bleeding and gastrointestinal perforations. METHODS: We performed a retrospective analysis of patients treated in our clinic for cerebral radiation injury who received only a singular treatment with bevacizumab. Single-shot w

Normal tissue tolerance and management of radiation injury.

The objective of effective cancer therapy includes preservation of normal tissue function and reducing injury as much as possible. Acutely responding tissues such as skin and mucous membranes generally show a reaction during the course of radiation therapy. Normally those reactions heal rapidly after radiation therapy is completed. The short-term injury is justified if a reasonable probability of local tumor control is expected with an increase in survival of several months. Late effects are more challenging to manage and, therefore, the probability of occurrence is reduced by the methods of i

Vascular response to radiation injury in the rat lung.

Changes in relative left-to-right lung blood flow ratios were followed as an index of vascular radiation injury in left-hemithorax-irradiated Sprague-Dawley rats. Single doses of 11 to 21 Gy gamma radiation resulted in a dose-dependent decrease in relative blood flow to the irradiated lung from 3 to 5 weeks after exposure during the development of pneumonitis. Blood flow returned to near normal by 5 weeks after lower doses (11-13.5 Gy). After a single dose of 15 Gy the left-to-right blood flow ratio recovered to 75% of normal at 12 weeks and leveled off. Following 18 Gy irradiation a second pe

Ureteral injury following experimental intraoperative radiation.

Beagle dogs were randomized to receive a range of total dose delivered in three different protocols. Sixteen dogs received external beam radiation therapy (EBRT), 32 dogs received intraoperative irradiation (IORT), and 32 dogs received combinations of external beam radiation therapy and intraoperative irradiation. A sublumbar field was irradiated which always included the left ureter. Dogs were observed for 5 years; sequential excretory urograms were done at 6 months, and 1 and 5 years. Morphometric analysis of tissues were also done. The canine ureter tolerated 17.5 Gy intraoperative irradiat

Radiation-induced lung injury: a hypersensitivity pneumonitis?

Radiation pneumonitis occurs 6 to 12 weeks after thoracic irradiation, and is thought to be due to direct radiation-induced lung injury. Four patients who developed pneumonitis after unilateral thoracic irradiation for carcinoma of the breast were studied with bronchoalveolar lavage, gallium scan of the lung, and respiratory function tests. On the irradiated side of the chest, all four patients showed an increase in total cells recovered from the lavage fluid and a marked increase in the percentage of lymphocytes. When results for the unirradiated lung were compared with results for the irradi

Delayed Radiation Injuries (Soft Tissue and Bony Necrosis) and Potential for Future Research.

The application of hyperbaric oxygen (HBO₂) to the treatment and prevention of late radiation injury (LRTI) is the focus for this chapter. Additional miscellaneous topics related to radiation exposure and HBO₂ will also be explored, including areas of interest for research. We will also discuss some of the pertinent literature demonstrating the safety of HBO₂ for the cancer patient. A review of billing records from CMS demonstrates that delayed radiation injuries continue to be the most frequent indication for hyperbaric treatments in the United States, constituting about 40% of all their bill…

Development of PLX-R18, a promising and novel countermeasure for radiation-associated injuries.

PLX-R18 is a novel cell-based product of ex vivo expanded adherent human-placenta-derived stromal cells. After intramuscular administration, these living cells are capable of secreting various cytokines that produce a therapeutic benefit. The endogenously secreted cytokines facilitate the recovery of hematopoietic progenitor cells and regenerate multiple blood lineage cells. Preclinical studies have demonstrated that PLX-R18 cells can prevent and also mitigate hematopoietic acute radiation syndrome in experimental animal models. This agent has an open US Food and Drug Administration investigat

Marasmius androsaceus mitigates depression-exacerbated intestinal radiation injuries through reprogramming hippocampal miRNA expression.

INTRODUCTION: Cancer patients commonly experience high levels of psychological stress, which poses significant risks to their well-being. Radiotherapy is a primary treatment modality for cancer; however, it often leads to intestinal injuries in these patients. Nevertheless, the impact of mental stress on radiotherapy-intertwined complications remains unclear. METHODS: To induce intestinal injury, we employed total abdominal irradiation in our experimental model. We conducted high-throughput sequencing to analyze the expression profile of miRNAs in the hippocampus. RESULTS: We observed that m

Prevention of work-related injuries and diseases: lessons from experience with ionizing radiation.

Almost immediately after the discovery of the Roentgen ray, in 1895, radiation injuries of various kinds began to be encountered in early X-ray workers, radium handlers, radiologists, and exposed patients. The injuries, which were predominantly acute reactions resulting from the killing of cells in affected tissues, were found to be preventable merely by keeping exposures below relevant threshold levels. By the middle of the twentieth century, however, it was realized that thresholds might not exist for certain effects of ionizing radiation, such as mutagenic, carcinogenic, and teratogenic eff…

[Clinics and management of radiation injuries of small bowel and colon(author's transl)].

In the Surgical and Gynecological Departments of the University of Mainz, Medical School, 50 patients were operated between 1964-1976 for mechanical and paralytical ileus and intestinal bleeding caused by radiation injury of the bowel. Diagnosis depends on the history of radiation therapy. At operation we have seen a recurrence of gynecological and other malignant growths only two times. Intestinal resection with anastomosis is the preferred method for the management of such radiation injuries. The postoperative dehiscence of the intestine was noticed only five times. In order to prevent highe…
