Anatomic design of a sensate plantar flap.

To date, full-thickness skin ulcers of the heel of the foot are still difficult wounds to cover. Many choices of coverage are available, but few provide long-term coverage due to bone exposure, deficient sensibility, and subsequent breakdown. An advancement flap based on the medial plantar artery with sensory nerve branches is presented with long-term follow-up. This flap is available and expendable, with an excellent are of rotation. Reliable circulation and sensibility are the key factors for successful reconstruction.

"Phantom" sensations following intraspinal injury.

The sensations associated at times with neural lesions in the spinal canal may have the following features and phases: 1) early anosognosia, i.e. lack of awareness of disability; 2) initial stimulation; 3) illusion of amputation; 4) disappearance of that illusion and 5) grotesque sensations. Detailed features of the phantoms such as changes in their size, position, and vividness as well as tactics evoking changes in the sensations are recorded. The special situations are described in which paraplegia is accompanied by actual amputation, or there is a phantom of an upper limb after avulsion of …

Effect of epidural analgesia on postpartum bladder sensation.

INTRODUCTION: This study investigates the effects of epidural analgesia on postpartum urinary bladder sensation. Urinary retention and inability to void are well known side effects of epidural analgesia but the causes are unknown. The incidence of postpartum hypotonic bladder injury is increased in women receiving epidural analgesia during labour. We hypothesise that long-acting local anaesthetics used in epidural analgesia can impair bladder sensation and can lead to an increased risk of asymptomatic overdistension. METHOD: Primiparous women between 38-42 weeks gestation who had received epi…

Trigeminal Schwannoma Surgery: Challenges in Preserving Facial Sensation.

Treatments of schwannoma have dramatically improved in the previous few decades, but preservation of the functions of the originating nerve, such as facial sensation in trigeminal schwannomas, still remains challenging. As the preservation of facial sensation in trigeminal schwannomas has not been analyzed in detail, we here review our surgical experience of more than 50 trigeminal schwannoma patients, particularly focusing on their facial sensation. Since the facial sensation in each trigeminal division showed a different perioperative course even in a single patient, we investigated patient-…

Referred Sensation Areas in a Bilateral Toes Amputee.

Various mechanisms in generating phantom limb pain (PLP) have been hypothesized in the literature. However, there still is no clear understanding of how PLP develops and why it presents. Amputation leads to permanent anatomical and physiological changes of the neural path previously supplying the brain with sensory input, as well as to formation of referred sensation areas (RSAs) on the stump or its vicinity. Sensations may be evoked in the lost body part upon stimulation of RSAs that may be exploited as artificial sensory input. In this work, we present the analysis of RSA maps from a 45-year…

Ocular filariasis: "Dancing sensation in the anterior chamber".

We report a case of ocular filariasis in a 28-year-old male who presented with a 2-month history of irritated acute red eye. Examination revealed a living and active large worm dancing in the anterior chamber of the left eye causing acute iridocyclitis, leading to drop in vision to 6/36. The worm whose size was approximately 33 mm in length was extracted under local anesthesia in toto using viscoelastics without damaging the ocular structures. Microscopic examination confirmed adult Wuchereria bancrofti.

[Afferent nerve activity in relation to bladder sensation].

Bladder afferent nerves are composed by myelinated Aδ- and unmyelinated C-fibers. During the storage phase of urine, distention of the bladder has long been considered to evoke afferent activity via Aδ-fibers connected in series with the smooth muscle fibers. In contrast, a previous study in cats revealed that more than 90% of C-fibers do not respond to normal bladder distension, being so called 〝silent〟 fibers. However, at least in rats, C-fibers can respond to normal bladder distension like Aδ-fibers, although they may also fulfill a potentially different role in the bladder sensory function

Sensation-seeking: Dopaminergic modulation and risk for psychopathology.

Sensation-seeking (SS) is a personality trait that refers to individual differences in motivation for intense and unusual sensory experiences. It describes a facet of human behaviour that has direct relevance for several psychopathologies associated with high social cost. Here, we first review ways of measuring SS behaviour in both humans and animals. We then present convergent evidence that implicates dopaminergic neurotransmission (particularly via D2-type receptors) in individual differences in SS trait. Both high tonic dopamine levels and hyper-reactive midbrain dopaminergic responses to s…

Does defective volume sensation contribute to detrusor underactivity?

AIMS: The urodynamic finding of detrusor underactivity (DU) in neurologically intact unobstructed patients may relate to impaired volume sensations rather than detrusor contractile defects. We hypothesized that DU patients would demonstrate higher volumes but similar wall stress at sensation thresholds, and similar voiding contractility, compared to other groups. METHODS: Chart review of urodynamic studies in neurologically normal, nonobstructed symptomatic patients. Urodynamic studies having the primary findings of DU, stress urinary incontinence (SUI), detrusor overactivity (DO), and increa…

Recovery of sensation after orthognathic treatment: patients' perspective.

INTRODUCTION: In this study, we considered patients' self-assessments to establish the recovery of sensory disturbance and the resultant dis … ons between the extent of sensory loss and the distress associated with it. CONCLUSIONS: Change in sensation occurs rapidly in the first 6 weeks postoperatively and more slowly thereafter. The distress caused by sensory loss is strongly related to the amount of sensory disturbance for both the face and the mouth.

Residual haptic sensation following stroke using ipsilateral stimulation.

In 1987, Weiskrantz and Zhang described a stroke patient with severe somatosensory loss who, nevertheless, demonstrated impressive residual sensory performance when required to touch the affected limb with her ipsilesional hand (self touch; ST). The current study set out to ascertain the prevalence and characteristics of self-touch enhancement (STE) in patients with unilateral stroke and hemihypaesthesia. Thirty-nine stroke patients who were referred with hemihypaesthesia fulfilled the criteria. STE was defined where a patient showed a statistically significant increase (p<0.05) in performance

Motion sickness history, food neophobia, and sensation seeking.

Motion sickness is believed to be caused by conflicting sensory signals, a situation that mimics the effects of ingesting certain toxins. Th … so individuals who are more susceptible may be more prone to avoid such experiences, i.e., be less sensation seeking. Finally, it was expected that food neophobia would be more frequent in individuals low on sensation seeking tendencies. Self-reported motion sickness history in 308 adults (M= 18.8 yr.; SD = 1.6) was correlated with scores on the Arnett Inv …

Functional bowel disease: roles of sensation and motility.

Functional bowel disorders are collections of symptoms attributable to the mid or lower gastrointestinal tract. The two most common disorders, irritable bowel syndrome and functional dyspepsia, have common etiopathogenetic features, notably psychosocial disturbances, dysmotility and heightened sensitivity. The control mechanisms, pathophysiology, investigation and potential pharmacotherapies of these disorders are reviewed. Serotonergic and adrenergic agents are among the novel approaches that may have a significant impact on these disorders.

Tibial nerve grafting for restoration of plantar sensation.

Five patients with a segmental loss of the tibial nerve and insensate plantar aspect of the foot were evaluated at an average of 5 years (2.5-9.5) postsural nerve grafting of the tibial nerve. Free-tissue transfer was required in three patients. The results as graded by restoration of superficial sensation, healing of plantar ulceration, and absence of neurogenic pain were four good and one fair at follow-up over 2 years. Assessment at 2 years or less yielded one good and four poor results, indicating a prolonged recovery period. Nerve grafting may be indicated in segmental injuries of the tib…
