Tuberculosis (TB) affecting the central nervous system (CNS) is rare, often mimicking other intracranial pathologies such as meningiomas, especially when located in the anterior skull base. Despite a global reduction in TB incidence, CNS TB continues to present diagnostic challenges due to its nonspecific imaging characteristics. We report a case of a 39-year-old male with symptoms including persistent headache, vertigo, and visual disturbances. Initial imaging suggested meningioma; however, histopathology revealed a tuberculoma. The patient responded well to antitubercular therapy following s

Central skull base osteomyelitis (CSBO) is a rare complication of infection in pediatric patients, especially when there are no comorbidities like immunosuppression or metabolic illness. The diagnosis of CSBO is a challenge in children, and imaging findings can mimic skull base tumor. We describe the clinical history and image diagnosis of a case in a 6-year-old girl with no relevant history who presented an extensive skull base lesion. She underwent tumor resection surgery. The intraoperative finding confirmed clivus osteomyelitis, and the histopathological studies discarded malignancy. After

An understanding of the anatomy and function of the skull base venous channels is fundamentally important for safe performance of endoscopic surgery for skull base lesions. This review focuses on 4 skull base venous channels-namely, the intercavernous sinuses, basilar plexus, osseous veins, and inferior petroclival vein-because these have been less recognized by neurosurgeons than other major venous channels such as the inferior petrosal sinus. We reviewed the literature concerning these 4 venous channels and discussed their anatomy and function. The literature review was mainly based on an el

Growing skull fracture (GSF) is a rare complication of pediatric head trauma. It usually arises from a linear fracture with an underlying dural tear, which results in herniation of the intracranial contents. In rare cases, the herniated intracranial contents can be restricted in the diploe, resulting in an expanded diploic cavity. If there is brain parenchyma herniating into the expanded diploic cavity, the condition is termed as intradiploic encephalocele (IE). Here we present a peculiar pediatric occipital GSF case that resulted from the silent progression of a linear fracture with a small b

OBJECTIVE: An endonasal endoscopic surgery to the anterior fossa skull base was developed in cadaver dissection as a minimally invasive surgical technique and, subsequently, used in patient treatment.
METHODS: Six cadaver head specimens were used. Ideal head positioning and various surgical routes were studied. To estimate the extent of surgical exposure provided by this technique, the width of the exposed anterior cranial fossa was measured between the medial margin of the orbits, the optic nerves and the carotid arteries. Three demonstrative patient cases are presented.
RESULTS: Ideal head

INTRODUCTION: Craniosynostoses affect 1/2000 births and their incidence is currently increasing. Without surgery, craniosynostosis can lead to neurological issues due to restrained brain growth and social stigma due to abnormal head shapes. Understanding growth patterns is essential to develop surgical planning approaches and predict short- and long-term post-operative results. Here we provide a systematic review of normal and pathological cranial vault growth models.
MATERIAL AND METHODS: The systematic review of the literature identified descriptive and comprehensive skull growth models wit

Knowledge of variations at the base of the skull may be important for academic, anthropological, forensic, and clinical purposes, especially in computed tomography and magnetic resonance imaging and while performing safe radical surgery. This report presents multiple variations at the base of an adult Indian skull. The variations noted include the presence of foramen of Civinini bilaterally, absence of carotid canal on the left side, reduced size of foramen lacerum on the left side, presence of petro-occipital foramen bilaterally, asymmetry between the jugular fossae, and asymmetrical occipita

A 53-year-old woman presented with right-sided otalgia radiating to the temporal region, angle of the mandible and upper neck. Otoscopy was unremarkable on examination and there were no signs of otitis externa. MRI revealed an infiltrative soft tissue mass to the right lateral aspect of the clivus. Transnasal and CT-guided biopsies were performed, however, these showed either inconclusive or benign tissue. Following multidisciplinary team assessment a diagnosis of spontaneous skull base osteomyelitis was made and treatment with intravenous antibiotics started. Failure to respond to antimicrobi

Skull hyperostosis is a frequently recognised feature of meningioma in feline and human patients, occurring at a frequency of around 4.5% of human cases. Evidence of osteolysis with extension of meningioma into, and in some cases through, the region of skull hyperostosis is much less commonly described in human patients. Here we present a 12-year-old cat with marked skull hyperostosis secondary to an intracranial meningioma, with magnetic resonance imaging and computed tomography evidence of tumour extension into the skull, centrally within the region of hyperostosis. Only a thin layer of bone

PURPOSE: Postoperative cerebrospinal fluid rhinorrhoea (CSFR) remains a frequent complication of endonasal approaches to pituitary and skull base tumours. Watertight skull base reconstruction is important in preventing CSFR. We sought to systematically review the current literature of available skull base repair techniques.
METHODS: Pubmed and Embase databases were searched for studies (2000-2020) that (a) reported on the endonasal resection of pituitary and skull base tumours, (b) focussed on skull base repair techniques and/or postoperative CSFR risk factors, and (c) included CSFR data. Rol

BACKGROUND AND OBJECTIVES: Skull base surgery requires precise 3-dimensional (3D) anatomic understanding and advanced drilling techniques within a confined operative space. Although cadaveric dissection remains the gold standard for training, its availability is limited. This study aimed to evaluate the educational utility of a commercially available full-skull 3D model for skull base surgery training.
METHODS: A full-skull 3D model reproducing the cranial base and brain was used in a hands-on training session involving 8 neurosurgeons, including 2 residents and 6 board-certified surgeons. Mu

Severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2), which causes coronavirus disease 2019 (COVID-19), is highly contagious with devastating impacts for healthcare systems worldwide. Medical staff are at high risk of viral contamination and it is imperative to know what personal protective equipment (PPE) is appropriate for each situation. Furthermore, elective clinics and operations have been reduced in order to mobilize manpower to the acute specialties combating the outbreak; appropriate differentiation between patients who require immediate care and those who can receive telephone

OBJECT: Habitual sleeping positions in infants can produce occipital plagiocephaly, which causes strabismus as well as skull and facial asymmetry. This condition can be managed using a hard helmet, but maintaining an infant in such a device is often intolerable. The authors studied whether the shape of a young canine skull could reform while inside a soft helmet and whether intracranial volume would be preserved by the compensatory growth of the skull.
METHODS: The authors tracked the head sizes of 14 1-week-old beagles who wore long, soft helmets (study group) and seven beagles who did not (

During the past few years the supine sleeping position has successfully been reintroduced for infants in order to prevent sudden infant death syndrome. However, the change in sleeping position has not been accompanied by a soft pillow under the infants head, which has probably caused a higher incidence of acquired skull deformities. These positional skull deformities need to be prevented or treated.

OBJECTIVE: IgG4-related disease (IgG4-RD) is an inflammatory process that uncommonly can present in the skull base and calvarium and mimic a tumor but the nature of this condition is not well summarized in the neurosurgical literature.
METHODS: A review was performed of 2 cases of IgG4-RD in the skull base highlighting the diagnostic challenges with assessment of these skull base lesions, and a systematic review of relevant literature was carried out.
RESULTS: A systematic review of the literature conducted in accordance with PRISMA guidelines identified 113 articles, with 184 cases of IgG4-

Several surgical procedures have been described to correct the deformities associated with craniosynostosis. To simplify the prediction of results, virtual planning techniques and image-guided surgery have been used. Digital planning can be transferred to the operating room using osteotomy and cutting guides or surgical navigation. We describe a novel bone fixation method that allows for anchoring of a cranial dynamic reference frame (DRF) in a steady manner. DRF can be used for registration and as a reference for surgical navigation in an infant's skull. We describe this novel technique to ov

AIM: To investigate the contribution of routine review of submillimetric multiplanar reformats to the diagnosis of skull-base fractures.
METHODS: A prospective analysis was performed of 407 cases referred over a 6-month period for CT of the skull following cranial trauma. The reformatted 5-mm axial sections and subsequently the high-resolution multiplanar reformats (HRMPRs) were viewed on an ADW 4.1 workstation using bone windows and algorithm. All skull-base fractures and related features, recorded by the consensus of two radiologists, were classified as anatomically significant or non-signi
