Antagonism of phase II succinylcholine block by neostigmine.

The neuromuscular effect of neostigmine, 1.25 mg/70 kg, was assessed in 40 adult patients 10 min after cessation of a succinylcholine infusion. The patients had received a thiopental-nitrous oxide anesthetic supplemented by halothane or fentanyl during which they were given at least 5 mg/kg succinylcholine over more than 90 min. Train-of-four monitoring was used. Neostigmine accelerated recovery of neuromuscular function in all patients. The degree of recovery was directly related to the train-of-four ratio, and the results in patients who had received halothane were no different from those wh…

The neuromuscular response of infants to a continuous infusion of succinylcholine.

The response of the adductor of the thumb to ulnar nerve stimulation (0.1 Hz) was evaluated during continuous infusion of succinylcholine (SCh) in 20 infants anesthetized with halothane (1%) and N2O2/O2. Train-of-four stimulation (2 Hz for 2 s) was used to differentiate between phase I and phase II block. Some infants were very resistant to the neuromuscular effects of SCh. These infants (Group 1) were younger in age, 57 +/- 15 days (mean +/- SE) and required 24.6 +/- 3.3 mg X kg-1h-1 SCh to achieve more than 90% depression of the twitch. Older infants (Group 2), 188 +/- 33 days, required sign…

Diazepam and succinylcholine chloride for restraint of the American alligator.

Diazepam followed by succinylcholine chloride was administered intramuscularly to 26 healthy mature female alligators on 2 occasions. The mean diazepam dosage was 0.37 mg/kg (range, 0.22 mg/kg to 0.62 mg/kg) and the mean succinylcholine chloride dosage was 0.24 mg/kg (range, 0.14 mg/kg to 0.37 mg/kg). This drug combination reduced stress and allowed adequate immobilization for restraint and handling. The reduced drug volume, low dosage of succinylcholine chloride required, short induction period, maintenance of respiration, and adequate degree of immobilization make this drug combination a goo…

Increased activation effects of succinylcholine in neurological patients.

Succinylcholine, a depolarizing muscle relaxant with both activating and desensitizing effects, is used to facilitate endotracheal intubation. The activating effects were found to be above-normal on induction of anesthesia in 7 neurological patients: generalized muscle spasm in 1 myotonic patient, contractures or prolonged contractions in "anatomically" denervated muscles (1 patient), in "functionally" denervated muscles (1 patient) and in "centrally" denervated muscles (4 patients). One of these four presented hyperkalemia and cardiocirculatory collapse. It is important to differentiate these…

Intravenous flunitrazepam in the prevention of the side effects of succinylcholine.

Muscular fasciculations, plasma potassium levels and postoperative muscle pains following succinylcholine injection were studied in 64 patients who had been given either dilute flunitrazepam or sodium thiopental to induce anesthesia. In the 32 patients with flunitrazepam-induced anesthesia, muscle fasciculations were present in only 8 and postoperative muscle pains in 4, while none of the patients had elevated plasma potassium. In the thiopental group, muscle fasciculations and postoperative muscle pains were present in 28 and 22 of the other 32 patients, respectively, while all had significan…

Postanesthetic skin erythema due to succinylcholine versus atracurium.

Intraoperative anaphylactic reactions may range from mild, erythema-like to anaphylactic shock, with tension crash and bronchospasm. The sub … of intubation, and non-depolarizing myorelaxants such as atracurium, cisatracurium and rocuronium. Succinylcholine has a short time of action and it is rapidly metabolized. Atracurium, although having a slightly longer time to action, has the benefit of a low risk of increased levels of potassium in blood, which is extremely important in patients with cardiac pa …

Survival following intentional succinylcholine injection for self-harm.

Succinylcholine is a short-acting depolarizing neuromuscular blocking agent. We describe a case where the above drug was employed for self-harm by a health-care worker. The patient, a 28-year-old female, was brought to the emergency department (ED) in impending respiratory arrest and altered mental status. On arrival, she had hypoxia, bradycardia, and hypotension. Although the cause for rapid deterioration in this patient was unknown, the ED physician still went ahead by resuscitating the patient's airway, breathing, and circulation. During the course of resuscitation, information was received…

Succinylcholine-induced cardiac arrest in children with undiagnosed myopathy.

Two paediatric cases are reported in which unexpected, life-threatening arrhythmias occurred. Routine induction of general anaesthesia with thiopentone, 5 mg.kg-1, in one and with halothane in the other, and succinylcholine 1.25-1.5 mg.kg-1 i.v. was followed by the development of wide complex tachyarrhythmia with hypotension in the first case and asystole in the second case despite pre-treatment with atropine in both cases. The first patient was resuscitated with tracheal intubation, 100% oxygen, manual ventilation and intravenous lidocaine and bicarbonate. The second patient required intubati…

Succinylcholine-induced cardiac arrest in unsuspected Duchenne muscular dystrophy.

A case history is presented of a three-year-old boy with unsuspected Duchenne muscular dystrophy, who suffered a cardiac arrest following the administration of a single dose of succinylcholine during a halothane anaesthetic. The arrest was associated with lack of fasciculations, muscle rigidity, hyperkalemia, myoglobinuria, and massive elevation of serum creatine phosphokinase. Asystole was prolonged and refractory to treatment, although cardiac activity was eventually restored. The possible cause of the circulatory collapse is discussed and reports of similar cases reviewed. Neither succinylc…
