WE-G-BRCD-09: Assessment of Quantitative Differences Between Different Deformable Image Registration Algorithms for Image Guided Radiotherapy of Head and Neck Cancer Patients.

PURPOSE: DIR algorithms differ by physical principles employed in their design which in turn determines complexity of the transformation that the algorithm allows. Indiscriminate application of different algorithms without regard for their limitations may lead to significant discrepancies in research results and in clinical procedures. We compare two different algorithms in Head and Neck (H&N) patients to assess what aspects of DIR use are most sensitive to differences between algorithms. METHODS: H&N patients are enrolled in a protocol which acquires weekly FBCT and daily double CBCT. Each F

Patient doses in image-guided radiotherapy: status in Europe for cone-beam CT imaging in the pelvic region.

BACKGROUND: Organ absorbed doses in cone-beam CT (CBCT) imaging are often neglected in image-guided radiation therapy (IGRT). However, frequent imaging for patient positioning can result in significant and unrecorded additional radiation exposure. This study aims to evaluate organ doses from kV-CBCT and assess if they are optimized and how, in prostate and pelvic patient positioning protocols across Europe. Status of quality assurance in IGRT CBCT imaging is assessed in general. MATERIALS AND METHODS: Data collected from a survey distributed across Europe on IGRT practices were compiled and a

The Thoracic Absorption Dose and Secondary Tumor Risk Caused by Different Imaging Methods in Image-Guided Particle Radiotherapy.

BACKGROUND: Image-guided radiation therapy uses imaging methods such as CBCT to effectively improve treatment precision. Kilovoltage-imaging technology provides high soft tissue contrast at low doses, whereas megavoltage-imaging technology better displays deep and bony structures at high doses. Proton therapy is more sensitive to tissue density and positional accuracy, so it requires more stringent image guidance and higher precision than traditional X-ray therapy. OBJECTIVE: This study evaluates radiation doses from CBCT systems (TrueBeam, Halcyon, ProBeam, TOMO) in both adult and pediatric

Long-term clinical outcomes of lipiodol marking using standard gastroscopy for image-guided radiotherapy of upper gastrointestinal cancers.

BACKGROUND: Image-guided radiotherapy (IGRT) has significantly improved the precision in which radiotherapy is delivered in cancer treatment. Typically, IGRT uses bony landmarks and key anatomical structures to locate the tumor. Recent studies have demonstrated the feasibility of peri-tumor fiducials in enabling even more accurate delineation of target and normal tissue. The use of gold coils as fiducials in gastrointestinal tumors has been extensively studied. However, placement requires expertise and specialized endoscopic ultrasound equipment. This article reports the long-term outcomes of

Evaluation of registration accuracy using short-arc cone-beam computed tomography for image-guided radiotherapy in breast cancer.

BACKGROUND: Image-guided radiotherapy (IGRT) using cone-beam computed tomography (CBCT) enables highly accurate imaging registration; however, increased radiation exposure remains a concern. Recently, short-arc CBCT (SA-CBCT), which reduces projection data and reconstructs images, has become available. Although previous studies have shown that image quality deteriorates when projection data are reduced, no study has directly evaluated the clinical usefulness of SA-CBCT or its effect on registration accuracy. PURPOSE: We verified the registration accuracy of SA-CBCT in breast tangential irradi

Assessment and implication of rectal filling on vaginal motion in postoperative carcinoma endometrium patients during image guided radiotherapy.

AIM: While delivering radiotherapy it is utmost important to minimize target motion to decrease margins in postoperative gynaecological patients. Hence certain bladder and rectum filling protocols are followed by each institute. During cone beam computed tomography (CBCT) verification, we observed that this motion was more affected by rectal filling. To verify, we retrospectively analysed the vaginal movement and its relation with bladder and rectum filling. MATERIALS AND METHODS: We evaluated CBCTs of 15 patients of carcinoma endometrium. Bladder and rectum both were contoured offline on eac

Automated algorithm for calculation of setup corrections and planning target volume margins for offline image-guided radiotherapy protocols.

PURPOSE: Each radiotherapy center should have a site-specific planning target volume (PTV) margins and image-guided (IG) radiotherapy (IGRT) correction protocols to compensate for the geometric errors that can occur during treatment. This study developed an automated algorithm for the calculation and evaluation of these parameters from cone beam computed tomography (CBCT)-based IG-intensity modulated radiotherapy (IG-IMRT) treatment. METHODS AND MATERIALS: A MATLAB algorithm was developed to extract the setup errors in three translational directions (x, y, and z) from the data logged by the C

Validation of a commercial deformable image registration for surface-guided radiotherapy using an ad hoc-developed deformable phantom.

PURPOSE: The use of optical surface systems (OSSs) for patient setup verification in external radiation therapy is increasing. To manage potential deformations in a patient's anatomy, a novel deformable image registration (DIR) tool has been applied in a commercial OSS. In this study we investigate the accuracy of the DIR as compared to rigid image registration (RR). METHODS AND MATERIALS: The positioning accuracy of the DIR and RR implemented in the OSS was investigated using an ad hoc-developed anthropomorphic deformable phantom, named Mary. The phantom consists of 33 slices of expanded pol
