The Takotsubo Syndrome: Where the Sympathetic Stress Reflex and the Dive Reflex Conflict.

Understanding of the pathophysiology of Takotsubo syndrome (TTS) remains incomplete. This is a hypothesis-driven editorial, proposing a new autonomic mechanism that may explain several important and puzzling features of TTS. Rather than involving a single reflex pathway, we suggest that TTS may result from the interaction and conflict of two evolutionarily conserved autonomic reflex responses. The first is the sympathetic stress response (SSR), a cardiovascular predominantly sympathomimetic reaction characterised by heightened adrenergic activity, tachycardia and cardiovascular stress. The sec

[Evaluation of the morphology of stapedial reflex in otosclerosis. Provoked otosclerotic stapedial reflex].

Stapedial reflex is used, amongst other pathologies, for the study of otosclerosis. In this retrospective study we have collected 188 cases of patients with otosclerosis whose diagnosis has been confirmed surgically and their first line relatives. We have performed a descriptive analysis of audiometric tests, tympanometries and ipsilateral stapedial reflexes in relation with the evolutive phase of the disease. Transmission hypoacusis has been seen in 54%, mixed hypoacusis in 29% and sensorineural hypoacusis 8% of cases. On and OFF stapedial reflexes have been seen in 18%, inverted reflexes in

Trigeminocervical reflexes elicited by stimulation of the infraorbital nerve: head retraction reflex.

In the current study, the effects of stimulation of the infraorbital nerve (ION) on the trigeminocervical reflexes (TCRs), recorded from the posterior neck muscles, was investigated and the results were compared with the results recorded by stimulation of the supraorbital nerve (SON). TCRs obtained by stimulation of the ION was evaluated as the electrophysiologic counterpart of the head retraction reflex. Twenty normal control subjects, 10 men and 10 women, were enrolled in the study. The SON and the ION were stimulated by using a bipolar surface electrode. Results were recorded by using eithe

Subthreshold stimulation of a serotonin 5-HT3 reflex attenuates cardiovascular reflexes.

Volume-sensitive and chemosensitive cardiopulmonary reflexes modulate volume homeostasis via renal sympathetic nerve activity (RSNA). Blunting of volume-sensitive cardiopulmonary reflexes is associated with volume retention, e.g., in hypertension, whereas the role of chemosensitive cardiopulmonary reflexes is largely unknown. To elucidate the possible role of chemosensitive cardiopulmonary reflexes in control of volume homeostasis, we investigated whether subthreshold stimulation of 5-HT3 receptors modulates the control of RSNA by volume-sensitive cardiopulmonary reflexes or the arterial baror

Effects of brainstem lesions on the masseter inhibitory reflex. Functional mechanisms of reflex pathways.

The masseter inhibitory reflex (MIR) was investigated in 16 patients with localized brainstem lesions involving the trigeminal system. The MIR consists of two phases of EMG silence (S1 and S2) evoked by stimulation of the mental nerve during maximal clenching of the teeth. The extent of the lesions was assessed by neurological examination, nuclear magnetic resonance imaging (MRI), and recording of trigeminal reflexes with known paths (jaw jerk and blink reflex). The MIR appeared to be abnormal in lesions involving the pontine tegmentum, from the pontomedullary junction to midpons, and some les

Reflex pathways in the abdominal prevertebral ganglia: evidence for a colo-colonic inhibitory reflex.

1. In vitro experiments were performed on preparations consisting of prevertebral ganglia attached to the entire colon of guinea-pigs. The colon was divided into an orad and a caudad segment and intraluminal pressure was recorded from the terminal end of each segment. Intracellular recordings were simultaneously obtained from neurones in the coeliac plexus. 2. The source of mechanosensory input from the colon paralleled the responses to mesenteric nerve stimulation. That is, section of the mesenteric nerve that contributed the strongest synaptic input to a neurone eliminated most of the mechan

Electrically induced flexion reflex in gait training of hemiplegic patients: induction of the reflex.

Since the introduction of the concept of functional electrical stimulation, the use of multiple stimulation sites to activate several paretic muscles in a coordinated fashion during ambulation has been suggested. Rather than stimulating multiple muscles, we have applied a train of electrical pulses to a single skin area to activate several muscles simultaneously through the induction of a flexion reflex, and evaluated the effectiveness of this reflex in producing or assisting the swing phase of the gait cycle in the hemiplegic patient. Two considerations in selecting the best stimulus site wer

Acoustic reflex and reflex decay. Occurrence in patients with cochlear and eighth nerve lesions.

Acoustic reflex and reflex decay tests were administered to 50 normal ears, 50 ears with hearing loss due to noise trauma, 50 ears that had Meniere disease, and 28 ears that had involvement of the eighth nerve. In one normal ear, ten noise trauma ears, 11 Meniere disease ears, and 24 eighth nerve lesion ears to reflexes or reflex decay that were suggestive or retrocochlear lesions were observed. Acoustic reflex and reflex decay results are also compared to tone decay results for these patients.

Pelviureteral inhibitory reflex and ureteropelvic excitatory reflex: role of the two reflexes in regulation of urine flow from the renal pelvis to the ureter.

The mechanism by which the ureteropelvic junction (UPJ) regulates the passage of urine from the renal pelvis to the ureter, and prevents urinary backflow from the the ureter to the renal pelvis, is not completely understood. The current communication studies this mechanism in 18 dogs. With the dogs under anesthesia, nephrostomy was done through which two catheters (one pressure and one balloon-tipped) were introduced into the UPJ and the renal pelvis, respectively. Renal pelvis distension with a balloon filled with 1 ml of saline effected a rise of renal pelvic pressure from a mean basal press

The Oculocardiac Reflex: A Review.

BACKGROUND: A typical oculocardiac reflex (OCR) is a moderate trigemino-vagal bradycardia elicited by tension on an extraocular muscle (EOM) during strabismus surgery; however, many other orbital stimuli can elicit cardiac slowing including retinopathy of prematurity examination. METHODS: World literature related to trigeminovagal and oculocardiac reflex covering over 15,000 patients including 51 randomized clinical trials and case reports are analyzed and reviewed. Under an ongoing observational trial in Alaska, anesthetic, patient and surgical influences on routine strabismus surgery using

Corporeal reflexivity and autism.

Ethnographic video recordings of high functioning children with autism or Aspergers Syndrome in everyday social encounters evidence their first person perspectives. High quality visual and audio data allow detailed analysis of children's bodies and talk as loci of reflexivity. Corporeal reflexivity involves displays of awareness of one's body as an experiencing subject and a physical object accessible to the gaze of others. Gaze, demeanor, actions, and sotto voce commentaries on unfolding situations indicate a range of moment-by-moment reflexive responses to social situations. Autism is associ

Acamprosate and primitive reflexes.

OBJECTIVE: To describe 3 cases of patients with alcohol dependence whose primitive reflexes resolved upon initiation of acamprosate 666 mg 3 times daily. CASE SUMMARIES: A 57-year-old man had a long-standing history of alcohol dependence and a prominent snout reflex. The snout reflex resolved within 24 hours of starting treatment with acamprosate. A 45-year-old man with a long history of alcohol dependence had both a snout and a grasp reflex. These reflexes were present throughout 3 admissions for alcohol detoxification and continued until the end of his third admission, when he elected to be

Visuoocular reflexes in presbyvertigo.

Using electronystagmography, we tested 100 patients (ages 65-80 years) manifesting the clinical signs of presbyvertigo, for the purpose of demonstrating their visuoocular reflexes. We administered not only simple optokinetic and eye-tracking stimuli but caloric modification of each one as well (with contrary directed optokinetic trace movement). The results were compared with those obtained in a group of 40 healthy subjects between the ages of 20 and 25 years. In the older group, we observed a statistically significant increased frequency of asymmetry of optokinetic nystagmus and pathological

Upper airway reflexes.

It is usually assumed that upper airway pressure receptors mediate the reflexes involved in sleep apneas, but many other receptors may be involved, including those responding to chemical stimuli. The reflexes to upper airway negative pressure have been further studied, and the timing of their inputs shown to be important. Their effects on the cardiovascular system, including cerebral blood flow, have been emphasized. The central nervous pathways for the upper airway reflexes and their relationship to the neuronal circuits of the respiratory rhythm generator are being analyzed, but no clear pat
