Cancer Risk Following Smoking Cessation in Korea.

IMPORTANCE: Tobacco smoking is associated with increased risk of various cancers, and smoking cessation has been associated with reduced cancer risks, but it is still unclear how many years of smoking cessation are required to significantly reduce the cancer risk. Therefore, investigating the association of smoking cessation with cancer is essential. OBJECTIVE: To investigate the time course of cancer risk according to the time elapsed since smoking cessation and the benefits of smoking cessation according to the age at quitting. DESIGN, SETTING, AND PARTICIPANTS: This population-based, retr…

Interventions for smoking cessation in hospitalised patients.

BACKGROUND: In 2020, 32.6% of the world's population used tobacco. Smoking contributes to many illnesses that require hospitalisation. A hospital admission may prompt a quit attempt. Initiating smoking cessation treatment, such as pharmacotherapy and/or counselling, in hospitals may be an effective preventive health strategy. Pharmacotherapies work to reduce withdrawal/craving and counselling provides behavioural skills for quitting smoking. This review updates the evidence on interventions for smoking cessation in hospitalised patients, to understand the most effective smoking cessation treat…

Nicotine receptor partial agonists for smoking cessation.

BACKGROUND: Nicotine receptor partial agonists may help people to stop smoking by a combination of maintaining moderate levels of dopamine to counteract withdrawal symptoms (acting as an agonist) and reducing smoking satisfaction (acting as an antagonist). This is an update of a Cochrane Review first published in 2007. OBJECTIVES: To assess the effectiveness of nicotine receptor partial agonists, including varenicline and cytisine, for smoking cessation. SEARCH METHODS: We searched the Cochrane Tobacco Addiction Group's Specialised Register in April 2022 for trials, using relevant terms in t…

Smoking cessation: health system challenges and opportunities.

The systematic integration of evidence-based tobacco treatment has yet to be broadly viewed as a standard-of-care. The Framework Convention on Tobacco Control recommends the provision of support for tobacco cessation. We argue that the provision of smoking cessation services in clinical settings is a fundamental clinical responsibility and permits the opportunity to more effectively assist with cessation. The role of clinicians in prioritising smoking cessation is essential in all settings. Clinical benefits of implementing cessation services in hospital settings have been recognised for three…

[The e-cigarette - means of smoking cessation?].

After several years of declining tobacco consumption, the number of smokers in Germany is currently stagnating or rising again. The reasons seem to be manifold, e. g. stress caused by the pandemic with social isolation, rising cost of living and war in Europe. With tobacco use still widespread in the German population, evidence-based tobacco cessation is rarely implemented.According to recent studies, e-cigarettes are involved in the pathogenesis of lung disease, cardiac and vascular damage. In addition, their ingredients also have carcinogenic effects. However, clinical studies on long-term us

[Quit smoking: a personalized cessation program required].

OBJECTIVE: With 13 million smokers in France in 2019, smoking remains a major health issue. The LAST (Lieux d'accompagnement à la santé sans tabac [Locations for Health Support without Tobacco]) project set up in New-Aquitaine aims to encourage smokers to get help to quit smoking. METHOD: In order to identify the barriers and levers to be mobilized to encourage smokers to seek help and then build with them the ideal cessation program, an individual qualitative study based on semi-structured interviews was conducted in May 2019 with 19 smokers at the contemplation or preparation step (transthe

Effectiveness of National Residential Smoking Cessation Program.

We aimed to investigate the effectiveness of the Korean national five-day residential smoking cessation program and the factors affecting the long-term smoking cessation of participants. The residential smoking cessation program (2017-2018) recruited smokers with a smoking duration ≥ 20 years and who have attempted to quit smoking more than twice and/or smokers with chronic morbidities. Participants underwent an intensive intervention, including individual psychological therapy, group therapy, medical counseling, and pharmacotherapy. The 6-month continuous abstinence rate (CAR) was assessed vi…

Evidence against e-cigarettes for smoking cessation.

Electronic cigarettes (e-cigarettes) have been touted as a safer alternative to cigarettes and even as a smoking cessation aid. The health risks associated with smoking are well known, and smoking cessation has been studied extensively with options including behavioral therapy and pharmacotherapy (nicotine replacement therapy [NRT], varenicline, and bupropion). Several studies analyzed the effects of e-cigarettes as a smoking cessation aid. When used for smoking cessation, those who successfully abstain from cigarette smoking have a higher rate of continuation on e-cigarettes than NRT or pharm…

Smoking Cessation and Cardiac Rehabilitation: A Priority!

Cardiac rehabilitation programmes afford the opportunity to enhance cardiovascular health and reduce the risk of subsequent cardiac events. They permit the management of cardiac risk factors while addressing significant psychosocial and vocational issues. Tobacco addiction is the most important of the modifiable cardiac risk factors, and smoking cessation is the most important secondary prevention strategy. Cessation is more likely when cessation pharmacotherapy is accompanied by supportive advice and strategic assistance. A systematic approach to the delivery of cessation services, such as th…

[Smoking cessation amongst patients with mental disorders].

Cigarette smoking is very prevalent amongst patients suffering from mental disorders. In recent years additional scientific proof has been gathered regarding nicotine addiction mechanisms as effective treatments for smoking cessation. Some of these therapies are included in the "Basket of Health Services" of the State of Israel. Despite high smoking prevalence, the success rate for smoking cessation is low amongst patients with mental disorders. In this paper the possible reasons for this phenomenon are presented, as well as the possible biological and behavioral models that support this sever…

Sun protection, probiotics and parental smoking cessation.

PURPOSE OF REVIEW: This article addresses three areas where new research demonstrates the potential to impact the health of children and adolescents: sun protection, probiotic use, and parental smoking cessation. RECENT FINDINGS: Skin cancer rates are rising, and sun exposure in childhood and exposure to tanning beds are important preventable risk factors. Research on the use of probiotics to treat a variety of intestinal and nonintestinal diseases is expanding. By counseling parents about smoking cessation, pediatricians can assist in decreasing childhood exposure to harmful second-hand and …

Interventions for smoking cessation in hospitalised patients.

BACKGROUND: Smoking contributes to reasons for hospitalisation, and the period of hospitalisation may be a good time to provide help with quitting. OBJECTIVES: To determine the effectiveness of interventions for smoking cessation that are initiated for hospitalised patients. SEARCH METHODS: We searched the Cochrane Tobacco Addiction Group register which includes papers identified from CENTRAL, MEDLINE, EMBASE and PsycINFO in December 2011 for studies of interventions for smoking cessation in hospitalised patients, using terms including (hospital and patient*) or hospitali* or inpatient* or a…

Abuse and smoking cessation in clinical practice.

AIMS AND OBJECTIVES: This discursive paper explores issues of abuse during smoking cessation counselling. BACKGROUND: During a training session for a smoking cessation intervention pilot study, nurses expressed concerns about issues of abuse that had previously surfaced during cessation counselling in their practice. Abused women are more likely to smoke. As guidelines recommend integrating cessation interventions into practice, issues of abuse are likely to surface. METHODS: A literature review and synthesis of abuse and smoking cessation was undertaken to arrive at recommendations for prac…

Pharmacotherapy for smoking cessation: present and future.

Tobacco dependence is a chronic disease that often requires repeated interventions and multiple attempts to quit. To date, three medications are FDA-approved for smoking cessation: nicotine replacement therapy, sustained-release bupropion, and varenicline. These treatments are effective across a broad range of populations, and are recommended for all smokers, including those with psychiatric or addictive comorbidity. Less is known however concerning the benefit-risk profile of these medications in pregnant women and adolescents. With these limitations in mind, clinicians should encourage and o…

Smoking cessation among sheltered homeless: a pilot.

OBJECTIVE: To test the feasibility and effect of a smoking cessation intervention among sheltered homeless. METHODS: Homeless smokers were enrolled in a 12-week group counseling program plus pharmacotherapy (n = 58). RESULTS: The mean number of sessions attended was 7.2; most participants used at least one type of medication (67%); and 75% completed 12-week end-of-treatment surveys. Carbon-monoxide-verified abstinence rates at 12 and 24 weeks were 15.5% and 13.6% respectively. CONCLUSION: Results support the feasibility of enrolling and retaining sheltered homeless in a smoking cessation pr…

Smoking cessation with varenicline: a suicidal fatality.

The most effective smoking cessation programs involve a combination of pharmacotherapy and behavioral and/or cognitive counseling to improve abstinence rates. Varenicline (Champix in France and the U.K.), the most recently approved agent for tobacco cessation, is the first drug in a new class (alpha4beta2 partial agonist) that binds to the nicotinic receptors to release dopamine and alleviate withdrawal symptoms. As the literature reports psychiatric disorders being linked to varenicline as an issue, we describe the case of a man who committed suicide while receiving therapy with this drug. Th…

Endocrine and metabolic effects of smoking cessation.

BACKGROUND: Cardiovascular, pulmonary, and oncological hazards of tobacco smoking have been well studied. Smoking may also have multiple effects on endocrine and metabolic systems affecting pituitary, thyroid, and adrenal glands; testicular and ovarian function; as well as energy balance; lipid, and glucose metabolism; and insulin resistance. Less is known about hormonal and metabolic effects that patients may experience while quitting smoking. SCOPE: The objective of this article is to review systematically data on the endocrine and metabolic effects of smoking cessation. Articles based upon…
