Optimizing use of assisted reproduction.

Infertile couples who have failed to achieve pregnancy now have new options that have become available in the last two decades. The costs are generally large and frequently not covered by insurance. Risks include multiple pregnancy and hyperstimulation syndrome. Evidence concerning results of several treatment options is presented. The large difference in success between age groups is displayed. Results to be expected from donor eggs to overcome the low success in older age groups (>38 years) are presented.

Genetic factors in assisted reproduction.

It is still unclear whether the procedures of assisted reproduction increase the risk of congenital malformations. Thus, it remains to be clarified whether an increased risk, if any, of congenital malformations in these children is caused by the procedure of assisted reproduction itself or by the underlying maternal and paternal background. From the genetic point of view, infertility patients seeking assisted reproduction have to be classified as a high-risk group. The prevalence of numerical chromosomal abnormalities is around 10% in these patients, compared with 0.85% in the general populati…

Female genital anomalies affecting reproduction.

OBJECTIVE: A multitude of female congenital anomalies are uncommon. However, their impact on reproduction can be profound. The aim of this review is to remind the practicing physician of the clinically relevant embryology and summarize the studies that look at the impact of such various anomalies on a woman's fecundity. We review particular surgical therapies that possibly may improve fertility in such women. DESIGN: Review and critique of available studies in which particular surgical therapies were done and whether they truly improved fertility in these women with congenital reproductive an…

Leptin and reproduction: a review.

OBJECTIVE: To review recent advances in understanding the role of leptin in the physiology and pathophysiology of reproduction, with a focus on relevant clinical situations. DESIGN: A MEDLINE computer search was performed to identify relevant articles. RESULT(S): Leptin, an adipocyte hormone important in regulating energy homeostasis, interacts with the reproductive axis at multiple sites, with stimulatory effects at the hypothalamus and pituitary and inhibitory actions at the gonads. More recently, leptin has been shown to play a role in other target reproductive organs, such as the endomet…

Autoimmune factors in assisted reproduction.

The present review highlights recent studies that investigated the possible influences of autoimmune factors in reproductive success or failure. These factors include antiphospholipid antibodies, antithyroid antibodies, antinuclear antibodies, antisperm antibodies, and antiovarian antibodies. The majority of recent work has focused on these potential autoimmune factors; however, controversy still exists over indicated testing and treatment options. An association of antiphospholipid antibodies and recurrent pregnancy loss has been established, and treatment with subcutaneous heparin appears mo

Assisted reproduction: a Jewish perspective.

Judaism values medicine as a noble profession. Physicians are mandated by the Bible to heal, and those who are ill are obligated to seek healing from their physicians. In Jewish thought, infertility is considered an illness. Hence, in spite of many Jewish legal and ethical questions, assisted reproductive techniques such as artificial insemination, in vitro fertilization and surrogate motherhood, within certain limited circumstances, are viewed with favor by most current rabbinic authorities, provided the couple is unable to have a child in the normal manner and after standard medical or surgi

Social determinants of human reproduction.

Developed countries have experienced both some population growth and unprecedented declines in fertility rates during the last half of the twentieth century. Couples now have fewer than two children on average in most European countries and they tend to postpone these births until a later age. A decline in male fertility has been suggested by some studies of semen quality, but there is contrasting evidence of shorter times to pregnancy for couples trying to conceive. An important economic factor is the income of young men relative to their parents' incomes, which determines how they rate the a

Environmental toxicants and female reproduction.

OBJECTIVE: To review current knowledge on the potential effects of environmental toxicants on female reproduction in laboratory animals, wildlife, and humans. DESIGN: Published literature about the effects of endocrine disruptors, heavy metals, solvents, pesticides, plastics, industrial chemicals, and cigarette smoke on female reproduction. RESULT(S): Published data indicate that chemical exposures may cause alterations in reproductive behavior and contribute to subfecundity, infertility, pregnancy loss, growth retardation, intrauterine fetal demise, birth defect, and ovarian failure in labo…

Biotechnological advances in goat reproduction.

Goat selection and reproduction have resulted from using conventional methods of natural mating and artificial insemination. Genetic improvements resulting from these are usually slow. Innovative developments in biotechnology rapidly propagate superior genes, offering hope for modeling and designing animals to fit market and environmental requirements. Use of Tris, citric acid, fructose, egg yolk, and glycerol extender has enabled goat sperm to be stored successfully for several years before being used in cervical or laparoscopic insemination. Laparoscopic recovery of goat embryos to reduce ad…

[Thyroid disease and reproduction dysfunction].

Thyroid disorders have been implicated in a broad spectrum of reproductive disorders ranging from abnormal sexual development to menstrual irregularities and infertility. Hyperthyroidism in the male is thought to cause gynecomastia. In the female hypo and hyperthyroidism results in changes in cycles length and amount of bleeding. The hypothyroidism in the male has less clear-cut effect on the reproductive system. Long-standing, untreated hypothyroidism is associated with galactorrhea. These abnormalities are reversible with adequate thyroid supplementation or collection of hyperthyroidism. Thu

The pineal gland and reproduction.

The hormonal activity of the pineal gland is influenced by both the dark-light cycle and the seasonal cycle, causing it to play an important role in the neuroendocrine control of reproductive physiology. This is especially evident in seasonally breeding animals, in which reproductive function is clearly influenced by seasonal variations in the duration of night and day. Humans are not seasonal breeders. Nevertheless, seasonal fluctuations have been described in human reproduction, and the pineal gland also appears to exert an important role in the neuroendocrine regulation of human reproductiv…

Assisted reproduction: a reassuring picture.

Many couples seek solutions to problems of infertility by using assisted reproduction techniques. Despite very different beginnings, children conceived after donor insemination, ovum donation, in-vitro fertilisation or gamete intrafallopian transfer show no adverse long term effects, either physically or psychosocially.

Concurrent tuboplasty and assisted reproduction.

OBJECTIVE: To review and summarize the role of combining assisted reproductive technologies (ART) with lysis of adhesions and microsurgical tuboplasty in selected groups of patients. DESIGN: A retrospective analysis of the available literature and a review of the results of concurrent fimbriectomy reversal and ART. CONCLUSION: In a relatively few and highly selected patients, the concurrent use of ART and laparotomy or laparoscopy for lysis of adhesions and tuboplasty can more safely access the ovaries for oocyte retrieval and address associated problems such as pelvic pain while reducing co

Fulminant tropical hepatitis--experimental reproduction.

After a review of the literature on epidemiological, serological and histopathological data concerning so-called "Amazon Black Fever" (Labrea disease), an experimental model of morphological reproduction of the disease is presented. The model is based on the probable causal synergism of Labrea hepatitis, i.e., viral injury plus protein-caloric malnutrition. Mice infected with the Oriboca arbovirus presented classic yellow-fever-like lesions of the liver, with a few malnourished animals showing the characteristic morular changes of the human disease.

Reproduction in female Onchocerca gibsoni.

The embryonic stages in the uteri of adult female Onchocerca gibsoni were studied to determine the reproductive potential of this worm. It was concluded that female O. gibsoni undergo cyclic phases of reproduction, each lasting about 14.5 weeks. Once insemination occurred and a cycle commenced, the rate of embryonic development was reasonably constant irrespective of the age of the worm. Mature young females had fewer embryos than older females but both contained a similar proportion (about 19%) of abnormal embryos. In comparison to the older female worms, a greater proportion of mature young …
