Engaging Black Women in Stress Urinary Incontinence Research Prioritization Through Intentional Recruitment and Alliance With Community Partners.

INTRODUCTION: Despite the disproportionate impact of stress urinary incontinence (SUI) on Black women and their underrepresentation in research, strategies for effectively engaging Black women remain underexplored. Here, we share insights gained from a Patient-Centered Outcomes Research Institute-funded patient engagement project on SUI in which increasing racial diversity of participants was prioritized and achieved. METHODS: A multiphase patient and stakeholder engagement project, conducted from June 2021 to November 2023, sought to train SUI patient partners and identify patient-generated

Patient and public involvement in the co-design and assessment of unobtrusive sensing technologies for care at home: a user-centric design approach.

BACKGROUND: There is growing interest in developing sensing solutions for remote health monitoring to support the safety and independence of older adults. To ensure these technologies are practical and relevant, people-centred design is essential. This study aims to explore the involvement of various stakeholders across different developmental stages to inform the design and assess the capabilities of unobtrusive sensing solutions being developed as part of the Advanced Care Research Centre (ACRC), Edinburgh, UK. METHODS: This study was conducted in two phases. In Phase I (Ideation), discussi

Implementing Shared Decision-Making in Haemophilia Care: Best Practice Recommendations From a Western European Multi-Stakeholder Delphi Process.

INTRODUCTION: Shared decision-making (SDM) is a collaborative process in which people with haemophilia (PWH) and healthcare professionals (HCPs) make treatment decisions informed by the best available evidence and aligned with patient values, preferences and goals. The expanding treatment landscape, including standard, extended and ultra-half-life factors, rebalancing agents, bispecific antibodies and gene therapy, has made SDM an essential element of quality haemophilia care. Despite recognition of its importance in international health policy frameworks and haemophilia-specific guidelines, i

Co-designing policy, practice, and research directions for whole-school physical activity: towards sustainable culture change.

BACKGROUND: Whole-school physical activity (WSPA) approaches are recognised globally as a key investment for increasing children's activity levels. Their success depends on translating national policy into effective school practices through systems-level thinking and multi-stakeholder collaboration. This study aimed to use an experience-based co-design process to produce and prioritise future directions for WSPA policy, practice and research by engaging international stakeholders to identify key roles, needs, and actionable recommendations. METHODS: The co-design process took place during a t

Expanding the definition of unmet medical need: Findings from a multistakeholder workshop.

BACKGROUND: Unmet medical need (UMN) is frequently referenced in health policy and health care decision-making, including the Centers for Medicare & Medicaid Services' (CMS) Drug Price Negotiation Program (DPNP). However, the concept of UMN is often defined inconsistently and has a narrow focus, primarily centered on clinical effectiveness and the availability of treatment. This approach may overlook the wider needs of patients, caregivers, and society as a whole. A broader conceptualization of UMN has been proposed, yet there remains a lack of an actionable implementation framework. OBJECTIV

Advancing the measurement of caregiver impact: Data collection priorities aligned with caregiver, health technology assessment, and policy stakeholders.

BACKGROUND: Family caregiving plays a central role in the US health care system, with substantial economic, health, and policy implications. In 2024, the estimated economic value of unpaid caregiving exceeded $1 trillion, with tens of millions of individuals providing care nationwide. However, caregiver impacts are inconsistently measured and rarely incorporated into clinical evidence, economic evaluations, and health technology assessments, limiting their use in coverage, formulary, and policy contexts. As a result, health care decision-making may not fully capture the broader benefits, costs

Navigating the Generosity of Medicines (Gen-MED): characterizing the complex relationships of medication donations in Japan through stakeholder and social network analysis.

BACKGROUND: Anthropogenic and natural hazards increase reliance on donated medicines. However, inappropriate donations (e.g., expired, unlabeled, or poorly matched to local needs) remain common and create negative impacts on disaster-affected communities. In Japan, the coordination mechanisms and roles among stakeholders remain insufficiently characterized. This study aimed to identify and characterize stakeholders involved in the donated medicine cycle in Japan and to examine their relationships. In addition, this study also obtained stakeholder opinions on our proposed collaborative initiati

Implementing innovative financing mechanisms in global health - perspectives of stakeholders from global health organisations: a qualitative study.

BACKGROUND: The COVID-19 pandemic and recent geopolitical shifts have strained traditional funding for global health, increasing the interest in innovative financing mechanisms. We explore how international organisations perceive and implement these mechanisms as complements to traditional funding. METHODS: In this qualitative study, we conducted semi-structured interviews with eleven senior experts from major global health organisations, gathered via purposive and snowball sampling between May and July 2024. We analysed data using systematic coding and thematic analysis, with trustworthiness

Caesarean section on maternal request - a qualitative study of stakeholders´ views.

BACKGROUND: Caesarean section on maternal request (CSMR) raises ethical and clinical challenges despite Sweden's low overall caesarean section (CS) rate. National written recommendations are restrictive, yet regional differences suggest unequal care. While previous research has focused on women and healthcare professionals, little is known about the views on CSMR of policymakers and other key stakeholders. The aim of this study was to investigate how stakeholders in the Swedish healthcare system view CSMR in relation to medical considerations, individual autonomy and societal values. METHODS:

Gabon's Experience: Factors Enabling and Impeding the COVID-19 Response.

This case study describes the country-level experience and response to the COVID-19 pandemic in Gabon between December 2020 and December 2022. We structured the presentation of COVID-19 response as an action cycle with five elements: (1) engagement, (2) assessment and surveillance measures used to track new cases, (3) planning, (4) action/implementation, and (5) evaluation. We describe the participatory monitoring and evaluation (M&E) process implemented in collaboration with the WHO Regional Office for Africa M&E team. The M&E system was used to organize and make sense of emerging data regard

Qualitative Systematic Review and Meta-Aggregation of Stakeholders' Perspectives on Stepped Care in Facilitating Return to Community After Hospitalization for Anxiety and Depression.

The stepped care model aims to dynamically align the intensity of care with the patients' needs, adjusting mental health interventions accordingly. While stepped up care for mental illness has been widely researched, there is limited evidence on how stepped down care supports recovery after acute mental illness. This JBI qualitative systematic review sought to understand stakeholders' perspectives of stepped care in supporting patients' transitions from acute mental healthcare to community settings. CINAHL, Medline, Embase, PsycINFO and Web of Science were searched, and data were synthesized u

Anticipating in vitro gametogenesis: Hopes and concerns for IVG among diverse stakeholders.

In vitro gametogenesis (IVG), the reconstitution of germ cell development in vitro, is an emerging stem cell-based technology with profound implications for reproductive science. Despite researchers' long-term goals for future clinical applications, little is currently known about the views of IVG held by the stakeholders potentially most affected by its introduction in humans. We conducted focus groups and interviews with 80 individuals with lived experience of infertility and/or LGBTQ+ family formation in the US, two intersecting groups of potential IVG users. Respondents expressed hope that

Addressing key gaps in implementation of mosquito larviciding to accelerate malaria vector control in southern Tanzania: results of a stakeholder engagement process in local district councils.

BACKGROUND: Larval source management was historically one of the most effective malaria control methods but is now widely deprioritized in Africa, where insecticide-treated nets (ITNs) and indoor residual spraying (IRS) are preferred. However, in Tanzania, following initial successes in urban Dar-es-Salaam starting early-2000s, the government now encourages larviciding in both rural and urban councils nationwide to complement other efforts; and a biolarvicide production-plant has been established outside the commercial capital. This study investigated key obstacles and opportunities relevant t

Beyond the Guidelines: How We Can Improve Healthcare for People With Tuberous Sclerosis Complex Around the World.

BACKGROUND: Tuberous Sclerosis Complex International (TSCi) is a consortium of organizations that supports individuals with tuberous sclerosis complex (TSC) around the world. To improve care for TSC on a global level, TSCi identified the need to expand understanding about existing resources available in other countries, what individuals and caregivers value in TSC care, key gaps between needs and reality in each country, and ways these gaps can be addressed by advocacy organizations around the world. METHODS: An iterative, mixed methods approach (the Improving Care project) was adopted to inc

Realist synthesis of co-production of services in a local authority context: what works, for whom, how and in what circumstances?

BACKGROUND: Co-production is increasingly promoted as a means of improving the design and delivery of local authority services, yet evidence on how and why it works remains limited. This realist synthesis explored how co-production operates within complex local authority systems, identifying the circumstances and mechanisms that enable or constrain meaningful involvement of stakeholders in co-production processes and sustained change to ensure that services are co-produced with professionals and those with lived experience. METHODS: Scoping searches identified key literature on co-production
