Soft tissue and joint infection.

Soft tissue infection may be acute or chronic and may be introduced by the haematogenous route, or by inoculation, including surgical infection, or spread from other areas. The spectrum of infecting organism differs in primary infection, in immunodeficiency or when foreign bodies, including prostheses, are present. Bacterial infections are usually more rapid than those due to fungi or atypical organisms. Inflammation usually begins as cellulitis, proceeding through necrosis to cavitation and abscess formation, sometimes complicated by haemorrhage. Imaging, apart from MRI in selected cases, is

[Anaerobic infections of the soft tissues].

Anaerobic soft tissue infections are still life threatening infections. Although their frequency is actually moderate; they remain severe because physicians are often insufficiently aware of them. Although the classification between myonecrosis and necrotizing fasciitis is still valid, these infections share so many points in common that they can be considered together. Their origin is often traumatic or surgical but may also be secondary to an ulcer or a small wound in a high-risk patient: arteriosclerotic, diabetic. Hypoxia, traumatic muscle crush, heavy bacterial contamination as well as in…

Childhood skin and soft tissue infections: new discoveries and guidelines regarding the management of bacterial soft tissue infections, molluscum contagiosum, and warts.

PURPOSE OF REVIEW: Pediatric skin and soft tissue infections (SSTIs) constitute a significant number of office-based pediatric visits. With SSTIs on the rise, it is not only important to effectively treat the individual, but to do so appropriately and cost-consciously. In this article, we highlight new research related to the treatment of bacterial skin infections, molluscum contagiosum, and cutaneous warts, with the goal of guiding pediatricians in their practice against these common skin conditions. RECENT FINDINGS: Recent data supports the use of topical antibiotics for noncomplicated impe…

Skin and Soft Tissue Infections in the Diabetic Patient.

Diabetes mellitus predisposes to skin and soft tissue infections (SSTIs), particularly but not exclusively in the feet. Hyperglycemia causes inflammation that disrupts the host immune system and leads to vascular and neurological injury, both of which contribute to infection risk and challenge diagnosis and management. A greater incidence of drug-resistant, polymicrobial, and deep SSTIs among persons with diabetes impacts antimicrobial selection and the need for surgical intervention. Diabetic foot ulcers are at substantial risk of recurrent infection and require a multifaceted preventative ap…

Outcomes and Predictors of Fungal Necrotizing Soft Tissue Infections.

INTRODUCTION: Fungal necrotizing soft tissue infections (NSTIs) are often diagnosed late due to the low likelihood, resulting in poor patient outcomes. Traditionally NSTI are treated with emergent surgical debridement followed by serial interventions. Adjunctive antibiotics are aimed at common bacterial infections along with toxin reduction; however, fungal treatment is often delayed. This study aimed to identify predictors, treatment patterns, and outcomes in fungal NSTIs. METHODS: A retrospective review of patients with NSTIs (n = 634) from a single tertiary academic center was conducted fr…

Necrotizing Soft Tissue Infections of the Hand and Wrist.

Necrotizing soft tissue infections of the upper extremity threaten life and limb. Their presentation is often difficult to recognize and multiple factors contribute to the varied presentation, speed of spread, and morbidity of the disease process. The only treatment to be definitively shown to improve outcomes is prompt surgical treatment. It should be expected to perform multiple debridements as well as reconstructive procedures once the infection and necrosis has been arrested. The use of a multidisciplinary team is mandatory to optimize care for these patients. Despite advances in knowledge…

Cotrimoxazole and clindamycin in skin and soft tissue infections.

PURPOSE OF REVIEW: The aim of this study was to present recent microbiological, experimental, clinical and tolerance data for cotrimoxazole and clindamycin in the specific field of skin and soft tissue infections (SSTIs). RECENT FINDINGS: Staphylococcus aureus and streptococci remain the leading cause of SSTIs. Cotrimoxazole is a good anti-Gram-positive agent with preserved activity against methicillin-susceptible and methicillin-resistant S. aureus (MRSA) and streptococci. Although clindamycin has good methicillin-susceptible S. aureus activity, a growing number of resistant MRSA and strepto…

Skin and Soft Tissue Infections in the Emergency Department.

This article covers the diagnosis and treatment of skin and soft tissue infections commonly encountered in the emergency department: impetigo, cutaneous abscesses, purulent cellulitis, nonpurulent cellulitis, and necrotizing skin and soft tissue infections. Most purulent infections in the United States are caused by methicillin-resistant Staphylococcus aureus. For abscesses, we emphasize the importance of incision and drainage. Nonpurulent infections are usually caused by streptococcal species and initial empiric antibiotics need not cover methicillin-resistant Staphylococcus aureus. For uncom…

Linezolid versus vancomycin for skin and soft tissue infections.

BACKGROUND: The morbidity and treatment costs associated with skin and soft tissue infections (SSTIs) are high. Linezolid and vancomycin are antibiotics that are commonly used in treating skin and soft-tissue infections, specifically those infections due to methicillin-resistant Staphylococcus aureus (MRSA). OBJECTIVES: To compare the effects and safety of linezolid and vancomycin for treating people with SSTIs. SEARCH METHODS: In May 2013 we conducted searches of the following databases: Cochrane Wounds Group Specialised Register; The Cochrane Central Register of Controlled Trials (CENTRAL)…

Necrotizing soft-tissue infections in obstetric and gynecologic patients.

For the clinician, necrotizing soft-tissue infections have remained a daunting opponent since the first writings on the subject over 2000 years ago. Early disease may be incorrectly diagnosed as cellulitis, and this delay in correctly diagnosing and expeditiously proceeding to radical surgical debridement may lead to a high degree of mortality. Although several inciting events and risk factors have been described that allow for the development and progression of this disease, the diagnosis is still made clinically. Only aggressive surgical management in combination with broad-spectrum antibiot

[Pathogenesis, classification and diagnosis of necrotizing soft tissue infections].

Necrotizing soft tissue infections are caused by a variety of pathogens and may affect different types of soft tissue. Even today mortality and lethality are very high. The primary symptoms of necrotizing soft tissue infections are local pain out of proportion, swelling, erythema and crepitation in cases of subcutaneous gas. A systemic inflammatory response syndrome (SIRS) is often associated. During the last decades early recognition and initiation of an adequate therapy were able to reduce lethality to an average of 20%. The physical examination remains the diagnostic gold standard and may b…

Epidemiology and prevention of skin and soft tissue infections.

Staphylococcus aureus and Streptococcus pyogenes remain the 2 major organisms responsible for most common skin and soft tissue infections. While methicillin-resistant S aureus (MRSA) typically is considered a hospital-acquired organism, outbreaks of community-acquired MRSA are being reported. Simple interventions can reduce the spread of this infection. Erysipelas is a streptococcal syndrome with high morbidity. The epidemiology has changed, with an increasing number of cases involving the lower extremities. Risk factors for erysipelas can be identified, which allow clinicians to target patien…

Skin and Soft Tissue Infections in Injection Drug Users.

Skin and soft tissue infections (SSTIs) are common among injection drug users (IDUs). Subcutaneous and intramuscular injection ("skin-popping") and the injection of "speedballs" (a mixture of heroin and cocaine) are important risk factors for SSTIs in this patient population. Female IDUs appear to be at greater risk of SSTIs than male IDUs, probably because of more difficult venous access. There are conflicting data regarding the impact of HIV and human T-cell lymphotrophic virus II infection on the risk of SSTIs in IDUs; however, an expanding body of evidence suggests that immunosuppressive e…

[Harmless or life threatening? Properly assessing soft tissue infections].

Soft tissue infections are highly diverse in presentation, and their severity is readily underestimated. Classifications in the literature are not uniform. Clinically, a recommended classification is based on the spread of the infection into deeper regions: superficial and deep soft tissue infections. Superficial infections, that is, those confined to the cutis and subcutis, include furuncles, erysipelas and phlegmons (acute suppurative inflammation). The second group--infection affecting fascia and muscle--include both classical gas gangrene and necrotizing fasciitis which, despite aggressive…
