Doc, when can he go back in the game?

In the injured athlete, several criteria must be considered before return to play. There are separate considerations for knee injuries, shoulder injuries, and general upper and lower extremity injuries, as well as concussion, hypertrophic cardiomyopathy, mononucleosis, and spondylolysis. It is important that surgical indications, postoperative rehabilitation, and risk for reinjury are reviewed by the surgeon before the athlete is allowed to resume activity.

Rethinking the Assessment of Arthrogenic Muscle Inhibition After ACL Reconstruction: Implications for Return-to-Sport Decision-Making-A Narrative Review.

Arthrogenic muscle inhibition (AMI) is a neuromuscular impairment commonly observed following anterior cruciate ligament reconstruction (ACLR). This condition, characterized by persistent quadricep inhibition due to altered afferent feedback, significantly impacts neuromuscular recovery, delaying return to running and sport. Despite advancements in rehabilitation strategies, AMI may persist for months or even years after ACLR, leading to muscle strength asymmetries, altered biomechanics, and an increased risk of reinjury. The mechanisms underlying AMI involve both peripheral (joint effusion, m

Treatment of complete rupture of the lateral ligaments of the ankle: a randomized clinical trial comparing cast immobilization with functional treatment.

This study compared the therapeutic efficacy between cast immobilization and functional treatment of grade III ruptures of the lateral ankle ligaments. Subjects ( n=121) had closed physeal cartilage, age under 35 years, grade III rupture without previous or associated injuries, and practiced regular sports. Patients were randomized into an immobilization group (21 days plaster cast) or a functional one (15 days strapping plus early controlled mobilization). Symptoms (pain, swelling, stiffness, subjective instability), joint laxity, return to preinjury activity (time and level) and rate of rein

Nerve entrapment after hamstring injury.

Hamstring muscle injuries are a frequent cause of athletic sequelae, and the frequency of reinjuries is high. Frequently, disability in sport is the consequence and performance is limited. A case report of a soccer player who was unable to play his sport after a minor hamstring muscle injury is presented. We introduce a previously undescribed lesion featured by a scar compromising a motor branch of the sciatic nerve to the long head of the biceps femoris muscle. Resection of the involved branch of the nerve resulted in complete pain relief and full sport capacity. This case report demonstrates…

Preexisting musculoskeletal conditions presenting as school sports injuries.

One hundred thousand insurance claims for secondary school injuries sustained between 1974 and 1986 were analyzed to determine the incidence of diagnosis and misdiagnosis of preexisting musculoskeletal conditions. Seventy-five percent of accident reports described acute injury to normal bones, joints, and soft tissues. Twenty percent were reinjuries or other sequelae of past trauma. Infection, congenital or developmental anomalies, or other nontraumatic conditions were found in 5% of the injuries reported. Patients with preexisting diseases had a higher morbidity than those with an acute prima…

Association of first anti-seizure medication choice with injuries in older adults with newly diagnosed epilepsy.

BACKGROUND: Epilepsy incidence increases exponentially in older adults, who are also at higher risk of adverse drug effects. Anti-seizure medications (ASM) may be associated with sedation and injuries, but discontinuation can result in seizures. We sought to determine whether there was an association between prescribing non-guideline concordant ASM and subsequent injury as this could inform care models. METHODS: Retrospective cohort study of adults 50 years or older with newly-diagnosed epilepsy in 2015-16, sampled from the MarketScan Databases. The outcome of interest was injury within 1-yea

Do Sports-related Concussions Induce Subsequent Injuries in Elite Male Football Players?

To assess the players' risk of a subsequent injury after sustaining concussive injuries and their return-to-competition in German professional men's football. A prospective injury database in the 1st Bundesliga was created encompassing 7 seasons (2014/15-2020/21). Cox proportional hazard model analyzed whether a concussive injury increased the risk of a subsequent injury in the first year after the index injury. 6,651 injuries were reported (n=182 concussive injuries). The incidence rate was 0.15 (95% CI 0.13-0.17) per 1000 football hours. A concussive injury was associated with only a slightl

Redislocation with a bony bankart lesion after arthroscopic Bankart repair.

This case report presents a traumatic dislocation after an arthroscopic Bankart repair in which the repair site was intact and failure occurred through a new bony interval. The new bony Bankart lesion was identified 3 years after the initial arthroscopic repair. This highlights the issue of whether a traumatic redislocation after a surgical repair for recurrent shoulder instability represents a failure of repair or a reinjury.

Hamstring Injuries in Athletes: Anatomy, Pathology, and Treatment.

Hamstring injuries are common in athletes and result in missed time from sport and activities. Recurrent injury is a persistent issue. Injury location and severity dictate treatment. Complete acute proximal hamstring avulsions are typically treated successfully with open or endoscopic surgery, while partial avulsions commonly are initially treated nonsurgically. If required, surgical repair results in high patient-reported outcomes, satisfaction, and return to activities. Chronic complete proximal avulsions have less predictable outcomes. Myotendinous injuries are typically treated nonsurgical

Passive terminal extension causes anterior tibial translation in some anterior cruciate ligament-deficient knees.

The purpose of the present study was to accurately measure anterior tibial translation during passive terminal extension (ATT-PTE) in anterior cruciate ligament (ACL)-deficient knees, and to investigate correlations between various characteristics of such knees and the magnitude of ATT-PTE. The subjects were 79 patients with unilateral ACL-deficient knees and little flexion contracture. All patients were confirmed to have ACL injury of one knee by arthroscopy. Lateral radiographs of the bilateral knees in passive terminal extension were compared, and ATT-PTE was measured using an original supe

Biomechanical Analysis After Anterior Cruciate Ligament Reconstruction at the Return-to-Sport Time Point.

BACKGROUND: Biomechanical alterations after anterior cruciate ligament reconstruction (ACLR) may increase the risk of knee reinjury. Furthermore, individuals may experience persistent biomechanical differences in the lower limbs after finishing the rehabilitation program and being ready to return to sport, which may lead to an increase in the risk of reinjury. Limited data exist on individuals after ACLR and biomechanical alterations during running in elite athletes. PURPOSE: To measure kinetic and kinematic data during overground running 6 to 8 months after ACLR in an elite sports cohort. S
