Adventitious Pulmonary Sound Detection: Leveraging SHAP Explanations and Gradient Boosting Insights.

Pulmonary illnesses are annually reported as highly prevalent. Patient outcomes can, however, be improved through the aid of automated processes for early diagnoses. This study aims to develop an automated method with a comprehensive explanation for diagnosing underlying adventitious sounds in respiratory diseases. For this purpose, stethoscope audio signal recordings are initially segmented to merely include either normal or adventitious sounds such as wheezing and crackling. A comprehensive feature set representing the temporal and spectral dynamics in the respiratory events is extracted to

Modeling and measurement of flow effects on tracheal sounds.

The analysis of breathing sounds measured over the extrathoracic trachea offers a noninvasive technique to monitor obstructions of the respiratory tract. Essential to development of this technique is a quantitative understanding of how such tracheal sounds are related to the underlying tract anatomy, airflow, and disease-induced obstructions. In this study, the first dynamic acoustic model of the respiratory tract was developed that takes into consideration such factors as turbulent sound sources and varying glottal aperture. Model predictions were compared to tracheal sounds measured on four

Disorders of the larynx.

The upper respiratory tract is a frequent cause of exercise intolerance in horses, particularly in racing horses. There are a myriad of laryngeal abnormalities that may restrict airflow at the rima glottidis. Careful endoscopic examination is a crucial part of the examination of any racing horse suffering from poor performance. There has recently been interest in spectrum analysis of respiratory sounds. It has been determined that laryngeal hemiplegia and dorsal displacement of the soft palate have unique sound patterns. Therefore, spectrum analysis of respiratory sounds may prove to be useful…

Effect of Subglottic Stenosis on Expiratory Sound Using Direct Noise Calculation.

Subglottic stenosis (SGS) is a rare yet potentially life-threatening condition that requires prompt identification and treatment. One of the primary symptoms of SGS is a respiratory sound that is tonal. To better understand the effect of SGS on expiratory sound, we used direct noise calculation to simulate sound production in a simplified axisymmetric configuration that included the trachea, the vocal folds, the supraglottal tract, and an open environmental space. This study focused on flow-sustained tones and explored the impact of various parameters, such as the SGS severity, the SGS distanc

The use of tracheal sounds for the diagnosis of sleep apnoea.

UNLABELLED: Tracheal sounds have been the subject of many research studies. In this review, we describe the state of the art, original work relevant to upper airways obstruction during sleep, and ongoing research concerning the methods used when analysing tracheal sounds. Tracheal sound sensors are a simple and noninvasive means of measurement and are more reliable than other breathing sensors. Developments in acoustic processing techniques and enhancements in tracheal sound signals over the past decade have led to improvements in the accuracy and clinical relevance of diagnoses based on this

Spectral analysis of cough sounds recorded with and without a nose clip.

The voluntary cough sounds of healthy volunteers, patients with chronic rhinitis, chronic rhinitis with bronchial asthma and asthma were recorded with and without a nose clip. A gated series of signals of the first cough sound lasting 200 ms was analysed by a system with a 20 ms delay of the first signal. In the case of filtered cough sounds, low-cut digitally, the mean values of averaged spectra of the healthy volunteers showed a first peak around 350 Hz, similar to the expiratory spectra of respiratory sounds. Second and third harmonics were also identified. The mean values of averaged spect…

Lung Auscultation Using the Smartphone-Feasibility Study in Real-World Clinical Practice.

Conventional lung auscultation is essential in the management of respiratory diseases. However, detecting adventitious sounds outside medical facilities remains challenging. We assessed the feasibility of lung auscultation using the smartphone built-in microphone in real-world clinical practice. We recruited 134 patients (median[interquartile range] 16[11-22.25]y; 54% male; 31% cystic fibrosis, 29% other respiratory diseases, 28% asthma; 12% no respiratory diseases) at the Pediatrics and Pulmonology departments of a tertiary hospital. First, clinicians performed conventional auscultation with

Monophonic and Polyphonic Wheezing Classification Based on Constrained Low-Rank Non-Negative Matrix Factorization.

The appearance of wheezing sounds is widely considered by physicians as a key indicator to detect early pulmonary disorders or even the severity associated with respiratory diseases, as occurs in the case of asthma and chronic obstructive pulmonary disease. From a physician's point of view, monophonic and polyphonic wheezing classification is still a challenging topic in biomedical signal processing since both types of wheezes are sinusoidal in nature. Unlike most of the classification algorithms in which interference caused by normal respiratory sounds is not addressed in depth, our first con…

A cross-species framework for classifying sound-movement couplings.

Sound and movement are entangled in animal communication. This is obviously true in the case of sound-constituting vibratory movements of biological structures which generate acoustic waves. A little less obvious is that other moving structures produce the energy required to sustain these vibrations. In many species, the respiratory system moves to generate the expiratory flow which powers the sound-constituting movements (sound-powering movements). The sound may acquire additional structure via upper tract movements, such as articulatory movements or head raising (sound-filtering movements).

Computerized acoustic detection of obstructive apnea.

Cardiac, respiratory and neurologic abnormalities have been identified as causes of Sudden Infant Death Syndrome (SIDS). Recurrent central a … miniature microphone placed over the trachea, a cassette tape recorder and a MINC-11 microcomputer. Respiratory sounds were recorded on 5 anesthesized rabbits in which partial and total airway obstruction was artificially induced. Sounds were analyzed by computer using fast Fourier transformations. Amplitude versus frequency was plotted for normal breathing, partial …

Detection of intratracheal accumulation of thick secretions by using continuous monitoring of respiratory acoustic spectrum: a preliminary analysis.

The accumulation of tracheobronchial secretions may contribute to a deterioration in pulmonary function and its early detection is important. In this study, we analyzed the respiratory sound spectrum in patients with intratracheal secretion, and compared acoustic characteristics before and after therapeutic endotracheal suctioning. After review of anesthetic records of liver transplant recipients, we included recipients with identified intratracheal secretion during surgery. Intraoperative breath sounds recorded through esophageal stethoscope were sampled in 20 s-period before and after suctio

Detecting Respiratory Pathologies Using Convolutional Neural Networks and Variational Autoencoders for Unbalancing Data.

The aim of this paper was the detection of pathologies through respiratory sounds. The ICBHI (International Conference on Biomedical and Health Informatics) Benchmark was used. This dataset is composed of 920 sounds of which 810 are of chronic diseases, 75 of non-chronic diseases and only 35 of healthy individuals. As more than 88% of the samples of the dataset are from the same class (Chronic), the use of a Variational Convolutional Autoencoder was proposed to generate new labeled data and other well known oversampling techniques after determining that the dataset classes are unbalanced. Once…
