The clinical features of severe COVID-19 with respiratory failure: A Chinese single-center retrospective study.

The global pandemic of COVID-19, caused by the novel coronavirus SARS-CoV-2, has resulted in widespread alterations to public health measures worldwide. This observational study aimed to assess the clinical features and results of respiratory failure in patients with severe COVID-19. A single-center observational study was performed at a Chinese hospital between November 1, 2022, and February 31, 2023. All 182 enrolled patients were diagnosed with respiratory failure, 84 patients were infected with COVID-19, and the other 98 patients were not infected. A review of available medical records at

[Respiratory tract symptoms and illnesses in rescue and clearance workers after the World Trade Center catastrophe].

The World Trade Center catastrophe and subsequent rescue and clearance operations caused unusual respiratory tract symptoms in fire fighters and rescue workers. Persistent cough was a common symptom, being extraordinarily often associated with the gastroesophageal reflux symptom. Irritant dusts caused reactive airways dysfunction syndrome (RADS). Tracheal hyperreactivity increased with the exposure, and the risk of asthma is estimated to have increased up to 12 times higher as compared with the normal population. Investigation and treatment of exposed persons have yielded generalizable informa

Respiratory center output and ventilatory timing in patients with acute airway (asthma) and alveolar (pneumonia) disease.

To investigate the mechanism for hyperventilation in two common but dissimilar conditions, asthma (A) and lobar pneumonia (P), we examined the ventilatory pattern in 17 A and six P subjects. When acutely ill, hyperventilation (PaCO2) was similar in the two groups. Minute ventilation (VE) however, was slightly greater in group A than in P. In A patients, measurements of occlusion pressure (dP/dT) and inspiratory flow rate (VT/ti) during quiet breathing showed enhancement of respiratory center output. By contrast, in P patients, dP/dT and VT/ti were not elevated. Tidal volume (VT) was 0.59 +/- 0…

Acute Respiratory Distress Syndrome in Trauma 2007-2019: Comprehensive Patient and Center-Level Retrospective Cohort Analysis.

OBJECTIVES: Acute respiratory distress syndrome (ARDS) represents a significant complication in trauma patients. Yet the epidemiology of ARDS in trauma remains incompletely characterized. We sought to define trends in ARDS frequency and the effect of temporal, patient, and center-level factors on outcomes with the hypothesis that ARDS independently predicts mortality. DESIGN: Retrospective cohort study. SETTING: Hospitals submitting data to the American College of Surgeons National Trauma Data Bank. PATIENTS: Injured patients 18 years old or older from 2007 to 2019 on mechanical ventilation

Impact of COVID-19 on viral respiratory infection epidemiology in young children: A single-center analysis.

BACKGROUND: The COVID-19 pandemic impacts different health aspects. Concomitant with the adoption of non-pharmaceutical interventions (NPIs) to reduce the spread of SARS-CoV-2, global surveillance studies reported a reduction in occurrence of respiratory pathogens like influenza A and B virus (IAV & IBV) and respiratory syncytial virus (RSV). We hypothesized to observe this collateral benefit on viral respiratory infection epidemiology in young children. METHODS: Respiratory samples of children aged below 6 years, presenting at the outpatient clinic, emergency department, or pediatric infecti

Sudden unexpected death owing to unilateral medial medullary infarction with early involvement of the respiratory center.

A 64-year-old woman was found dead in her home. At autopsy, although relatively fresh bruises were found on her body, no lethal injury was observed in an internal observation. Mild edematous swelling of the right half of the medulla oblongata was observed. There was acute medial medullary infarction (MMI), which mainly involved the nucleus hypoglossi, medial lemniscus, hypoglossal root, inferior olivary nucleus, and pyramidal tract. Subacute infarction of the lower part of the cerebellum was also found, and severe atherosclerosis of the right vertebral artery containing thrombi was found as th

The upper respiratory pyramid: early factors and later treatment utilization in World Trade Center exposed firefighters.

BACKGROUND: We investigated early post 9/11 factors that could predict rhinosinusitis healthcare utilization costs up to 11 years later in 8,079 World Trade Center-exposed rescue/recovery workers. METHODS: We used bivariate and multivariate analytic techniques to investigate utilization outcomes; we also used a pyramid framework to describe rhinosinusitis healthcare groups at early (by 9/11/2005) and late (by 9/11/2012) time points. RESULTS: Multivariate models showed that pre-9/11/2005 chronic rhinosinusitis diagnoses and nasal symptoms predicted final year healthcare utilization outcomes m

Respiratory pathogen epidemiology in children under 5 years hospitalized with acute respiratory infections in Qingdao, China: a retrospective, single-center, repeated cross-sectional surveillance study.

BACKGROUND: During the coronavirus disease 2019 (COVID-19) pandemic, non-pharmaceutical interventions (NPIs) reduced the transmission of severe acute respiratory syndrome coronavirus 2 (SARS‑CoV‑2) while simultaneously affecting the prevalence of respiratory pathogens to varying degrees. Currently, epidemiological characteristics vary across different regions and among different pathogens, and there is a lack of continuous surveillance data following the outbreak. This study aims to analyze the epidemiological characteristics of six respiratory pathogens in hospitalized children under the age

Impact of a Multi-Center Collaborative on Antibiotic Duration for Acute Respiratory Infections in Pediatric Urgent Care.

BACKGROUND: Updated guidance for common acute respiratory infections (ARIs) recommends shorter antibiotic durations. While stewardship efforts often focus on appropriate antibiotic selection, it remains unclear whether initiatives targeting first-line choice also influence duration. DESIGN/METHODS: We conducted a secondary analysis of a national quality improvement collaborative including 92 pediatric urgent care sites across 9 organizations in 22 states and Washington, D.C. (April 2022-September 2024). The collaborative emphasized equitable first-line antibiotic selection; duration was not a

Activities of an ECMO Center for Severe Respiratory Failure: ECMO Retrieval and Beyond, A 4-Year Experience.

OBJECTIVE: Beyond retrieval and management of patients with severe acute respiratory distress syndrome, an extracorporeal membrane oxygenation (ECMO) center also encompasses several other actions, such as on-call consultations, advice, and counseling, to the physicians at the peripheral centers, but few data are available on this topic. Therefore, the authors describe the composite activities of retrieval and counseling of an ECMO center since 2014. DESIGN: The referral calls addressed to the authors' ECMO center for patients with respiratory failure were prospectively recorded in a dedicated

The Magnitude of Postconvulsive Leukocytosis Mirrors the Severity of Periconvulsive Respiratory Compromise: A Single Center Retrospective Study.

Background: Generalized epileptic convulsions frequently exhibit transient respiratory symptoms and non-infectious leukocytosis. While these peri-ictal effects appear to arise independently from one another, the possibility that they stem from a common ictal pathophysiological response has yet to be explored. We aimed to investigate whether peri-ictal respiratory symptoms and postictal leukocytosis coexist. Methods: We performed a single center retrospective chart review of 446 patients brought to our emergency department between January 1, 2017 and August 23, 2018 for the care of generalized
