Patients with mycetoma usually present late with advanced disease, which is attributed to lack of medical and health facilities in endemic areas, poor health education and low socio-economic status. This is a cross-sectional molecular epidemiological study on equine piroplasmosis (EP) affecting horses and donkeys in the Sudan. The study evaluated 499 samples from geographically distinct regions in eastern, central and western parts of the country. PCR amplification of the 18S rRNA gene of both Thelieria equi and Babesia caballi was carried out. Horses from all sampled areas were found positive to T. equi DNA but no B. caballi was detected. Onchocerciasis is endemic in southern Sudan. The prevalence in human population may range from 10% to 40%. In the Eastern Equatorial Region of Southern Sudan, more cases are found in Nimule on the Sudan-Uganda border than in rural highlands of Labone and Chukudum basins. The first authentic case of African histoplasmosis from the Sudan is reported. The patient came from Dindir, along the southern part of the Blue Nile River. He had cutaneous and visceral leishmaniasis as well as the cutaneous form of African histoplasmosis. The latter was proved by culture and histopathology to be due to Histoplasma duboisii. The fungus was confined to the skin. H. duboisii may be more prevalent, and further studies in Dindir area are needed.

A metabolite of Sudan I (1-phenylazo-2-naphthol) was isolated after in vitro incubation with the supernatant of guinea pig liver homogenate (S-9). The metabolite was found to be 4'-hydroxy-1-phenylazo-2-naphthol by gas chromatography-mass spectrometry (GC-MS), and proton and carbon-13 nuclear magnetic resonance (NMR) analyses. It elicited positive reactions in guinea pigs sensitized to Sudan I. It was also shown to be allergenic. The results suggest that para-hydroxylation of the phenyl group of Sudan I may play an important rôle in its allergenicity.

426 4- 17-year-old children (89% males) from the Jonglei Province and 50 12- and 15-year-old children from Juba town in Southern Sudan were examined according to WHO criteria (9). An infectious keratoconjunctivitis occurring in goats in the Sudan is described. Experimental transmission using infective ocular discharges resulted in a conjunctivitis in young kids and a severe keratoconjunctivitis in an adult goat. A pleomorphic organism which resembled Rickettsia spp was found in conjunctival smears from naturally and experimentally infected animals. Neither Mycoplasma nor Chlamydia spp were isolated in microbiological cultures.

This paper describes the first epidemic of cutaneous leishmaniasis to be reported from the Sudan. The epidemic occurred in villages in an area extending for about 100 km along the banks of the Nile between Shendi and Atbara. The disease affected people of all ages and the skin lesions, which were nodular or nodulo-ulcerative, were usually multiple.
