A 15-year-old girl of Turkish descent had for one year complained of severe recurrent fever-associated deep back pains. Since she was three years of age she had suffered from repeated attacks of fever and severe abdominal pain which ceased spontaneously in 1-3 days. On physical examination the sacrum and iliosacral joints were very painful to percussion, and she limped. Radiography revealed symmetric destructive sacroiliitis. Despite the unusual location of the arthritis, the triad of fever, abdominal pain and arthritis, as well as her belonging to an ethnic "at risk" group, pointed to the dia…

Technetium scintigraphy established sacroiliitis in a 21-year-old man who had complained of hip pain for two weeks. Brucella melitensis was cultured from hip-joint fluid. He was treated with 160 mg trimethoprim and 800 mg sulphamethoxazole, three times daily for six weeks. He also had prophylactically 600 mg rifampicin daily by mouth for the last three weeks. He was symptom-free four weeks after onset of treatment. At follow-up one year later there was no sign of recurrence. Brucella titres and blood sedimentation rate were within normal limits.

Brucellosis is an uncommon infectious disease in the United States though it remains endemic in many parts of the world. Signs and symptoms of the disease are highly variable, with musculoskeletal complaints occurring frequently. The combination of an uncommon infection presenting with protean manifestations often results in missed or delayed diagnosis in the Western world.

Two patients are described with unilateral pyogenic sacroiliitis, 1 complicating intravenous heroin addiction and the other complicating a septic skin lesion. The need for diagnostic alertness in the early recognition of this uncommon condition is illustrated and the apparent lack of relationship of pyogenic sacroiliitis to idiopathic ankylosing spondylitis is discussed.

BACKGROUND: Sacroiliitis is a debilitating condition causing significant lower back pain. Current treatment options, including medication, physical therapy, and invasive procedures, have limitations. High-intensity focused ultrasound (HIFU) offers a noninvasive, image-guided approach to treat chronic pain. In this study, we aimed to evaluate the safety and efficacy of HIFU for the treatment of sacroiliitis in patients with sacroiliac joint (SIJ) pain.
METHODS: A single-arm, open-label, pilot study was conducted on eight patients with SIJ pain. HIFU treatments were performed using a fluoroscop…

OBJECTIVES: The aim of this study was to determine the prevalence of "rheumatic" and "non-rheumatic" changes of the sacro-iliac joints (SIJ) … ≥ 1 cm.
RESULTS: In 45 cases (21%), the MRI findings matched the clinical diagnosis of "rheumatic" sacroiliitis. Interestingly, in 99 cases (47%) the SIJ changes were classified as "non-rheumatic". L5-S1 degenerative changes, scoliosis and pelvic asymmetry were most frequently encountered as concomitant phenomena in our study.
CONCLUSIONS: MRI of the sacroil …

Sarcoidosis is a multisystem disorder of unknown etiology characterized by the presence of non-caseating granulomas in the organs affected. Sarcoid arthropathy is a rare manifestation, and sacroiliitis is an unusual first manifestation of the disorder.

We describe an HLA-B27 positive patient in whom posttraumatic pyogenic sacroiliitis led to complete unilateral sacroiliac joint ankylosis in the absence of any signs indicative of HLA-B27 associated spondyloarthropathy. Sacroiliitis is the pathologic hallmark - and usually one of the earliest pathologic manifestations - of ankylosing spondylitis (AS). Bilateral sacroiliitis is typical for ankylosing spondylitis. The frequency of asymmetric sacroiliitis may be higher in other inflammatory disorders, e.g., reactive arthritis, Reiters syndrome, spondylitis associated with psoriasis, or inflammato…

BACKGROUND: Sacroiliitis is a rare infection and an unusual cause of back pain during pregnancy. Because pregnancy and infections commonly associated with pregnancy are risk factors, this diagnosis should be considered in the gravida with sacroiliac pain.
CASE: A 17-year-old woman at 24 weeks' gestation, with a history of illicit drug use, presented to a local emergency room with back and buttock pain. Bacteriuria and pyuria were diagnosed, and cefazolin was initiated. Blood cultures grew Staphylococcus aureus and Escherichia coli. Despite prolonged antibiotic therapy for possible endocarditi…

We describe 10 cases of pyogenic sacroiliitis occurring in a rural population. Seven were male and 3 were female with a mean age of 22.4 years. None was a recent intravenous drug abuser. Five patients had a history of recent pelvic trauma. 99mTechnetium scintiscans revealed increased sacroiliac (SI) joint uptake in 8 of 8 cases. Blood cultures were positive in 60% of patients. Staphylococcus aureus was isolated in 7 cases from blood and/or SI aspirates and Hemophilus influenzae type B in one case. Nine of 10 patients recovered completely. One underwent arthrodesis for recurrent SI pain without…

Computed tomography (CT) and conventional radiography of the sacroiliac joint were compared in 43 patients. CT appeared to be far more sensitive and equally specific in the recognition of sacroiliitis. In a number of patients with sacroiliitis diagnosed by both techniques, CT demonstrated abnormalities that were not demonstrated by conventional radiographs. Of those patients with clinical evidence of sacroiliitis and HLA-B27 positivity, 50% had negative or equivocal radiographs compared to 19% who had negative computed tomographic images for sacroiliitis.

Sacroiliitis can be demonstrated radiologically in over one-third of patients with chronic Reiter's syndrome. We have used a bone scan technique with digital analysis which will quantitatively assess the sacroiliac joints, and have found abnormalities in 24 of 33 patients with active Reiter's syndrome. It seems likely therefore that sacroiliitis is a common feature in patients with early Reiter's syndrome and while in most patients it resolves with resolution of disease activity, in some it may persist with the development of overt radiological changes.
