Breathing patterns in relation to sleep stages in acute unilateral lateral medullary infarction: An exploratory study.

Our aim was to perform an exploratory study of various irregular breathing patterns (IBPs) across different sleep stages in patients with acute unilateral lateral medullary infarction (ULMI) and compare them to apparently healthy individuals. Polysomnography (PSG) was analyzed for IBPs, such as periodic breathing, ataxic breathing and tachypnea. IBPs were found in 52 % of healthy and 90 % of ULMI subjects (p = 0.001) and occurred in long (≥ 10 min) episodes in 8% of healthy and 68 % of ULMI (p < 0.001). In healthy subjects, short (< 10 min) episodes of mild to moderate ataxic breathing were ob…

First rapid eye movement sleep periods and sleep-onset rapid eye movement periods in sleep-stage sequencing of hypersomnias.

OBJECTIVES: Discrimination between narcolepsy, idiopathic hypersomnia, and behavior-induced inadequate sleep syndrome (BIISS) is based on clinical features and on specific nocturnal polysomnography (NPSG) and multiple sleep latency test (MSLT) results. However, previous studies have cast doubt on the specificity and sensitivity of these diagnostic tools. METHODS: Eleven variables of the NPSG were analyzed in 101 patients who were retrospectively diagnosed with narcolepsy with cataplexy (N+C) (n=24), narcolepsy without cataplexy (N-C) (n=38), idiopathic hypersomnia with long sleep period (IHL)

The transition from slow-wave sleep to paradoxical sleep: evolving facts and concepts of the neurophysiological processes underlying the intermediate stage of sleep.

Paradoxical sleep in rats, cats and mice is usually preceded and sometimes followed by a short-lasting (a few seconds) electroencephalogram (EEG) stage characterized by high-amplitude spindles in the anterior cortex and low-frequency theta rhythm in the dorsal hippocampus. The former is an index of advanced slow-wave sleep; the latter is an index of limbic activation since it occurs during active waking and paradoxical sleep. Barbiturates and benzodiazepines extend this intermediate stage at the expense of paradoxical sleep while concomitantly barbiturates suppress the pontine reticular activa

A Deep Transfer Learning Approach for Sleep Stage Classification and Sleep Apnea Detection Using Wrist-Worn Consumer Sleep Technologies.

Obstructive sleep apnea (OSA) is a common, underdiagnosed sleep-related breathing disorder with serious health implications Objective - We propose a deep transfer learning approach for sleep stage classification and sleep apnea (SA) detection using wrist-worn consumer sleep technologies (CST). Methods - Our model is based on a deep convolutional neural network (DNN) utilizing accelerometers and photo-plethysmography signals from nocturnal recordings. The DNN was trained and tested on internal datasets that include raw data from clinical and wrist-worn devices; external validation was performed

Sleep-stage dependent patterning of slowly propagating brain activity.

Endogenous brain activity plays a key role during sleep, facilitating important functions like learning and memory consolidation. Its precise mechanisms are not yet understood. Recent functional MRI (fMRI) research has revealed structured multi-second waves that travel across the cortex along a hierarchical functional gradient during wakefulness. However, how these waves are modulated across different sleep stages remains unclear. Here, we present compelling evidence of significant changes in both the frequency and spatiotemporal patterns of these waves across sleep stages. Notably, during rap…

Non-Intrusive Sleep Staging with Integration of Ballistocardiography and Audio Signals.

This paper investigates the use of audio analysis alongside non-intrusive sensors to measure cardiorespiratory parameters for sleep stage classification. A ballistocardiogram (BCG) detected cardiorespiratory signals through vibrations on the mattress, while an Apple Watch Ultra 2 used optical sensors on the wrist for measurement. The cardiorespiratory signals were processed with non-linear methods and autocorrelation functions to create a dataset from the entire system, which was then trained in a long short-term memory (LSTM) model to classify sleep stages. The output was validated using a po…

Deep Learning in Automatic Sleep Staging With a Single Channel Electroencephalography.

This study centers on automatic sleep staging with a single channel electroencephalography (EEG), with some significant findings for sleep staging. In this study, we proposed a deep learning-based network by integrating attention mechanism and bidirectional long short-term memory neural network (AT-BiLSTM) to classify wakefulness, rapid eye movement (REM) sleep and non-REM (NREM) sleep stages N1, N2 and N3. The AT-BiLSTM network outperformed five other networks and achieved an accuracy of 83.78%, a Cohen's kappa coefficient of 0.766 and a macro F1-score of 82.14% on the PhysioNet Sleep-EDF Exp…

Altered sleep structure in patients with end-stage renal disease.

OBJECTIVE: Although symptoms of sleep disturbances are widely recognized in end-stage renal disease (ESRD), the effect of uremia on sleep structure has not been well investigated. We hypothesized that compared to individuals without ESRD, those with ESRD would have altered sleep structure after controlling for the severity of sleep apnea (SA). METHODS: We studied 57 ESRD patients (42 men) and 57 controls (46 men) who had undergone polysomnography. Control subjects were matched to the ESRD patients by age, body mass index (BMI), frequency of periodic leg movements per hour of sleep, and the fr

Sleep apnoea syndrome and early stage cirrhosis: a pilot study.

OBJECTIVES: Hepatic encephalopathy in patients with end-stage liver cirrhosis is associated with alterations in sleep patterns. Cirrhosis may also affect pulmonary function and it might be involved in the development of obstructive sleep apnoea syndrome (OSAS) in patients with ascites. We carried out a study to evaluate the presence of OSAS in cirrhotic patients without evidence of ascites (early stage cirrhosis). METHODS: We investigated 20 patients with Child A or B cirrhosis (19 and one, respectively) and 10 non-cirrhotic patients with chronic viral hepatitis (disease control group). All s

Sleep stage transition and changes in autonomic function in newborn infants.

The relationships between sleep stage, startle response, electroencephalogram (EEG) pattern, and autonomic function in sleeping full-term infants was examined. Using autoregressive analysis of heart rate variability (HRV), 12 infants at a mean post-conception age of 41.1 weeks were studied. Based on HRV characteristics, the present study shows sympathetic nervous system activation during active sleep (AS) and distinct changes in autonomic function based on the startle response and EEG features during quiet sleep (QS). The findings might reflect structural and functional maturation of the centr

Multimodal Sleep Stage and Sleep Apnea Classification Using Vision Transformer: A Multitask Explainable Learning Approach.

Sleep is an essential component of human physiology, contributing significantly to overall health and quality of life. Accurate sleep staging and disorder detection are crucial for assessing sleep quality. Studies in the literature have proposed PSG-based approaches and machine-learning methods utilizing single-modality signals. However, existing methods often lack multimodal, multilabel frameworks and address sleep stages and disorders classification separately. In this paper, we propose a 1D-Vision Transformer for simultaneous classification of sleep stages and sleep disorders. Our method ex…

Diagnostic value of sleep stage dissociation as visualized on a 2-dimensional sleep state space in human narcolepsy.

BACKGROUND: Type 1 narcolepsy (NT1) is characterized by symptoms believed to represent Rapid Eye Movement (REM) sleep stage dissociations, occurrences where features of wake and REM sleep are intermingled, resulting in a mixed state. We hypothesized that sleep stage dissociations can be objectively detected through the analysis of nocturnal Polysomnography (PSG) data, and that those affecting REM sleep can be used as a diagnostic feature for narcolepsy. NEW METHOD: A Linear Discriminant Analysis (LDA) model using 38 features extracted from EOG, EMG and EEG was used in control subjects to sele

Sleep-stage sequencing of sleep-onset REM periods in MSLT predicts treatment response in patients with narcolepsy.

Current treatment recommendations for narcolepsy suggest that modafinil should be used as a first-line treatment ahead of conventional stimulants or sodium oxybate. In this study, performed in a tertiary sleep disorders centre, treatment responses were examined following these recommendations, and the ability of sleep-stage sequencing of sleep-onset rapid eye movement periods in the multiple sleep latency test to predict treatment response. Over a 3.5-year period, 255 patients were retrospectively identified in the authors' database as patients diagnosed with narcolepsy, type 1 (with cataplexy

Importance of sleep stage- and body position-dependence of sleep apnoea in determining benefits to auto-CPAP therapy.

The influence of sleep stage- and body position-dependence of sleep apnoea on treatment efficacy and compliance between conventional continuous positive airway pressure (CPAP) and auto CPAP therapy was evaluated. Thirty-three newly treated sleep apnoea hypopnoea syndrome (SAHS) patients were randomly allocated to conventional or auto-CPAP therapy. Six patients of each treatment group were classified as having sleep stage- and body position-dependent obstructive breathing abnormalities according to the results of the baseline sleep study. After 3 weeks of treatment, the Epworth sleepiness score
