The necrosis of the ligamentum teres hepatis is a very rare and unrecognized pathology. Two cases only were reported in the literature. The presence of generalized or epigastric peritoneal signs, simulates acute cholecystitis or perforated pyloric ulcer. The diagnosis could be suggested by ultrasonography and CT scan of the abdomen revealing a hyperechogenic and hypodense focal lesion at the junction of the segments III and IV of the liver, associated with inflammatory signs and/or collection along the ligamentum teres. A case of clear-cell epithelioid leiomyoma of the round ligament in a 69-year-old woman is described. The neoplasm presented as a firm left inguinal mass. A preoperative computed tomography (CT) scan demonstrated an enhancing lesion extending extra-abdominally from the region of the external inguinal ring. An unusual case of recurrent endometrial carcinoma presenting as a round ligament metastasis is discussed. The primary lesion detected six years earlier was a well-differentiated lesion with less than 50% myometrial invasion located in the fundus. A persistent inguinal swelling represented a metastasis to the round ligament. Histologic examination revealed a poorly differentiated adenocarcinoma. Patients with early stage well-differentiated endometrial carcinoma are not immune from late recurrence.

A technique of laparoscopic ligamentum teres cardiopexy for reflux oesophagitis unresponsive to medical treatment is described. The procedure has been performed on five patients and on postoperative testing has been shown to achieve competence of the lower oesophageal sphincter by lengthening of the intra-abdominal segment, accentuation of the angle of His and elevation of the pressure in the high pressure zone. All the patients have been relieved of their symptoms by the operation and have not sustained any adverse effects but the follow-up period is short.

A 3-year-old girl with idiopathic extrahepatic portal vein obstruction undergoing mesenterico-left portal vein bypass (MLPVB), or Rex shunt, for recurrent variceal bleeding was intraoperatively discovered to have duplicated round ligaments and Rex recessi. The left and right portal veins connected to their respective round ligaments but had minimal communication within the liver. Recurrent adnexal torsion is a rare gynecological emergency. We report a case of recurrent ipsilateral adnexal torsion in a woman with polycystic ovaries, previously submitted to a laparoscopic plication of the utero-ovarian ligament.
