Secondary spinal cord changes and spinal deformity following traumatic spinal cord injury.

Secondary spinal cord changes can follow spinal cord injuries (SCIs). This retrospective study was to uncover the chronic secondary changes that affect the spinal cord following severe injuries and to evaluate the influence of residual spinal deformity in the development of posttraumatic spinal cord changes. Fifty-eight patients (39 male, 19 female) with complete traumatic SCI and recent Magnetic resonance imaging (MRI) follow-up were reviewed retrospectively. A minimum of 2 years duration between trauma and MRI study was required (mean 2.9 years [2.1-4.7]). Two groups of patients were formed:…

Relationship between Spinal Cord Volume and Spinal Cord Injury due to Spinal Shortening.

Vertebral column resection is associated with a risk of spinal cord injury. In the present study, using a goat model, we aimed to investigate the relationship between changes in spinal cord volume and spinal cord injury due to spinal shortening, and to quantify the spinal cord volume per 1-mm height in order to clarify a safe limit for shortening. Vertebral column resection was performed at T10 in 10 goats. The spinal cord was shortened until the somatosensory-evoked potential was decreased by 50% from the baseline amplitude or delayed by 10% relative to the baseline peak latency. A wake-up te

Implications of the gut microbiome in spinal cord injuries.

Spinal cord injuries (SCIs) present complex challenges in medical treatment and rehabilitation, profoundly affecting the patient's physiological and neurological status. Emerging research on the gut microbiome has unveiled its potential role in influencing SCI outcomes and recovery. The gut microbiome undergoes significant changes following SCIs, which influence systemic inflammation and increase susceptibility to secondary complications, such as infections and chronic pain. These effects are linked to altered permeability, immune system dysregulation, and activation of the gut-brain axis, whi…

Vena cava filters in spinal cord injuries: evolving technology.

BACKGROUND: Asymptomatic deep venous thrombosis (DVT) has been reported in 60% to 100% of persons with spinal cord injury (SCI). Several guidelines have been published detailing recommended venous thromboembolism (VTE) prophylaxis after acute SCI. Low-molecular-weight heparin, intermittent pneumatic compression (IPC) devices, and/or graduated compression stockings are recommended. Vena cava filters (VCFs) are recommended for secondary prophylaxis in certain situations. OBJECTIVE: To clarify the use of vena cava filters in patients with SCI. METHODS: Literature review. RESULTS: Prophylactic

Combined injuries of the brachial plexus and spinal cord.

A total of 11 patients with combined traumatic injuries of the brachial plexus and spinal cord were reviewed retrospectively. Brachial plexus paralysis in such dual injuries tends to be diagnosed and treated late and the prognosis is usually poor. The associated injuries, which were all on the same side as the plexus lesion, were to the head (nine cases), shoulder girdle (five), thorax (nine) and upper limb (seven). These other injuries were responsible for the delayed diagnosis of brachial plexus paralysis and the poor prognosis was probably because of the delay in starting treatment and the

Management of pediatric cervical spine and spinal cord injuries.

STANDARDS: There is insufficient evidence to support diagnostic standards. GUIDELINES: In children who have experienced trauma and are alert, conversant, have no neurological deficit, no midline cervical tenderness, and no painful distracting injury, and are not intoxicated, cervical spine x-rays are not necessary to exclude cervical spine injury and are not recommended. In children who have experienced trauma and who are either not alert, nonconversant, or have neurological deficit, midline cervical tenderness, or painful distracting injury, or are intoxicated, it is recommended that anterop

Bone mineral density in patients with spinal cord injuries.

The bone mineral density (BMD) of 157 patients with a spinal cord injury (SCI) was measured using dual-photon absorptiometry (Lunar DP-4). The BMD of the second to fourth lumbar vertebrae (L2-L4) and the right femoral neck was measured and the mean density registered in grams per square centimetre. The likelihood of a fracture was based on the percentage of BMD in young normals and the degree of risk of fracture. The following criteria were used to separate the patients: (1) complete or incomplete immobility due to the SCI; (2) the time between the SCI and measurement of BMD was short (<or= 3

[Possibility of controlling orthostatic instability in spinal cord injuries].

Hemodynamics were studied in 10 patients with high (cervical) spinal cord injury in a change to a vertical position. They were fitted with special antioverexertion gear (AOG) consisting of an abdominal, two thigh, and two knee inflatable rubber cuffs in which pressure of +50 mm Hg was produced. Measurements were taken every minute of arterial pressure, volumetric rate of blood flow in the upper limb (by venous-occlusion plethysmography), cardiac output and stroke volume (by body integral rheography); peripheral resistance and vascular resistance were calculated from a formula. Change of the pa

Acute spinal cord injuries in the Lebanon War, 1982.

Our experience with 17 patients with spinal cord injuries (SCI) acquired in the Lebanon War, 1982, is described. The SCI were due to gunshot wounds in 12 patients and to other causes in 5. Two laparotomies and one thoracotomy were performed. Corticosteroids were not seen to influence recovery, nor was laminectomy, which was performed in three cases. Complications such as pressure sores, hydronephrosis, ileus and deep vein thrombosis were rare or did not occur. Three high quadriplegics died. Based on our experience, we recommend conservative treatment and rehabilitation in acute SCI.

Extinction and synesthesia in patients with spinal cord injuries.

Extinction and synesthesia were studied in 50 patients with spinal cord injury with various levels and extents. Extinction was found in 20 (40 per cent) and synesthesia in 6 (12 per cent) of the 40 males and 10 females. No correlations were found between either of the two phenomena and parameters of patient's age, lesion's age, lesion's level and extent, or accompanying head injury. The latter occurred in 22/50 patients (44 per cent), diagnosed by a history of amnesia. Extinction was tested by synchronous double stimulation unilaterally and bilaterally, symmetrically and asymmetrically. It was
