BACKGROUND: Fibro-osseous lesions include a variety of bone lesions with different clinical and histopathological features.
CASE DESCRIPTION: We report a case of cemento-ossifying lesion involving the left ethmoid and sphenoid sinus in a 17-year-old male patient. Computed tomography showed an expansile hyperdense lesion with sclerosed peripheral mantle epicentered on the left ethmoid and sphenoid sinus and extending into the left nasal cavity. Magnetic resonance imaging was done for preoperative planning. Transnasal endoscopic resection was performed and histopathological examination confirme

OBJECTIVE: To report the case of a woman who had combined aplasia of sphenoid, frontal, and maxillary sinuses accompanied by ethmoid sinus hypoplasia.
METHODS AND RESULTS: A 47-year-old woman presented with complaints of headache, nasal obstruction, and postnasal drainage. She had no previous history of either facial trauma or systemic diseases affecting the skeletal system. She had a history of functional endoscopic sinus surgery twice in an other institution. However, the surgeries did not significantly alter her symptoms. Coronal and axial computed tomographic scans of the nose and the par

Isolated sphenoid sinusitis is often misdiagnosed because of its rarity and varied clinical presentation. Presentation is often both subtle and suggestive of other intracranial etiologic sites, as determined by the anatomic relationships of the sphenoid sinus. As well, the microbiology of acute sphenoid sinusitis has not yet been clearly defined. We present three cases of acute sphenoid sinusitis and a review of the English medical literature. We isolated Staphylococcus aureus in two of our three patients at the Hospital for Sick Children and also found it to be the predominant organism in 48%…

OBJECTIVES: Endonasal sphenoidotomy is a high risk operation: fatal complications due to direct lesion to the internal carotid artery have been reported. We conducted a prospective analysis of carotid prolapse within the sphenoid cavity in order to draw attention to this dangerous anatomic variant.
METHODS: Tomodensitometry of the sinuses was performed in 150 patients (mean age 35 years; range 20-70) with inflammatory nasal symptoms of less than one year duration (mean 5.2 months). Cases with benign or malignant tumours, polyposis or a past history of sinus trauma were excluded. Transversal s

Developmental patterns of sphenoidal sinuses were determined from lateral radiographs of 1702 patients from newborn to 15 years of age. Pneumatization of the conchal sinuses was seen as early as 6 months of age; 90% showed sphenoidal sinus pneumatization by 4 years of age; 100% by 10 years of age. Extension of pneumatization into the basisphenoid may be evident after 4 years of age, and 95% showed basisphenoidal pneumatization after age 12. An apparent lack of pneumatization of the sphenoidal sinuses in patients over 10 should stimulate investigation for sphenoidal sinus disease. Although isol

A case of sphenoid sinusitis and internal carotid aneurysm presenting with multiple cranial nerve palsy is reported. A 46-year-old man with high fever and neck rigidity developed progressive visual loss and ophthalmoplegia on both sides, and ptosis, mydriasis and facial numbness on the right side. CT scan and MRI revealed abnormal shadow of the sphenoid sinus and the cavernous sinus. Sphenoidectomy and antibiotic therapy improved left visual acuity, but the right visual disturbance remained. Cerebral angiography performed 20 days after the operation, showed an aneurysm of the cavernous portion…

Acute isolated sphenoid sinusitis in children is relatively rare, often presents with atypical symptoms, and may result in intracranial complications; therefore, early diagnosis is essential. We report the case of a 14-year-old girl who presented with a headache and a positive Kernig sign. Computed tomography (CT) demonstrated fluid retention in the sphenoid sinus, and lumbar puncture showed no abnormalities in the cerebrospinal fluid (CSF). Based on these findings, she was diagnosed with acute isolated sphenoid sinusitis without intracranial complications. However, the patient showed a poor r…

The abducens nerve, which is vulnerable because of its complex anatomy at the skull base, is seldom affected by acute or severe sphenoid sinusitis. Notably, abducens nerve palsy following asymptomatic chronic rhinosinusitis (CRS) in a healthy young individual after a mild upper respiratory infection (URI) remains undocumented in the literature. Herein, we report a case of acute unilateral abducens neuropathy in a healthy 35-year-old woman with CRS in the ipsilateral sphenoid sinus, following a mild URI 2 weeks earlier. She presented with sudden-onset diplopia, was afebrile, and had normal seru…

Miller Fisher syndrome (MFS), a variant of Guillain-Barré syndrome, is a rare disorder typically characterized by a triad of ataxia, areflexia, and ophthalmoplegia, which may have a highly variable clinical presentation. We report a case of MFS in a 45-year-old female presenting with sphenoid sinusitis and sixth nerve palsy. She underwent endoscopic sphenoid sinusotomy without improvement, had postoperative deterioration, was diagnosed with MFS, and was treated with intravenous immunoglobulin with complete response. Because of the potential severity of Guillain-Barré syndrome, great vigilance …

A previously healthy 56-year-old woman presented with right-sided ophthalmic pain and diplopia following headache and fever. A neurological examination revealed 3rd and 6th right cranial nerve palsies. Brain magnetic resonance imaging (MRI) and 3D-computed tomography (CT) angiography (CTA) showed right-sided sphenoid sinusitis, cavernous sinus thrombophlebitis, and aneurysms in the right intracavernous carotid artery and in a portion of internal carotid-posterior communicating artery. We diagnosed her condition as cavernous sinus syndrome with an infectious aneurysm secondary to sphenoiditis; …

Acute inflammatory optic neuritis (retrobulbar optic neuritis) is an uncommon but described complication of acute sinusitis. It is due to a close anatomic relationship between the sinus and the optic nerve. Inflammatory mechanism is dominant with a possible bilateral neuritis, especially among children. In the reported case, as usual in childhood, visual acuity loss has been particularly severe, and recovery slow (it takes usually 6 weeks) but almost complete. Primary treatment is medical, with antibiotics and corticosteroids that must be prescribed for a prolonged time.

OBJECTIVE: The sphenoid sinus is a key anatomic structure in transsphenoidal procedures due to its central location and proximity to vital structures. A comprehensive understanding of the dimensional relationship between the sphenoid sinus and sella is essential for advancing transsphenoidal surgery and enhancing its safety. Therefore, this study aims to quantitatively analyse the relationship between the volume and linear dimensions of these structures using specialized radiologic software on adult CT images.
METHODS: A total of 220 paranasal sinus computed tomography scans were evaluated. S

Although endoscopic sinus surgery (ESS) is a standard procedure for adults and is considered a safe and effective procedure in children as well, some uncertainty remains when it comes to children, especially with regard to the future facial and sinus development. Overall, limited ESS does not affect sinus development, and extended ESS does not affect facial features. However, whether or not full-house ESS affects the sinus development, especially the frontal and sphenoid sinuses, remains unclear. The authors herein report a 15-year-old child who received full-house ESS and later re-examined th
